Phase I trial of doxorubicin-containing low temperature sensitive liposomes in spontaneous canine tumors.

PURPOSE: To determine the maximum tolerated dose, dose-limiting toxicities, and pharmacokinetic characteristics of doxorubicin encapsulated in a low temperature sensitive liposome (LTSL) when given concurrently with local hyperthermia to canine solid tumors. EXPERIMENTAL DESIGN: Privately owned dogs with solid tumors (carcinomas or sarcomas) were treated. The tumors did not involve bone and were located at sites amenable to local hyperthermia. LTSL-doxorubicin was given (0.7-1.0 mg/kg i.v.) over 30 minutes during local tumor hyperthermia in a standard phase I dose escalation study. Three treatments, given 3 weeks apart, were scheduled. Toxicity was monitored for an additional month. Pharmacokinetics were evaluated during the first treatment cycle. RESULTS: Twenty-one patients were enrolled: 18 with sarcomas and 3 with carcinomas. Grade 4 neutropenia and acute death secondary to liver failure, possibly drug related, were the dose-limiting toxicities. The maximum tolerated dose was 0.93 mg/kg. Other toxicities, with the possible exception of renal damage, were consistent with those observed following free doxorubicin administration. Of the 20 dogs that received > or = 2 doses of LTSL-doxorubicin, 12 had stable disease, and 6 had a partial response to treatment. Pharmacokinetic variables were more similar to those of free doxorubicin than the marketed liposomal product. Tumor drug concentrations at a dose of 1.0 mg/kg averaged 9.12 +/- 6.17 ng/mg tissue. CONCLUSION: LTSL-doxorubicin offers a novel approach to improving drug delivery to solid tumors. It was well tolerated and resulted in favorable response profiles in these patients. Additional evaluation in human patients is warranted.     

含左氧氟沙星及卷曲霉素方案治疗耐多药肺结核近期疗效分析

目的　观察和评价含左氧氟沙星和卷曲霉素联合化疗方案在耐多药肺结核 (MDR PTB)治疗中的疗效。方法　将 177例MDR PTB患者分为治疗组 88例和对照组 89例。化疗方案 :治疗组以左氧氟沙星和卷曲霉素为主 ,联合利福喷汀、异烟肼、对氨基水杨酸钠、吡嗪酰胺 ;对照组用链霉素、乙胺丁醇 ,联用药物同治疗组 ,疗程均为 2 1个月。结果　共有 16 1例患者完成化疗疗程 ,治疗组 82例 ,痰菌阴转率 83% ;对照组 79例 ,痰菌阴转率 5 8% ;痰菌阴转率治疗组明显高于对照组 (P <0 0 1) ;治疗组病灶显效率 5 0 % ,空洞闭合率 6 3% ,治疗组优于对照组 (P <0 0 1) ;治疗组的药物不良反应率为 31% ,对照组为 35 % ,两组比较差异无显著性 (P >0 0 5 )。结论　含左氧氟沙星和卷曲霉素的方案治疗MDR PTB ,有助于痰菌阴转和病变吸收好转 ,药物不良反应低 ,值得在临床上推广应用

实施归口管理对肺结核病人发现率和治愈率的影响分析

目的　探讨实施结核病归口管理对提高涂阳肺结核病人发现率和治愈率的影响。方法　对 1994年至2 0 0 0年 2 0个参加卫生部加强与促进结核病控制项目县的项目报表和年转归报表进行分析。结果　由于项目县实施了结核病归口管理 ,涂阳病人的发现率、治愈率明显高于未实施结核病归口管理的非项目县。结论　实施结核病归口管理是提高肺结核病人特别是具有传染性肺结核病人发现率和治愈率的有效措施     

自杀行为及其干预的研究进展

自杀已经成为一个世界范围内的重大公共卫生问题。中国的自杀特征具有独特性。自杀行为的发生往往是内在因素与外部因素相互作用的结果，它与社会学、心理学、伦理学、生物学、精神病学等各方面密切相关。自杀原因虽然错综复杂，但我们要提高意识，认识到自杀是一种可以预防的公共卫生问题。     

Genetic Analyses of the Mouse Deafness Mutations Varitint-Waddler (Va) and Jerker (Espnje)

Genetic studies on spontaneous mouse mutants with hearing defects have provided important insights into the function of genes expressed in inner ear hair cells. Here we report on our genetic analyses of the deaf mutants varitint-waddler (Va) and jerker (Espnje). A high-resolution genetic map localizes VaJ to a 0.14 ± 0.08 cM region between D3Mit85 and D3Mit259 on distal chromosome 3. By comparative mapping, the human ortholog resides at 1p22.3 between markers D1S3449 and D1S2252. To study the effect of different genetic backgrounds on the hearing phenotype, Espnje and VaJ were crossed to various inbred strains. Auditory-evoked brainstem response tests on F2 progeny demonstrate that expression, inheritance, and penetrance of the hearing phenotype are solely controlled by the mutant allele. To test for a genetic interaction between Espnje and Cdh23v, auditory function was analyzed in double heterozygotes; no significant increases of thresholds of sound pressure levels were observed. The results establish the framework for cloning the Va gene and provide valuable insights into the genetics of deafness mutations in the mouse.     

谷参肠安胶囊长期毒性试验

本文报告谷参肠安胶囊大鼠长期毒性试验,结果表明,谷参肠安胶囊对动物的一般状况,体重增长、外周血象、肝肾功能及病理组织学检查等未见明显毒性。     

海南省第四次全国结核病流行病学抽样调查分析

目的：评价世行贷款结核病项目实施效果，掌握疫情现状，为制度2001－2010年防治规划提供理论依据。方法：按照《第四次全国结核病流行病学抽样调查工作手册》对2个随机抽样点人群进行流调。结果：活动性肺结核13例，患病率和涂阳患病率434．8／10万，33．4／10万，较1990年流设疫情分别下降了43．4％和27．3％，BCG接种率34．9％，较1996年流调提高了一倍。结论：以控制传染源为目的：以DOTS为核心的结核病控制策略是成功的，可行的，政府的行政干预及保持必要的经费投入，对结核病控制可持续发展具有重要意义。农村儿童及新生儿BCG接种工作亟待改善。
