Atrial fibrillation (AF) may be associated with activation of atrial natriuretic peptide (ANP). The exact trigger for the release of ANP is still being debated. Atrial volume, pressure, and wall stretch are considered to be the main determinants of ANP activation. The aim of the study was to evaluate plasma ANP concentrations in patients with persistent AF and to analyze the echocardiographic determinants of ANP concentration in this group. The study population included 67 patients, 59 ± 7 years of age, with a median AF duration of 5.5 months (range 0.1–12). The relationship between plasma ANP concentrations and echocardiographic left atrial (LA) diameter and volume, and left ventricular (LV) diameter and ejection fraction (EF) was analyzed by logistic regression analysis. The median baseline plasma ANP concentration was 63 pg/mL (range 21–126) in the study group versus 34 pg/mL (range 16–73) in a control group. The mean left antero-posterior atrial dimension, LA volume, LV enddiastolic diameter, and LVEF were 48 mm, 104 mL, 52 mm, and 54%, respectively. A significant linear positive correlation was found between plasma ANP concentration and maximal LA volume (r = 0.62, P < 0.01). A negative correlation was found between LVEF and plasma ANP concentration (r =−0.42, P = 0.01). However, by multivariate regression analysis, no echocardiographic parameter was an independent predictor of plasma ANP concentration. Plasma ANP concentrations were independent of echocardiographic measurements of LA size or LV size and function in patients with persistent AF.  相似文献

QRS Fragmentation and the Risk of Sudden Cardiac Death in MADIT II. Background: QRS fragmentation (fQRS) has been reported as a useful ECG parameter in predicting mortality in high‐risk postinfarction patients. Its prognostic value for sudden cardiac death (SCD) and ventricular arrhythmias in ischemic cardiomyopathy (ICM) remains unknown. Methods: MADIT II enrollment 12‐lead ECGs were analyzed for fQRS defined as RSR’ patterns (≥1 R’ or notching of S or R wave) in patients with a normal QRS duration and >2 notches on the R or S wave in patients with abnormal QRS duration, present in 2 contiguous leads. Exclusion criteria included a paced rhythm and an uninterpretable or incomplete ECG. Study endpoints included SCD, SCD or appropriate implantable cardioverter defibrillator (ICD) shock, and total mortality (TM). Results: Of the 1,232 ECGs reviewed, 1,040 were of suitable quality for fQRS analysis. QRS fragmentation was found in 33% of patients in any leads, in 10% of patients in anterior leads, in 8% of patients in lateral leads and in 21% of patients in inferior leads. Anterior and lateral location of QRS fragmentation was not associated with follow‐up events. Inferior location of fQRS was found to be predictive of SCD/ICD shock (hazard ratio [HR] 1.46, P = 0.032), SCD (HR 2.05, P = 0.007), and TM (HR 1.44, P = 0.036). This association was driven primarily by the increase in events found in LBBB patients: SCD/ICD shock (HR 2.05, P = 0.046), SCD (HR 4.24, P = 0.002), and TM (HR 2.82, P = 0.001). Conclusions: Fragmented QRS, especially identified in inferior leads, is predictive of SCD, SCD or appropriate ICD shock, and all‐cause mortality in patients with ICM. Identifying inferior fQRS in patients with LBBB is of particular prognostic significance and should reinforce the use of ICD therapy in this high‐risk group. Many recent studies have shown transesophageal programmed atrial pacing (TP) as a very practical, safe and convenient way for assessment of sinus node function and AV conduction. On the other hand, permanent atrial pacing is known to be superior to ventricular pacing due to arrhythmogenic and hemodynamic reasons. This is the reason why we decided to use TP as a method of choosing patients with sick sinus syndrome (SSS) for permanent atrial pacing. Sixty-three patients with symptomatic (58) and asymptomatic (5) SSS in a variety of clinical situations were examined in this way. The following electrophysiological features were examined: sinus cycle length, sinus node recovery time as well as corrected time, secondary pause after overdrive stimulation, sinoatrial conduction time, Wenckebach point, induction of supraventricular arrhythmias by S1, S2, S3 programmed stimulation and burst pacing. Patients with abnormal parameters were examined once more after intravenous atropine 0.2 mg/kg to evaluate parasympathetic component. Standard 12-lead ECG was performed in ail, and Holter monitoring in most of patients.
