Zinc homeostasis during lactation in a population with a low zinc intake.   总被引:2,自引:0,他引:2  
BACKGROUND: There is a major increase in endogenous zinc excretion, specifically via the mammary gland, in early human lactation. Whereas fractional absorption of dietary zinc has been reported to increase in early human lactation, it is not known to what extent adaptive mechanisms may maintain zinc homeostasis, especially when dietary zinc intake is relatively low. OBJECTIVE: The objective of this study was to quantitate major variables of zinc homeostasis during early lactation in subjects from a population whose habitual dietary zinc intake is low. DESIGN: We studied 18 free-living lactating women from a rural community of northeast China whose infants were exclusively breast-fed. The subjects were studied at approximately 2 mo of lactation with use of stable isotopes of zinc and metabolic collection techniques. Milk volume was measured with use of a deuterium enrichment method. RESULTS: The mean (+/-SD) secretion of zinc in milk was 2.01 +/- 0.97 mg/d, the intake of zinc was 7.64 +/- 1.61 mg/d, and the fractional absorption of zinc was 0.53 +/- 0.09, for a total daily zinc absorption of 4.00 +/- 0.71 mg/d. Endogenous zinc excretion in urine and feces was 0.30 +/- 0.10 and 1.66 +/- 0.97 mg/d, respectively. CONCLUSIONS: Zinc balance, including zinc secreted in breast milk, was maintained at approximately 2 mo of lactation in women whose habitual diet was low in zinc. Homeostasis was achieved by high fractional absorption of zinc and intestinal conservation of endogenous fecal zinc.  相似文献

Prevalence of anal incontinence in adults and impact on quality-of-life   总被引:2,自引:0,他引:2  
OBJECTIVE: To investigate the prevalence of anal incontinence in the general population and in patients consulting gastroenterologist and gynecologist practices in the Rh?ne Alpes area. METHODS: For the first study a questionnaire was sent to a sample of 2800 people selected randomly from the electoral roll. Another study of patients selected randomly among patients attending gynecology and gastroenterology consultations was performed. A Jorge & Wexner score above or equal to 5 was used to define anal incontinence. RESULTS: For the first study, a total of 706 questionnaires was analyzed: the prevalence of anal incontinence was 5.1% [95% CI: 3.6-7.0] and the scores of each dimension of the SF-12 Health Survey were significantly lower among incontinent people than among continent people. The prevalence was significantly higher for women (7.5% [5.0-10.7]) than for men (2.4% [1.1-4.7]). Eighty-four physicians returned 835 valid questionnaires. The prevalence was 13.1% [10.1-16.6] among patients attending gastroenterology consultations and 5.0% [3.1-7.6] among those attending gynecology consultations. For 84.8% of the incontinent patients, the physician was unaware of the patient's disorder. CONCLUSION: The prevalence figures we obtained coincide with data in the literature. This disorder is common and affects the patient's quality-of-life, but remains underestimated and under-diagnosed.  相似文献

Background/purpose

Intestinal parasitic infections (IPIs) among school aged children (SAC) in Nigeria remains endemic, hence the need for regular surveillance to attract the attention of policy makers. This cross-sectional study investigated the current prevalence and factors associated with intestinal parasitic infections among school aged children in an urban slum of Lagos City, Nigeria.

Methods

Single stool samples from 384 school aged children (188 boys and 196 girls) were examined by employing Merthiolate-iodine-formaldehyde concentration (MIFC) and Kato-Katz methods. Demographic characteristics and risk factors were obtained by questionnaires investigation.

Results

The overall prevalence was 86.2% in school children, out of them 39.1% had polyparasitism. IPIs showed the highest to the lowest prevalence of 62% (238/384), 25% (97/384), 12.3% (47/384), 11.8% (45/384), 9.9% (38/384), 8.4% (32/384), 3.4% (13/384), and 0.5% (2/384) found in Ascaris lumbricoides, Entamoeba histolytica/dispar, Giardia duodenalis, Endolimax nana, Entamoeba coli, Trichuris trichiura, Blastocystis hominis, and hookworm infections, respectively. MIFC technique showed superiority to Kato-Katz technique in the detection of IPIs (p < 0.0001). Drinking untreated water was a significant risk factor for these school aged children in acquiring protozoan infections after multivariate adjustment (OR = 1.86, 95% CI = 1.08–3.20, p = 0.02).

Conclusion

Intestinal parasitic infections are very severe among school aged children in the urban slums, thus regular mass de-worming programs, health education, and the provision of safe drinking water is recommended to combat IPIs among the school aged children.  相似文献

