The controlled treatment outcome studies that examined the efficacy of EMDR in the treatment of posttraumatic stress disorder have yielded a range of results, with the efficacy of EMDR varying across studies. The current study sought to determine if differences in outcome were related to methodological differences. The research was reviewed to identify methodological strengths, weaknesses, and empirical findings. The relationships between effect size and methodology ratings were examined, using the Gold Standard (GS) Scale (adapted from Foa & Meadows, 1997). Results indicated a significant relationship between scores on the GS Scale and effect size, with more rigorous studies according to the GS Scale reporting larger effect sizes. There was also a significant correlation between effect size and treatment fidelity. Additional methodological components not detected by the GS Scale were identified, and suggestions were made for a Revised GS Scale. We conclude by noting that methodological rigor removes noise and thereby decreases error measurement, allowing for the more accurate detection of true treatment effects in EMDR studies. 相似文献

A case of sclerosing epithelioid fibrosarcoma and its appearance on MRI is presented. The tumor showed a zonal architecture
on MRI with a large central core of very low signal intensity and a peripheral rim of intermediate to high signal intensity
on T1- and T2-weighted spin echo pulse sequences. The core showed decreased cellularity with dense collagen deposition on
histologic examination, and the peripheral zone increased cellularity with increased nuclear atypia. The presence of a prominent
region of very low signal intensity on T1- and T2-weighted images can be seen with neural tumors, giant cell tumor of the

PURPOSE: Studies have shown that medical students become more cynical and less altruistic as they advance in training. However, these studies were conducted in traditional medical schools, and many used unvalidated tools. This study examined students' attitudes in a problem-based learning (PBL) curriculum using reliable and valid measures. METHOD: Medical students and PGY-1 residents at Mercer University School of Medicine in Macon, Georgia, completed Wrightsman's Philosophies of Human Natures Scale (PHNS) in 1999 and 2000. Chronbach's alpha assessed internal reliability among subscales, and test-retest reliability coefficients confirmed acceptable reliability. For 114 students who completed both surveys, changes in PHNS scores were analyzed, with particular attention to the subscales of trustworthiness, altruism, and cynicism. RESULTS: Students assessed at the beginning of their second year increased the extent to which they believed people are trustworthy and increased their beliefs in how altruistic people are. They also showed a significant decrease in cynicism. There was not a significant change in trustworthiness, altruism, or cynicism among the participants beyond first year. In general, female students held less cynical views about others and believed people to be more trustworthy. CONCLUSIONS: Contrary to prior reports, this study found that more advanced trainees were not more cynical or less altruistic than their more junior counterparts. Indeed, a significant and positive change of attitudes among the participants during their first year of medical school refuted earlier reports. Thus, results of earlier studies and the effect of a PBL curriculum on attitudes of medical students need to be re-examined. 相似文献

The authors examined the efficacy, speed, and incidence of symptom worsening for 3 treatments of posttraumatic stress disorder (PTSD): prolonged exposure, relaxation training, or eye movement desensitization and reprocessing (EMDR; N = 60). Treaments did not differ in attrition, in the incidence of symptom worsening, or in their effects on numbing and hyperarousal symptoms. Compared with EMDR and relaxation training, exposure therapy (a) produced significantly larger reductions in avoidance and reexperiencing symptoms, (b) tended to be faster at reducing avoidance, and (c) tended to yield a greater proportion of participants who no longer met criteria for PTSD after treatment. This study investigated hemisphere differences in sensitivity to orthographic and phonological similarity using a task that did not require deliberate metalinguistic comparisons between words. Two experiments investigated the influence of an unattended distractor item on the pronunciation of target words in the right visual field (RVF) and left visual field (LVF) in neurologically intact persons. Word and pseudoword distractors that were both orthographically and phonologically similar to the target word produced equivalent facilitation across visual fields (Exp. 1). When orthographic and phonological influences were separated in Exp. 2, each dimension produced reliable facilitation, and to the same extent in each visual field. These results, and others in the literature, are difficult to reconcile with the view that the intact right hemisphere is completely unable to access phonology from print. If subsequent research confirms these findings, it would suggest that passive activation of phonology in reading can be dissociated from articulatory mechanisms, and that left hemisphere superiority in some phonological judgements may depend more on the availability of articulatory rehearsal than on privileged access to phonological codes. 相似文献
