Objective
To evaluate the safety of adult tonsillectomy as a day care surgeryMethods
Records of 664 patients above the age of 12 years, who underwent tonsillectomy were analysed. These patients were operated in our department within January 1999 and September 2002. Only 13 patients (1.95%) had post operative bleeding and were readmitted after discharge. The data of these patients were collected from medical records as well from the morbidity meeting proceedings and was reviewed retrospectively. Statistical analysis was done by using ‘one sample t-test’.Results
Overall incidence of post operative bleeding was 1.95%. None of our pateients bled with 12 hours of Surgery, 0.3% had bleeding between 12 to 24 hours after tonsillectomy and 1.65% encountered bleeding between 24 hours to the 6th post operative day. Bleeding was not major in majority of patients as only 0.45% of the patients required second anesthesia to control the bleeding and only 0.35% of patients required blood transfusion to replace the lost blood volume. In addition, higher incidence of bleeding was seen in males, in the older age group of 31–40 years, patients with obstructive sleep aponea and in patients where tonsillectomy was done by electrocautry dissection.Conclusion
Based on these observations, we conclude that Adult tonsillectomy can be safely preformed in our setup. 相似文献5.

miRNAs are small noncoding RNAs with gene regulatory functions, frequently dysregulated in human cancers. Specific signatures of differentially expressed miRNAs can be used in the diagnosis of cancer and in some cases harbor prognostic implications. The biology of cancer is dictated not only by cancer cells but also by the surrounding tumor microenvironment. In particular, the role of miRNAs within the tumor microenvironment is emerging as of paramount importance. This review will focus on the current knowledge of the role of miRNAs and both cellular and stromal components of the tumor microenvironment. We will also discuss more recent findings, showing that miRNAs can be found inside of exosomes and mediate the cross-talk between cancer cells and surrounding cells, leading to the discovery of new fascinating molecular mechanisms leading to a better understanding of the cancer ‘niche’ and how these noncoding RNAs can become very promising diagnostic molecules. 相似文献

The aims of this study were to find the incidence of bifurcation of the inferior dental nerve (IDN) canal, to describe the characteristics of this variant, and to examine the sensitivity and specificity of dental panoramic tomography to identify it. We classified bifurcations by size and position relative to the main canal and the lower third molar using cone-beam computed tomography (CT) and dental panoramic tomography. In our study of 281 patients, 106 (38%) had bifurcations, and in one quarter, these were classified as large accessory canals. Bifurcations were most commonly found posterior to the lower third molar (n = 64, 57%) or within 2 mm of the roots of the third molar (n = 40, 38%). The sensitivity and specificity of dental panoramic tomography to identify all bifurcations was 11% (95% CI: 5.67 to 17.97) and 91% (95% CI: 85.58 to 94.68), respectively; this was 33% (95% CI: 15.63 to 55.32) and 94% (95% CI: 90.34 to 96.50), respectively, for large bifurcations. Our use of cone-beam CT suggested an incidence of bifid canals of 38%, with a variation in size and distribution in relation to the lower third molar. Historically, renal involvement has not been a commonly recognized complication in patients with β-thalassemia major (β-TM). Herein, we studied the impact of iron overload on glomerular filtration rate (GFR) estimated by cystatin C based GFR (Cyst C eGFR). We enrolled 149 patients with β-TM in a cross sectional study in a single center in Oman. We investigated the correlation between measurement of serum ferritin and Cyst C eGFR. We used univariable linear regression to study the impact of serum ferritin on Cyst C eGFR and backwards stepwise regression to adjust for potential confounders. We included 78 males and 71 females with a mean age of 17.3?±?9 years (range 2.5–38.5). Seventeen patients had diabetes mellitus. Patients were taking deferiprone (DFP) and deferoxamine (DFO) (26 patients), DFP (58 patients), deferasirox (DFX) (62 patients) and one patient was taking only DFO. There was a very weak negative linear relationship between serum ferritin and Cyst C eGFR (correlation coefficient ?0.25). In the univariable analyses, serum ferritin (p?=?0.004), diabetes status (p?0.001) and chelation therapy (p?0.001) were statistically significant. In the multivariable model, age (p?=?0.033), chelation with DFX (p?=?0.05) and diabetes status (p?0.001) were statistically significant. We found a very weak inverse linear correlation between serum ferritin and Cyst C eGFR. However, when concomitant use of chelation therapy was considered, serum ferritin did not associate with glomerular function. Prospective and larger studies are needed to confirm these findings. 相似文献
