Background

Following the Nepal earthquake in April 2015, UNFPA estimated that 1.4 million women of reproductive age were affected, with approximately 93,000 pregnant and 28,000 at risk of sexual violence. A set of priority reproductive health (RH) actions, the Minimum Initial Services Package (MISP), was initiated by government, international and local actors. The purpose of this study was to identify the facilitators and barriers affecting the implementation of priority RH services in two districts.

Methods

In September 2015, a mixed methods study design was used in Kathmandu and Sindhupalchowk districts to assess the implementation of the priority RH services five months post-earthquake. Data collection activities included 32 focus group discussions with male and female participants aged 18–49; 26 key informant interviews with RH, gender-based violence (GBV), and human immunodeficiency virus (HIV) experts; and 17 health facility assessments.

Results

The implementation of priority RH services was achieved in both districts. In Kathmandu implementation of emergency RH services started within days of the earthquake. Facilitating factors for successful implementation included disaster preparedness; leadership and commitment among national, international, and district level actors; resource mobilization; strong national level coordination; existing reproductive and child health services and community outreach programs; and supply chain management. Barriers included inadequate MISP training for RH coordinators and managers; weak communication between national and district level stakeholders; inadequate staffing; under-resourced and fewer facilities in rural areas; limited attention given to local GBV and HIV organizations; low availability of clinical management of rape services; and low awareness of GBV services and benefits of timely care.

Conclusion

Ensuring RH is included in emergency preparedness and immediate response efforts and is continued through the transition to comprehensive care is critical for national governments and humanitarian response agencies. The MISP for RH remains a critical component of response efforts, and the humanitarian community should consider these learnings in future emergency response.

Rationale:Bacterial and fungal infections in Coronavirus Disease-19 (COVID-19) patients have been inadequately investigated and reported thus far. The safety profile of tocilizumab (TCZ) administration in candidemia patient still debatable.Patient concerns:A 54 year-old woman presenting with weakness on the left side of her body was diagnosed with COVID-19. After 7 days of admission, her condition worsened and developed respiratory distress and was having respiratory distress despite standard treatment.Diagnoses:Acute respiratory distress syndrome (ARDS) in COVID 19 was diagnoses based on real time-PCR swab, deterioration of PaO2/FiO2 and increased of acute phase reactants.Interventions:Anti Interleukin–6 (IL-6) was considered to tackle her inflammatory condition. Prior to TCZ administration, blood culture was performed and the result came with Candida tropicalis in the absence of bacterial growth.Outcomes:No major complications associated with intravenous antifungal or TCZ occurred. After 40 days of hospitalization, the patient''s clinical condition improved and was finally discharged.Lessons:This case underscores the safety profile of giving TCZ in candidemia as a secondary infection in severe COVID-19 patient.  相似文献

Cancer therapy is a strategic measure in inhibiting breast cancer stem cell (BCSC) pathways. Naringenin, a citrus flavonoid, was found to increase breast cancer cells’ sensitivity to chemotherapeutic agents. Bioinformatics study and 3D tumorsphere in vitro modeling in breast cancer (mammosphere) were used in this study, which aims to explore the potential therapeutic targets of naringenin (PTTNs) in inhibiting BCSCs. Bioinformatic analyses identified direct target proteins (DTPs), indirect target proteins (ITPs), naringenin-mediated proteins (NMPs), BCSC regulatory genes, and PTTNs. The PTTNs were further analyzed for gene ontology, Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment, protein–protein interaction (PPI) networks, and hub protein selection. Mammospheres were cultured in serum-free media. The effects of naringenin were measured by MTT-based cytotoxicity, mammosphere forming potential (MFP), colony formation, scratch wound-healing assay, and flow cytometry-based cell cycle analyses and apoptosis assays. Gene expression analysis was performed using real-time quantitative polymerase chain reaction (q-RT PCR). Bioinformatics analysis revealed p53 and estrogen receptor alpha (ERα) as PTTNs, and KEGG pathway enrichment analysis revealed that TGF-ß and Wnt/ß-catenin pathways are regulated by PTTNs. Naringenin demonstrated cytotoxicity and inhibited mammosphere and colony formation, migration, and epithelial to mesenchymal transition in the mammosphere. The mRNA of tumor suppressors P53 and ERα were downregulated in the mammosphere, but were significantly upregulated upon naringenin treatment. By modulating the P53 and ERα mRNA, naringenin has the potential of inhibiting BCSCs. Further studies on the molecular mechanism and formulation of naringenin in BCSCs would be beneficial for its development as a BCSC-targeting drug.  相似文献

