Intramuscular ziprasidone treatment of acute psychotic agitation in elderly patients with schizophrenia.

OBJECTIVE: Intramuscular (i.m.) ziprasidone treatment has been shown to be effective and well tolerated in reducing the symptoms of acute psychosis in adults. Few data are available as to safety in the elderly. The growing utilization of health services by elderly psychiatric patients warrants an evaluation in this population. METHOD: Consecutive elderly patients (60 years of and older) admitted to a psychogeriatric ward in a large, university-affiliated tertiary psychiatric center were treated by i.m. ziprasidone for acute psychotic agitation. Patients received three days of flexible-dose i.m. ziprasidone. After an initial dose of 10-20 mg, a subsequent dose of 10-20 mg could be given after 12 hours if needed (maximum daily dose: 40 mg). RESULTS: All treatment emergent side effects and adverse events along with the investigators' assessments of severity were systematically recorded as the primary outcome. The Brief Psychiatric Rating Scale (BPRS) and the Behavioral Activity Rating Scale (BARS) were the secondary outcomes. Twenty-one patients, six male and 15 female, mean age 71.4 +/- 1.3 years (range: 60-81 years) were enrolled. All had completed the three days i.m. ziprasidone treatment. There was one adverse event in a patient with untreated benign prostatic hypertrophy who developed urinary retention. Two side effects of mild severity that resolved spontaneously were observed: blurred vision and sedation. The BPRS decreased by 26.8 points after three days of treatment (p = 0.001). The BARS score, reflecting agitation, decreased significantly after each injection, reaching maximal decrease of 2.14 points at completion of study (p = 0.001). CONCLUSION: Intramuscular ziprasidone in this series of elderly patients suggests acceptable safety and efficacy in the management of acute psychotic agitation among elderly patients with schizophrenia.     

多级图像对比度放大技术在膝关节摄影中的应用

目的:分析评价膝关节摄影中多级图像对比度放大技术(MUSICA)参数设定的成像效果,为实际临床应用提供理论指导。方法:随机抽取膝关节侧位软拷贝图像70例,以骨皮质、骨小梁、肌间隙、髌上囊、皮下脂肪为比对目标,由三位观察者对其显示情况进行分析,并对结果进行统计分析。结果:MUSICA处于较小值(0～2)时,适合于软组织显示,但图像锐利度欠缺;处于较高值时(4～6)适合于观察骨皮质、小梁等细节信息,但较多地出现伪影,共25例;处于2～4时整体影像对比度适中,如实反映人体密度结构。结论:作图像处理时应将MUSICA为2～4设定为常规,实际应用通常情况下可以选择该处理方法,但应根据具体要求适当调整MUSICA参数值。     

还原型谷胱甘肽降低糖尿病大鼠心肌NF-κB和诱导型一氧化氮合酶表达

以链脲佐菌素制备糖尿病大鼠模型，造模后以还原型谷胱甘肽（GSH）治疗12周，结果显示GSH治疗组较糖尿病组心肌组织病理学损害改善，NF—κB活性降低，诱导型一氧化氮合酶表达下降。     

Symmetrical interdigital keratoderma of the hands

We report the case of a 26-year-old man affected by a symmetrical keratoderma localized to the interdigital spaces of the fingers. No occupational, traumatic, or irritant factors were discovered. Clinical and histological features were consistent with the diagnosis of symmetrical interdigital hyperkeratosis, a sporadic disorder described by Frei in 1926. We believe this condition to be less rare than the few cases reported in the literature would suggest.

Closed flexible intramedullary nailing of adolescent femoral shaft fractures

Twenty-five femoral shaft fractures in twenty-three patients aged 10-16 years with open epiphyses and treated with flexible intramedullary nailing were studied retrospectively. Clinical and roentgenographic follow-up averaged 28 months. Hospitalization, which included patients with multiple injuries, averaged 11.7 days. Patients ambulated without assist devices as early as 2 weeks postoperatively (average of 7.7 weeks). All fractures healed with no leg length inequality--21 with anatomic alignment and full range of motion of the hip and knee. Three patients sustained intraoperative extension of the fracture resulting in healing with angular or rotatory malalignment. One patient with associated knee ligamentous injury had less than full knee flexion at follow-up. All patients had normal gait (except one patient with residual hemiplegia) and were able to participate in full activities including athletics. Early ambulation and functional recovery with low morbidity and cost suggest that this procedure should be considered for the treatment of femoral shaft fractures in this age group.     

Postural stability by computerized posturography in minor head trauma

Mild head injuries are very common among young children. Often, these injuries are followed by a variety of subjective complaints termed posttraumatic syndrome. Posturography (balance test) was performed immediately after the trauma in 21 children who had sustained mild head injury. Significant difference in performance was observed in head-injured children in all subparts of the test as compared with a control group. We conclude that posturography may serve as a simple cost-effective method in qualifying the posttraumatic imbalance.
