Experience in hemoconcentration by ultrafiltration in operations using artificial circulation at the All-Union Scientific Center of Surgery of the USSR Academy of Medical Sciences

The article deals with the results of using ultrafiltration hemoconcentration in 649 operations conducted on the open heart with extracorporeal circulation at the All-Union Scientific Center of Surgery, USSR AMS during 1983-1989. The method allows control of hemodilution and colloid-osmotic pressure of plasma during the indicated interventions, lessens the loss of blood, and reduces the expenditure of donor blood.     

Intraoperative dilatation of the ilio-femoral segment combined with reconstructive operations on the femoral arteries in "multi-level" lesions in patients from the risk group

The authors analyse experience in the treatment of 96 patients at an increased risk of intervention. Ischemia of the lower extremities with relatively maintained peripheral vascular bed was the main criterion of an indicated operation. The malleolar systolic pressure index of at least 0.35 +/- 0.15 was considered essential in this case. With consideration for the risk, the interventions were carried out mainly under peridural anesthesia. Intraoperative dilatation of the iliac arteries was conducted under radiotelecontrol in combination with profundoplasty, thrombendarterectomy from the femoral arteries, and femoropopliteal autovenoplasty. The efficacy of the operations was appraised by intraoperative angiography, electromanometry, and blood flow measurement. The results were studied in follow-up periods of up to 3 years. In the group of 96 patients 8 had poor results. Amputation of the extremity was performed in different periods.

Neural Correlates of the Results-of-Action Acceptor in a Functional Biotechnical Complex

This report presents data illustrating the neurophysiological features of efferent-afferent convergence on cortical neurons. During combination of stimulus of pyramidal tract axons with electrocutaneous reinforcement, some of the study neurons were found to change the parameters of their activity evoked by stimulation of this tract: evoked responses started to reproduce the structure of responses to the reinforcement. The most dynamic rearrangements of pyramidal tract responses were obtained in experiments in which the study neurons were included in a biotechnical complex with feedback, the complex consisting of neuron-computer-stimulator-animal and actually being an analog of a natural functional system. The role of efferent-afferent convergence on CNS neurons in the development of the results-of-action acceptor of a functional system for a voluntary behavioral act is discussed.     

Effect of autologous sera on mitogenic stimulation of peripheral blood lymphocytes of mini-pigs chronically intoxicated with alcohol

There are no statistically significant differences in the stimulation index for purified lymphocytes of control mini-pigs and mini-pigs with chronic alcohol intoxication. Autologous sera of control and experimental animals strongly suppress mitogen-induced blast transformation of lymphocytes without death of these cells. There are no statistically significant intergroup differences in the absolute number (per mm³) of T lymphocytes in peripheral blood. Stress-Doppler echocardiography in the diagnosis of ischemic heart disease in an ambulatory setting]

Coronary arteriography, bicycle ergometry and transesophageal atrial pacing (TAP) in combination with Doppler echocardiography (stress-Doppler echocardiography) were used to evaluate cardialgias in 30 outpatients. Stress-Doppler echocardiography showed a high (94%) sensitivity and a high (86%) specificity to detect coronary heart disease (CHD). There was a high correlation (r = 0.79, p less than 0.001) between the wall motion values obtained by echocardiography during TAP and coronary rating. The left ventricular (LV) diastolic filling (DF) was studied by pulsed wave Doppler echocardiography in the postpacing period. The Doppler-derived parameters of LV DF obtained in that period appeared to be moderately sensitive (75%) and specific (64%) in detecting CHD. In post-pacing ischemia, the "pseudonormalized" LV filling pattern was observed in 5 of 6 patients (sensitivity 80%) having three-vessel disease and major left (or equivalent) coronary stenosis.
