Evaluation of the micro-erythrocyte sedimentation rate in relation to hemoglobin and hematocrit in newborn infections

The reliability of micro-erythrocyte sedimentation rate (mESR) was studied for its value in the diagnosis of infections in 37 healthy newborns and in 37 with infections. The mESR values amount to 5.1 +/- 4.3 mm/h in healthy newborns. In newborns with infection, mESR values were found to be clearly higher, that is 22.7 +/- 9.9 mm/h. An important relationship between that of the mESR value and the hemoglobin (Hb) and packed cell volume (PCV) levels was revealed. For evaluation of the diagnostic value of the parameters of infection that have been studied, the Odds ratio was used. As with other similar inflammation indices, mESR should always be interpreted in combination with clinical findings in newborns with infections.     

Typhlitis complicating induction therapy in adult acute myeloid leukemia

In a retrospective analysis of 161 consecutive adult patients with de novo acute myeloid leukemia undergoing induction therapy, including cytarabine, etoposide and anthracyclines, seven patients (4.3%) developed typhlitis. All presented severe neutropenia, fever, abdominal pain and tenderness within 16 days from starting chemotherapy (median 11 days; range 5-16). Three patients underwent surgery and survived, four were treated only with supportive therapy: two recovered and two died. In our experience early recognition of typhlitis and rapid recovery of the neutrophils are the most important determinants of the results of surgical and/or medical approaches. The management of typhlitis, a life-threatening condition, is controversial and depends on many factors characterizing each patient, which must be evaluated in collaboration between the surgeon and the hematologist.     

Association between duration of neonatal hospital stay and morbidity in the first month of life

The morbidity of 506 healthy full-term newborns was studied in the first month of life in relation to the time they stayed in the hospital. The average time for the newborns who were born by vaginal delivery was 73.3+/-11.7 hours, while for those who were born by cesarean section it was 135+31.5 hours. Thirty-seven newborns presented health problems during the neonatal period (7.3%) and only 2% needed hospital readmission. The commonest problem in the newborns we studied was jaundice which appeared from the fourth to sixth day of life. During the second fortnight the commonest problems were infections of the respiratory tract. From the results of our study it is obvious that only a small percentage of readmissions could have been avoided if the original stay in hospital had been prolonged.     

Mizolastine in the treatment of seasonal allergic rhinoconjunctivitis: a European clinical experience with 5408 patients managed in daily practice (PANEOS SAR Study)

BACKGROUND: Mizolastine, a potent H1 antihistamine with additional antiallergic properties, is marketed for the treatment of allergic rhinoconjunctivitis and urticaria. The objective was to investigate the safety and effectiveness of mizolastine under conditions of daily practice in patients with seasonal allergic rhinoconjunctivitis (SAR). METHODS: In an open multicenter study, mizolastine 10 mg daily was administered for 14 days during the pollen season. Nasal and ocular symptoms, time to onset of symptom relief, and effect of the drug on diurnal alertness were evaluated. Safety was evaluated on the basis of self-reported adverse events (AE). RESULTS: A total of 5408 patients (36+/-14 years of age, females=57%) with a history of SAR for 8+/-9 years were treated for a mean of 17.1+/-5.0 days. SAR symptoms improved in 93% and decreased by at least 50% in 86% of patients; 78% reported improvement after the first drug intake and 51% from the first hour. Sixty-nine percent considered mizolastine more effective than other antihistamines taken previously. The incidence of AE was low (3.8%). CONCLUSION: The high responder rate, the rapid onset of action, and the low incidence of AE observed in this large multicenter study confirm the previously reported beneficial efficacy and safety of mizolastine in the management of SAR.     

Lecithin therapy of hereditary ataxia

Eight patients with Friedreich's ataxia and eight others with syndrome of spino-cerebellar degeneration received oral lecithin (21 g daily) for a six-month period. No relevant clinical change was note either during or soon after treatment. Statistical analysis was also irrelevant in the total number of patients. Similar results emerged when grouping the patients at the stage of the illness (stage II in contrast with stage III-IV). The outcome of therapeutical trials in literature is discussed in comparison with results.     

"Syndrome in syndrome": Wernicke syndrome due to afferent loop syndrome. Case report and review of the literature

Wernicke syndrome is a rare neurological pathology due to a deficit in vitamin B1. The syndrome is common among alcohol abusers, patients with malignant tumor or gastrointestinal diseases, those who undergo hemodialysis or long-term peritoneal dialysis, pregnant women with hyperemesis, women who breast-feed, patients with hyperthyroidism or anorexia nervosa or gastric or jejunal-ileal bypass surgery for obesity, patients submitted to gastric surgery or prolonged total parenteral nutrition or prolonged intravenous therapy. We report a case of Wernicke syndrome due to afferent loop syndrome characterized by incoercible vomiting.
