The tricarboxylic acid cycle in human skeletal muscle: is there a role for nutritional intervention?

The tricarboxylic acid (TCA) cycle is essential for oxidative energy production. The expansion (anaplerosis) of the intermediates of the TCA cycle is achieved via a number of pathways, and is known to be influenced by metabolic status and nutritional and pharmacological interventions. Contraction is associated with anaplerosis in skeletal muscle, and some authors have suggested that the rate of anaplerosis can limit oxidative energy delivery. The results of more recent studies, however, are consistent with the idea that expansion of the muscle TCA intermediate pool is principally a reflection of muscle pyruvate availability, and is of little functional importance to TCA cycle flux, thereby indicating that any intervention aimed at increasing TCA intermediates expansion will be of little practical value. 相似文献

Motor unit action potential (MUAP) components are investigated by means of single fiber computer simulations and clinical measurements. The single fiber simulations have essentially full bandwidth without noise, whereas the clinical measurements were made with a 3-10,000-Hz bandwidth utilizing approximately 1000 averages to reduce noise optimally. These parameters allow the recording of a MUAP's complete "physiologic" duration including its very slow onset and termination. The simulation results demonstrate a constant waveform onset regardless of the electrode's recording location along the fiber. A far-field potential is initiated when the action potential encounters the muscle fiber's termination. The simulated waveform's and clinically recorded MUAP's near-field component extends between the potential's onset and its corresponding far-field potential's onset. This near-field component's duration should vary with fiber length, and this prediction is clinically confirmed by measuring three different muscle lengths. The far-field potential reveals a constant duration, independent of fiber length, and appears to be associated with the muscle fiber's intracellular action potential duration. A more complete understanding of the components contributing to MUAP duration should provide a more fundamental basis for quantitative clinical MUAP duration measurements. 相似文献

Changes in muscle elasticity are expected in patients with untreated myositis. The purpose of this study was to define the accuracy of shear-wave elastography (SWE) in diagnosing myositis. This case control study included 21 patients (mean age, 49.4 y; 12 women) with myositis who underwent SWE, magnetic resonance imaging (MRI) and biopsy of the involved muscle group. SWE was performed accordingly in a control group (n = 24; mean age, 51.2 y; 8 women). Blood tests consisted of creatine kinase (CK) and aldolase. Two operators performed SWE in longitudinal and transverse planes of muscular fibers, quantifying the mean shear-wave velocity (SWV) and the pattern of stiffness. On MRI, short-TI inversion recovery (STIR) signal hyperintensity and T1 contrast enhancement of muscle was considered diagnostic for myositis. The patient group suffered from different types of myositis (nine patients with polymyositis, eight with dermatomyositis and four with other types of myositis). Blood tests showed significantly increased CK and aldolase values in patients with myositis (p < 0.001 and p < 0.0001). MRI showed a sensitivity of 0.95. In the patient group, the mean SWVs of longitudinal and transverse measurements were 2.8 ± 1.4 m/s and 3.1 ± 1.2 m/s, respectively. In the control group, SWVs were 2.3 ± 0.5 m/s and 2.4 ± 0.5 m/s, respectively. The difference between transverse measurements was significant (p = 0.02). Increased heterogeneity as a marker for myositis in transverse SWE showed a sensitivity of 0.8, specificity of 0.79, positive predictive value (PPV) of 0.76 and negative predictive value (NPV) of 0.82. Inter-observer difference was very low (κ = 0.92). Increased heterogeneity in both planes compared with histologic results showed a sensitivity of 0.56, specificity of 0.93, PPV of 0.91 and NPV of 0.62. Spearman correlation between CK <1000 U/L and SWE was 0.54. In conclusion, transverse orientation SWE may serve as an imaging biomarker for the diagnosis of myositis through the display of a heterogeneous pattern and increased absolute SWV values of inflamed muscles. 相似文献