Twenty-six patients were candidates for permanent AAI pacing. Failures occurred in eight patients usually due to low P wave amplitude and electrode instability. Eighteen patients received AAI pacing systems: eight with brady-tachycardia syndrome, nine with brady-arrhythmia and one with sinoatrial block. In the follow-up of 5–28 months in one patient occurred high degree AV block (11°) during digitalis therapy. Reduction of doses made this block disappear. Examination of Wenckebach point and possibility of inducation of supraventricular lachyarrhythmias in cases of atrial overexcitability are particularly useful in selecting patients for AAI pacing. Background: Neointimal formation and vascular remodeling are major mechanisms contributing to coronary restenosis after percutaneous transluminal coronary angioplasty. Intracoronary γ- and β-radiation have been shown to inhibit neointimal formation in balloon-injured porcine coronary arteries. However, the effects of Intracoronary radiation (ICR) dose and dose rate on vascular remodeling versus inhibition of neointimal formation have not been evaluated. Methods and Results: Forty-one Hanford Miniature swine were subjected to oversized balloon injury in the left anterior descending and right coronary artery. A spiral centering catheter with perfusion capabilities was placed in the injured segment for circumferentially uniform radiation dose delivery to the vessel wall. Each artery was randomly afterloaded with a dummy wire or an active wire with the 32P source encapsulated in either a short (3 mm) or long (27 mm) segment at the distal end of the wire. The 3-mm source wire was used to deliver 650, 1,300, and 1,900 cGy while the 27-mm source wire was used to deliver 1,200 and 3,500 cGy of β-radiation to the adventitia (~ 0.5 mm into the vessel wall). The dose rate for ICR varied from 2.5 to 140 cGylsec, depending on the mCi strength of the 32P source at the time of endovascular radiation. One month later, repeat coronary angiography and intravascular ultrasound (IVUS) to measure the external elastic lamina (EEL) area were done. The animals were sacrificed and the coronary vasculature was perfusion fixed. Morphometric, quantitative coronary angiographic, and IVUS analyses were carried out in a blinded fashion. A significant reduction in percent area stenosis (PAS) and neointimal area (NA) was observed on morphometry in coronary arteries treated with 3,500 cGy of β-radiation. The PAS and NA was 44%± 13% and 0.96 ± 0.25 mm2 in the control groups versus 19%± 14% and 0.30 ± 0.23 mm2 in the 3,500-cGy group (P < 0.02). There was no significant difference on morphometry between the control and the other four β-radiationtreated groups. There was no significant improvement in the change in minimum lumen diameter (ΔMLD) between the control and the five radiation-treated groups. Further analysis of angiographic data revealed that the apparent lack of beneficial effect on angiography was due to significant reductions in lumen diameter in the subgroups of arteries subjected to ICR at a dose rate > 50 cGylsec with the 3-mm source wire only. The ΔMLD was -1.39 ± 0.49, –1.79 ± 0.64, and -1.79 ± 0.39 mm in the 650-, 1,300-, and 1,900-cGy groups treated with a dose rate > 50 cGylsec versus -0.56 ± 0.95 in control (P < 0.05). This reduction in lumen diameter on angiography was associated with a significant reduction in vessel (EEL) area especially in the groups treated with the 3 mm source at a dose rate > 50 cGylsec. The EEL area was 8.8 ± 1.7, 9.9 ± 1.7, and 8.9 ± 0.6 mm2 in the 650, 1,300, and 1,900 cGy groups treated at a dose rate > 50 cGylsec compared to 11.7 ± 1.6 mm2 in control arteries (P < 0.001). Conclusions: Endovascular β-radiation at high doses (3,500 cGy to adventitia) and dose rate < 50 cGylsec inhibits restenosis after balloon injury in the porcine model of coronary restenosis. This dose and dose rate is associated with a neutral effect on vascular remodeling. While lower doses of ICR did not worsen the PAS, a reduction in MLD and EEL area were observed. However, this adverse effect on angiographic restenosis and vascular remodeling appears to be largely limited to the subgroups treated at dose rates > 50 cGylsec.  相似文献

ABSTRACT: The mechanism of spermatozoalymphocyte interaction was studied using spermatozoa and lymphocytes obtained from 11 healthy individuals and from 2 patients with spermatocoele. The lymphocytes activated by spermatozoa in 3-day cultures were used as inducers of fresh, unstimulated autologous or allogeneic lymphocytes. In 9 cases of 13 tested sperm samples, a slight proliferative response of lymphocytes stimulated by spermatozoa-induced lymphocytes was observed. The final effect did not depend on whether an autologous or an allogeneic system was used in the first and the second incubation. A possible explanation of the observed reaction is discussed.  相似文献