Syk, a non-receptor tyrosine kinase, is an essential signaling molecule in B cells and other hematopoietic cells. Recently, its unexpected diverse functions were recognized in the regulation of cellular adhesion, innate immune recognition, vascular development, and carcinogenesis. Despite its pleiotropic role, only a few substrate proteins have been identified. To find new substrate proteins for Syk, we performed a systemic in vitro kinase assay using GST fusion peptides to determine the substrate specificity surrounding the tyrosine residue to be phosphorylated. Substitution of amino acid residues surrounding tyrosine 178 of BLNK, a principal Syk substrate in B cell receptor-mediated signaling, revealed that acidic residues at sites -5 to -1 were necessary for phosphorylation by Syk. Valine at site +1 was also influential in phosphorylation and a substitution of Pro on site +3 to a basic amino acid residue, Lys, resulted in attenuated phosphorylation. On the basis of these results, a general consensus phosphorylation motif for Syk was determined and several new candidate target proteins were identified in protein database searches. Of the candidate proteins, BCAR-1 (breast cancer anti-estrogen resistance 1) was confirmed to be phosphorylated by Syk in an in vitro kinase assay using a full-length protein of BCAR-1. Furthermore, BCAR-1 was tyrosine phosphorylated upon the overexpression of Syk in HEK-293T cells. These results suggest that more Syk substrates can be found using an in vitro kinase approach and show for the first time that BCAR-1 is a physiological substrate of Syk.  相似文献

The aim of this study was to evaluate the role of three polymorphisms (D543N, TGTG deletion in 3' UTR, INT4 G/C) of NRAMP1 to susceptibility to lung tuberculosis (TB) disease. The results showed that homozygous TGTG deletion in 3'UTR of NRAMP1 was found more frequent in lung tuberculosis patients than in healthy nurses working in a TB ward. This 4 base deletion might cause malfunction of NRAMP1 protein so that it fails as Fe2+ ion transporter causing macrophage unable to eliminate Mycobacterium tuberculosis.  相似文献

A multistep and high-cost biodiesel production could be simplified using the direct transesterification (DT) method. A controllable device has been developed and applied to study the effects of the ratio of rubber seeds to methanol, catalyst concentration, reaction time and rotational speed on biodiesel conversion and fatty acid (FA) yield extraction. The controllable crushing device (CCD) assisted the DT of rubber seeds and operated at ambient temperature and pressure achieved a maximum biodiesel conversion of 97.5 ± 0.6% in a reaction time of 7 minutes. The biodiesel quality estimated based on the FA extraction profile is comparable with the biodiesel standard. In terms of energy efficiency and reaction time, the CCD saved 71–98% energy consumption and reduced the reaction time up to 99%.

Objective

Stress is one of the responses experienced by families with child victims of sexual violence. The purpose of this study was to explore in depth the stress experienced by families dealing with child victims of sexual violence.

Method

This qualitative study employed the phenomenology approach. A purposive sample of six families participated in the study. Data were analyzed using Colaizzi's method.

Results

This study discussed the experience of family with children who are victims of sexual violence. This study generated the following 5 themes: 1) sexual violence and the subsequent behavior changes in children as a source of family stress; 2) Family stress as a response to changes in the family process; 3) social support as sources of the family's strengths; 4) spiritual activities for coping with stress, and 5) parenting changes as a family learning and evaluation.

Conclusions

Results suggested the development of the School Mental Health Unit to help families, and the community, identify and prevent sexual violence. In addition, the School Mental Health Unit would be a useful source for students who experience sexual violence to function well at school.  相似文献
