Analysis of high frequency and low amplitude late potentials in the body surface electrocardiogram, in acute and chronic stages of ischemic cardiopathy

A study to evaluate late potentials (LP) was carried out in 50 patients diagnosed as suffering from coronary heart disease. All the patients were divided into two groups: Group A, was made of 25 patients in the acute period of myocardial infarction, and group B, was made of other 25 patients in the chronic phase, with more of 3 months of evolution from the acute event. Average age in group A was 54.2 years, and 58.1 years in group B. Twenty normal volunteer subjects (group C) were also studied as a reference group. All the subjects in the 3 groups were in normal sinus rhythm, and no one of them showed bundle branch block in the basal ECG. Analysis of the vector magnitude was made for each point of the averaged wave, as the result of the mean root square of the addition of bipolar leads X, Y and Z, using a LP-3000 Fidelity Medical Inc. equipment, with a four poles Butterworth filter. LP were detected in 32% of the patients in group A, and in 56% of the group B (p less than 0.001). No LP was observed in patients of the group C. Concerning the localization of myocardial infarction, maximum CPK value, or Lown's grade of ventricular arrhythmias observed in coronary care unit, or in the 3rd week Holter monitoring, did not show significant statistical correlationship with the presence of LP in group A.(ABSTRACT TRUNCATED AT 250 WORDS)     

Costs of high-dose chemotherapy and peripheral blood progenitor cell autograft for breast cancer.

The aim of the study was to analyze the real cost of single or tandem high-dose chemotherapy (HDC) and peripheral blood progenitor cell autologous transplant (PBPCT) in patients with breast cancer. We analyzed the costs of 40 PBPCT performed in 20 patients. Tandem transplant was planned for each patient. Resources used and direct costs were identified for each patient. The study was carried out using the hospital perspective and monetary values were reported in 1999 Euro. The mean cost of whole procedure for single transplant was 20,816.63 Euro, while the mean cost of tandem transplant was 38,770.83 Euro. The cost distribution in the two groups was similar: the most expensive phase of procedure was the supportive phase post transplant (about 60% of total cost), with the categories of cost most represented being professional fees (about 28%) and pharmacy (about 35%). Awaiting more convincing trials of the clinical advantage of HDC in breast cancer, our analytical evaluation of transplant costs for different therapeutic options, single or tandem, permits identification of the most expensive categories in order to intervene for cost savings.     

Defibrotide affects the anticoagulant and lipase-releasing activities of heparin

Defibrotide (DEF), an antithrombotic drug with no anticoagulant activity, given concomitantly with heparin potentiates its effects on thrombin time and lipase-releasing activity. Unlike heparin, DEF has hardly any lipase-releasing activity of its own. Most likely DEF interferes with pharmacokinetics of heparin. It is clear from the data reported here that when used in combination with DEF heparin should be administered at low doses and the coagulation parameters carefully followed.     

ACCESS TO NEW INFORMATION AND COMMUNICATION TECHNOLOGIES AMONG HOMELESS PEOPLE IN MADRID (SPAIN)

The article analyzes access to various information and communication technologies (ICTs; mobile phone, computer, Internet, e‐mail, and social network sites) among a representative sample of homeless people in Madrid. The information was collected using a structured interview. The results show that new ICTs are used to some extent among homeless people in Madrid, although at much lower percentages than the general population in Spain. We observed a relatively widespread use of mobile phones, a medium‐low level use of the Internet, computer, and e‐mail, and hardly any use of social networks, with significant differences according to the interviewees' age, level of education, and Spanish or foreign nationality. The digital divide has an especially negative effect on elderly homeless people, those with lower levels of education, and those of Spanish origin.     

Absorption through the peritoneum of the macromolecular profibrinolytic drug defibrotide in the rabbit.

Defibrotide is the sodium salt of a polydeoxyribonucleotide extracted from mammalian lungs by controlled depolymerization. This macromolecular compound (mean molecular weight 20,000 d) has profibrinolytic activity. It also is antithrombotic after both intravenous and oral administration. The purpose of this study was to see whether defibrotide could activate the fibrinolytic system after intraperitoneal administration, because it has been observed that in ascitic patients treated intraperitoneally with defibrotide, no occlusion of the peritoneal-jugular catheter valve draining the ascitic fluid (according to LeVeen et al.) occurred. The profibrinolytic activity of defibrotide and its plasma levels were dose dependent, with a statistically significant correlation between the fibrinolytic activities and the AUC of the plasma levels of defibrotide. No rebound effect was observed during the duration of the experiments (24 hours). We hypothesize that the fibrinolytic activity elicited by defibrotide prevented the deposition of fibrin in the catheter valve, enabling normal drainage of the ascitic fluid. The lack of rebound makes defibrotide a safe drug. The mechanism by which this macromolecular compound is absorbed is still unknown.

Animal movement and establishment of vaccinia virus Cantagalo strain in Amazon biome, Brazil

To understand the emergence of vaccinia virus Cantagalo strain in the Amazon biome of Brazil, during 2008-2010 we conducted a molecular and epidemiologic survey of poxvirus outbreaks. Data indicate that animal movement was the major cause of virus dissemination within Rondonia State, leading to the establishment and spread of this pathogen.     

Antioxidant administration prevents memory impairment in an animal model of maple syrup urine disease

Maple syrup urine disease (MSUD) is an autosomal recessive metabolic disorder resulting from deficiency of branched-chain α-keto acid dehydrogenase complex leading to branched chain amino acids (BCAA) leucine, isoleucine, and valine accumulation as well as their corresponding transaminated branched-chain α-keto acids. MSUD patients present neurological dysfunction and cognitive impairment. Here, we investigated whether acute and chronic administration of a BCAA pool causes impairment of acquisition and retention of avoidance memory in young rats. We have used two administration protocols. Acute administration consisted of three subcutaneous administrations of the BCAA pool (15.8 μL/g body weight at 1-h intervals) containing 190 mmol/L leucine, 59 mmol/L isoleucine, and 69 mmol/L valine or saline solution (0.85% NaCl; control group) in 30 days old Wistar rats. Chronic administration consisted of two subcutaneous administrations of BCAA pool for 21 days in 7 days old Wistar rats. N-acetylcysteine (NAC; 20 mg/kg) and deferoxamine (DFX; 20 mg/kg) co administration influence on behavioral parameters after chronic BCAA administration was also investigated. BCAA administration induced long-term memory impairment in the inhibitory avoidance and CMIA (continuous multiple-trials step-down inhibitory avoidance) tasks whereas with no alterations in CMIA retention memory. Inhibitory avoidance alterations were prevented by NAC and DFX. BCAA administration did not impair the neuropsychiatric state, muscle tone and strength, and autonomous function evaluated with the SHIRPA (SmithKline/Harwell/ImperialCollege/RoyalHospital/Phenotype Assessment) protocol. Taken together, our results indicate that alterations of motor activity or emotionality probably did not contribute to memory impairment after BCAA administration and NAC and DFX effects suggest that cognition impairment after BCAA administration may be caused by oxidative brain damage.
