Echinococcal infection of the heart is rare. The preoperativefindings, with special attention to two-dimensional echocardiography(2DE), of six patients with cardiac echinococcosis and theirsurgical and pathological correlations are reported. Cardiachydatid cysts (HCs) were located in the cardiac apex in threepatients, in the upper part of the interventricular septum extendingtowards the anterior aspect of the heart in one and in the postero-superiorright atrial wall in another patient. The remaining patienthad multiple intrapericardial cysts. In three patients the cystspresented as well defined, rounded, echolucent masses withinthe myocardial wall bulging into the cardiac chambers. In twopatients, the cysts had ruptured into a cardiac chamber withloss of the characteristic ‘cystic’ appearance;these cysts presented as an echogenic or solid mass protrudinginto a cardiac chamber. Finally, another patient had one HCwith echolucent appearance and another HC in a different locationwith echogenic appearance; this last cyst corresponded to adegenerated HC. In two cases the cyst showed a loculated internalaspect. In one patient the myocardial segment involved by thecyst had a dyskinetic movement. In all six patients, 2DE accuratelydemonstrated the location and morphological details of the cardiaccysts, permitted recognition of the ruptured and/or degeneratedcysts and was superior to thoracic computed tomography and angiographyin the preoperative assessment of these patients. Nuclear magneticresonance imaging (one patient) gave no further informationto that obtained by 2DE. We conclude that 2DE is the techniqueof choice for an early diagnosis of this rare entity.  相似文献

Cardiac tamponade complicating catheter ablation of atrial fibrillation (AF) occurs in approximately 1% of pulmonary vein isolation (PVI), and up to 6% of linear ablation procedures. We reviewed 348 consecutive AF ablation (including repeat) procedures over 1 year, which all included PVI, with additional linear lesions at the mitral isthmus in 73%, and cavotricuspid isthmus (CTI) in 76%. An irrigated-tip ablation catheter was used, with power limited to 25–35 W for PVI and 45–60 W for linear lesions. Tamponade occurred in seven men and three women (2.9% of the population) during the creation of linear ablation lesions. Mechanical perforations occurred in two patients, and "popping" during radiofrequency (RF) energy delivery at the mitral isthmus in six, and at the CTI in two patients. Peak RF power was significantly higher in patients with than without tamponade (53 ± 4 W vs 48 ± 7 W; P = 0.02), and was greater than 48 W in all cases of "popping." In the following year, RF power for linear ablation was limited to ≤42 W. Among 398 procedures, tamponade occurred in four patients (1.0%; P = 0.047 vs first year), three from "popping" and one from mechanical trauma. Procedural success rate remained the same despite reduction of power. Risk of tamponade was highest during linear ablation, mainly associated with high energy delivery and "popping." Reducing the energy limited, though did not eliminate this complication.  相似文献

Mode switching algorithms are commonly used to protect the ventricles against high rates induced by atrial tachycardia. In the case of atrial fibrillation (AF), the response of these algorithms depends on the quality of atrial sensing. The Chorum 7234 DDDR pacemaker uses a new mode switching algorithm, based on a statistical analysis of the atrial rhythm. It includes two criteria of diagnosis: "high" if more than 28 of 32 cycles are abnormally accelerated; and "low" if more than 36 of 64 cycles are abnormally accelerated. Methods: From a taped database of electrophysiological studies, episodes of AF lasting more than 2 minutes were selected. A tape recorder replayed the atrial signals into an external Chorum device. Each episode was replayed eight times with a programmed atrial sensitivity increasing from 0.4 –2.0 mV. For each criterion of diagnosis and each programmed sensitivity, the percentage of atrial sensing, the time to switching, and the mean ventricular rate were measured. Ten episodes of AF from 10 patients (9 men and 1 woman; ages 62 ± 16 years) were included: 1.95 ± 0.97 mV and 196 ± 64 ms. The sensitivity of the algorithm to diagnose atrial tachycardia reached 100%, for an atrial sensitivity set between 0.4 and 1.0 mV. The mean percentages of atrial sensed events were 74%± 18% and 46%± 9% for the "high" and "low" criteria, respectively. The mean diagnostic times were 28 ± 26 seconds and 68 ± 27 seconds, respectively. Sensing of < 23% of AF events resulted in failure to diagnose the arrhythmias by both algorithms. In the event of diagnostic failure, the mean ventricular pacing rate was 79 ± 9 ppm. Conclusion: Up to an atrial sensitivity of 1 mV, 100% of AF episodes were diagnosed. The Chorum mode switching algorithms are 100% reliable if > 45% of the AF waves are sensed. In the event of switching failure, the ventricle is protected by an average rate remaining below 80 ppm. Special software allowing the memorization of 24-hour minute ventilation can be loaded into the memoiy of the Chorus RM, a DDDR pacemaker driven by minute ventilation. This feature was tested in the postimplant period in 13 patients. Measurements of minute ventiiation, respiratory rate, and respiratory amplitude were analyzed according to prospectively defined diurnal and nocturnal time periods. Minute ventilation decreased by 39% (P < 0.001) from the diurnal to the nocturnal phase, while respiratory rate and amplitude decreased by 18% and 28%, respectively (P < 0.001 each). Thus, minute ventilation allowed discrimination between sleep and waking hours. This information could be utilized to modulate the backup rate of the pacemaker.  相似文献

Background: A continuous popliteal sciatic nerve block (CPSNB) has been performed with increasing frequency for post‐operative analgesia after foot surgery. Major complications associated with the placement of a perineural catheter remain rarely studied. The aim of this study was to prospectively determine the incidence of major complications (neurological and infectious) in post‐operative adult patients with a continuous popliteal catheter inserted by the anatomical posterior approach for analgesia after foot surgery. Methods: All popliteal catheters were placed pre‐operatively under sterile conditions with the aid of a nerve stimulator technique. The primary outcome measure was the incidence of major complications including infection and neuropathy. As a secondary outcome, adverse effects as well as other complications were also evaluated. Data were expressed as median [25th–75th percentiles]. Results: A total of 400 patients were included in the study during a 2‐year period. The median time the catheter remained indwelling was 47 h [23, 54]. Major complications included three events (0.75%) with one infection (0.25%) and two neuropathies (0.50%). Three blocks were unsuccessful and the catheter insertion was difficult in 12 patients (3%). During the CPSNB procedure, one patient reported slight paraesthesia during stimulation. Patient satisfaction was scored at 4 for 89%, 3 for 6% and 2 for 5% on the analogue scale. Conclusions: Major complications after the use of CPSN are not in fact rare. The incidence of severe neuropathy or infection complications is, respectively, 0.50% and 0.25%. However, the insertion of CPSN could be considered effective and is associated with only a few minor complications.  相似文献