OBJECTIVE: To determine dose-dependent relationship between ingested fat and its oxidation in the immediate post-prandial period in humans. DESIGN: Subjects were randomly selected for the study at the Dunn Clinical Nutrition Centre, Cambridge, UK. Subjects ingested naturally enriched 13C corn-oil doses (range 20-140g) in a whole-body indirect calorimeter, and were studied for 8 h. Ingested fat oxidation was estimated from the subject's breath 13C enrichment and total carbon dioxide production. Total fat and carbohydrate oxidation were estimated from non-protein oxygen and carbon dioxide exchanges. Endogenous fat oxidation was estimated as the difference between total fat and ingested fat oxidation. RESULTS: The amount of fat dose oxidized was nonlinearly related to the amount ingested. On average, 25.6+/-2.7% of the mean fat dose was oxidized. A significant (r = - 0.72, P < 0.001) inverse correlation was found between the amount of fat dose and the proportion oxidized. Endogenous carbohydrate oxidation was negatively and significantly correlated to fat dose oxidized (r= -0.61, P < 0.01), but it was not correlated to endogenous fat oxidation. CONCLUSIONS: There was a nonlinear relationship between amount of fat dose and its quantity that was oxidized in the immediate post-prandial period. The inverse relationship between the size of the fat load and the proportion that was oxidized post-prandially implies increased dietary fat storage beyond about 50 g in a normal resting adult. This has important implications for 13CO2-based studies.  相似文献

The role of ingested fat in the etiology of obesity is controversial. The aims of this study were to determine the contributions of ingested fat oxidation to: 1) 24-h total energy expenditure (TEE), and 2) substrate oxidation during acute stationary cycle exercises in adult humans. Healthy, moderately obese (n = 18; BMI = 31 +/- 1 kg/m2) subjects (8 men; 10 women) were each studied in a whole-room calorimeter for 24 h. They were fed mixed meals (55, 30, and 15% as energy from carbohydrate, fat and protein, respectively) to maintain energy balance. Each subject performed 1255-kJ cycle exercises at 50% VO2max in the calorimeter. Study test meal fat was labeled with carbon-13 (13C). Ingested fat oxidation was estimated from breath 13CO2 excretion and the subject's chamber CO2 production. Total fat and carbohydrate oxidations were estimated from nonprotein respiratory quotient (NP-RQ) values. Endogenous fat oxidation was estimated as the difference between total fat and ingested fat oxidations. TEE was estimated from gas exchanges; 28 +/- 3% of ingested fat was oxidized and it provided 8 +/- 1% of 24-h TEE. During cycle exercises, ingested fat provided 50% of total fat oxidized and 13.0 +/- 2% of energy expended. Endogenous fat oxidation contributed 10.4 +/- 3% of energy expenditure during cycle exercises. This study extended to 24-h observations of previous studies that lasted 6-9 h on ingested fat oxidation in humans. Understanding the factors that promote ingested fat oxidation could lead to more effective obesity intervention programs.  相似文献

Tritiated catechol has been used to follow the pharmacokinetics and metabolic fate of inhaled catechol in cigarette smoke in BC3F1Cum mice. The presence of [3H]catechol in the smoke was verified by silica gel chromatography, high-performance liquid chromatography, and gas chromatography/mass spectrometry. Mice were exposed to 10% (vv) 2R1 cigarette smoke on the Walton Horizontal Smoking Machine under standard conditions of 35 ml puff volume, 2 sec/puff, 10 puffs/cigarette. The deposition and distribution of inhaled catechol were determined in all internal tissues, urine, and feces. Data showed that clearance was occurring during the 10-min smoke exposure period. Immediately after exposure, over 50% of the radioactivity was found in the blood, with 10% found in the lung, and approximately 12% in the respiratory tract. Over 91% of the inhaled radioactivity was found in the urine 120 min after exposure. Less than 0.5% of the total dose was found in the lung at this time. We conclude that catechol in smoke is rapidly absorbed, redistributed, and excreted from mice exposed to whole cigarette smoke.  相似文献

PURPOSE: To evaluate the criterion validity of the 7-Day Physical Activity Recall (7D-PAR) and factors associated with reporting error, in a sample of moderately overweight, young adult men and women. METHODS: Average total daily energy expenditure (TDEE) and physical activity energy expenditure (PAEE) from the 7D-PAR were compared with the same parameters assessed by doubly labeled water in 17 men, age = 23.9 +/- 3.8 yr, and 29 women, age = 23.3 +/- 4.6 yr, who volunteered to participate in a 16-month supervised aerobic exercise trial. PAEE was estimated from the 7D-PAR and from DLW [0.9 * TDEE -resting metabolic rate (RMR) (indirect calorimetry)]. In addition, peak oxygen uptake and percent body fat were obtained. RESULTS: No significant differences in TDEE (kJ.d-1) were noted between the 7D-PAR (11825 +/- 1779) and DLW (11922 +/- 2516) for the complete sample (N = 46) or for men (7D-PAR = 13198 +/- 1638, DLW = 13885 +/- 2754) or women (7D-PAR = 11018 +/- 1323, DLW = 10771 +/- 1457. The mean PAEE from the 7D-PAR was not different from DLW in the total sample (7D-PAR = 3286 +/- 502, DLW = 3508 +/- 1863) as well as in men (7D-PAR = 3650 +/- 490, DLW = 3989 +/- 2461) and women (3073 +/- 377, DLW = 3223 +/- 1360). In a regression model, PAEE, peak oxygen uptake, gender and percent fat accounted for 86% of the reporting error in total daily energy expenditure when using the 7D-PAR. CONCLUSION: The 7D-PAR provided a reasonable estimate of both the mean TDEE and PAEE in this sample; however, estimates of energy expenditure on an individual basis using the PAR were subject to considerable error.  相似文献