Background: The diagnosis of the impaired left ventricle (LV) diastolic function during atrial fibrillation (AF) using traditional methods is very difficult. Natriuretic peptides seem to be useful for assessment of diastolic function in patients with AF. Aim: To evaluate the influence of LV diastolic dysfunction on natriuretic peptides concentrations and to assess the diagnostic value of atrial natriuretic peptide (ANP) and B‐type natriuretic peptide (BNP) in patients with AF and impaired LV diastolic function. Methods: The study included 42 patients (23 males, 19 females), aged 58.6 ± 8.2 years with nonvalvular persistent AF with preserved LV systolic function who were converted into sinus rhythm by DC cardioversion (CV) and maintained sinus rhythm for at least 30 days. Echocardiography (ECG), ANP, and BNP level measurements were taken at baseline 24 hours before CV and 24 hours and 30 days after CV. On the 30th day following CV in patients with sinus rhythm, Doppler ECG was performed to assess LV diastolic function. Results: Thirty days after CV, normal LV diastolic function in 15 patients and impaired diastolic function in 27 patients was diagnosed: 20 with impaired LV relaxation and seven with impaired LV compliance. During AF and 24 hours, and 30 days after sinus rhythm restoration, significantly higher ANP and BNP levels were observed in patients with LV diastolic dysfunction as compared to the subgroup with normal LV diastolic function. The average values of ANP during AF in patients with normal and impaired diastolic function were 167.3 ± 70.1 pg/mL and 298.7 ± 83.6 pg/mL, respectively (P < 0.001), and the average values of BNP in the above mentioned subgroups were 49.5 ± 14.7 pg/mL and 145.6 ± 49.6 pg/mL respectively (P < 0.001). While comparing the diagnostic value of both natriuretic peptides it was noted that BNP was a more specific and sensitive marker of impaired LV diastolic function. ANP value >220.7 pg/mL measured during AF identified patients with impaired LV diastolic function with 85% sensitivity and 90% specificity. BNP value >74.7 pg/mL proved 95% sensitive and 100% specific in the diagnosing of such a group. Conclusions: The increase of ANP/BNP concentration in patients with AF results not only from the presence of AF, but also reflects the impaired LV diastolic function. Natriuretic peptides, especially BNP, may be useful in diagnosing LV diastolic dysfunction in patients with AF.  相似文献

Background: Measurement of natriuretic peptide's (NP) release in response to hemodynamic stress may be complementary to its baseline assessment in individuals. Atrial natriuretic peptide (ANP) and B‐type natriuretic peptide (BNP) increase in patients with atrial fibrillation (AF) and decrease after successful cardioversion, suggesting that AF may stimulate secretion of NPs. However, there are conflicting data on the predictive value of NPs on the cardioversion outcome. Objectives: The purpose of this study was to investigate whether baseline and exercise‐induced NP plasma levels can be useful in predicting successful cardioversion of persistent AF and maintenance of sinus rhythm during 6‐month follow‐up. Methods: A prospective study enrolled 77 consecutive subjects with persistent AF with normal left ventricular function, referred for elective cardioversion. Patients underwent a modified Bruce protocol treadmill exercise test 24 hours before cardioversion. Blood samples for ANP and BNP analyses were obtained at rest and 5 minutes after exercise peak. Results: The group of successful cardioversion and stable sinus rhythm presented higher exercise ANP (110.6 ± 41.2 pg/mL vs 43.8 ± 36.1; pg/mL, P < 0.0001) and lower BNP increase (5.2 ± 5.2 pg/mL vs 40.5 ± 34.2 pg/mL, P < 0.0001) than the group of unsuccessful cardioversion or AF recurrence. Using an optimized cutoff level of ≤12% of relative exercise‐induced increase in BNP concentration, and of >50 pg/mL of ANP increase, successful cardioversion can be predicted with high accuracy. Conclusions: An increase in ANP and stability of BNP plasma concentration during exercise testing are independently associated with successful cardioversion and maintenance of sinus rhythm during 6‐month follow‐up. Background : Prophylactic therapy with the implantable cardioverter defibrillator (ICD) reduces the mortality among patients with left ventricular dysfunction; however, life-prolonging device therapy has been shown to be associated with an increased risk for subsequent heart failure (HF) events. There are limited data on the effect of the primary types of HF medications, angiotensin converting enzyme inhibitors (ACE-I), and beta-blockers on HF progression in ICD-treated patients.
Methods : Multivariate Cox proportional hazards regression analysis was used to assess the effect of time-dependent medical therapy with ACE-I and beta-blockers on the development of HF in patients with an ICD in the Multicenter Automatic Defibrillator Trail (MADIT) II.
Results: In multivariate analysis, ICD therapy was associated with a significant 39% increase in the risk of HF as compared with conventional medical therapy. ACE-I and beta-blockers exhibited a graded efficacy for the reduction in the risk of HF events in ICD-treated patients: the greatest risk reduction of HF was seen in patients taking combination therapy (HR = 0.36, P < 0.001), followed by patients using beta-blockers only (HR = 0.51, P = 0.017) and ACE-I only (HR = 0.64, P = 0.071). Beta-blocker subtypes (metoprolol [HR = 0.49, P = 0.001] and carvedilol [HR = 0.58, P = 0.004]) exhibited similar efficacy. Consistent results were demonstrated when the combined endpoint of HF or death was assessed.
Conclusions : ICD-treated patients experience an increased risk for HF events that can be significantly attenuated by medical therapy with beta-blockers and ACE-inhibitors.  相似文献