Imaging in the major planes (horizontal, coronal, and sagittal) of the uterus is important for determining anatomy and allowing the findings to be standardized, and for evaluating and diagnosing different pathological conditions in clinical practice. Examination of the coronal plane is an important step in identifying uterine pathologies and their relationships to the endometrial canal. Three‐dimensional (3D) ultrasound reveals the normal anatomy better and improves the depiction of abnormal anatomy, as the coronal plane of the uterus can easily be obtained using 3D reconstruction techniques. Our pictorial essay demonstrates that adding 3D ultrasound to a routine gynecological workup can be beneficial for clinicians, enabling a precise diagnosis to be made. In addition, the volumes obtained and stored by 3D ultrasound can allow students or residents to become more familiar with normal and abnormal pelvic structures. Clin. Cavernous haemangioma (cavernoma) is a benign vascular lesion, exceptionally located in cauda equina. We report a case, diagnosed and operated in the Department of Neurosurgery from Pitesti County Emergency Hospital, of a 60-year-old woman with history of lumbar region distress, who presented with low back pain, paravertebral muscle contracture, and bilateral lumbar radiculopathy, with sudden onset after lifting effort. The preoperative diagnosis was done using computed tomography (CT) and magnetic resonance imaging (MRI), and the patient underwent surgery—two level laminectomy, dural incision, and tumor dissection from the cauda equina nerve roots under operatory microscope. Histopathological examination confirmed the positive diagnosis of cavernoma of cauda equina. The patient''s outcome was favorable, without postoperative neurological deficits. 相似文献

The exact origin and precise morphologic explanation of positive sharp waves (PSWs) are presently lacking. Observing normal needle electromyographic insertional activity reveals two types of waveforms: (1) biphasic negative/positive spikes, and (2) positive spikes followed by a small negative phase. In the end-plate region, it is possible to occasionally observe a biphasic end-plate spike transform into a monophasic positive end-plate waveform. It is postulated that this waveform is simply a form of intracellular recording for the biphasic end-plate spike or a form of extracellularly recorded but blocked single muscle fiber discharge. Similarly, the observed monophasic positive insertional activity may be an intracellularly recorded single muscle fiber discharge or a blocked extracellular discharge originating about the needle electrode. Applying this reasoning to PSWs suggests that they may also be an intracellular recording of a fibrillation potential, or needle-induced extracellular blocked local single muscle fiber discharge. The recording characteristics of the monopolar needle in three dimensions have not been well established. A simple spherical recording territory is commonly assumed with the very tip proposed to have a greater spatial recording sensitivity by some authors. We demonstrate, by enlarged physical modeling in a homogeneous volume conductor, that the recorded amplitude diminishes more gradually radially away from the conical surface than distally past the tip or proximal to the insulation edge. The sensitivity over the exposed metallic surface is found to be uniformly proportional to the area, which results in relatively less sensitivity at the tip than the middle and proximal portions of the conical recording surface. The overall spatial amplitude recording characteristics can be better described by an apple shape than a sphere, centered at the midportion of the exposed conical surface. A better appreciation of the actual spatial recording characteristics of the monopolar needle electrode can result in more accurate physiologic interpretations of quantitative motor unit analysis. © 1996 John Wiley & Sons,Inc. 相似文献

Recent research on the pathogenesis of spondyloarthritis and related immune-mediated diseases associated with human leukocyte antigen class I molecule B27 (HLA-B27) has led to significant progress in terms of management and prognosis, with multiple treatments being constantly evaluated and implemented. Correlations between the genetic background of spondyloarthritis and inflammatory bowel diseases and the inflammatory processes involving gut microbiota have been established. This knowledge has allowed progress in pharmacological therapy. The role of diet in the pathogenesis and treatment of diseases pertaining to the HLA-B27 spectrum is of great significance, considering possible future applications in individualized medicine. Diet impacts the composition of gut microbiota, representing a substrate for the synthesis of metabolites affecting the mucosal immune system. Certain pro-inflammatory mediators, such as emulsifiers and microparticles, induce a more profound cytokine response, promoting inflammation. Numerous diets, including the low-starch diet, the Mediterranean diet, diets with low contents of fermentable oligosaccharides, disaccharides, monosaccharides and polyols (low-FODMAP diets), gluten-free diets and fasting, have been analysed and correlated with patients’ symptomatology and dietary adherence.
