Background: The feasibility study described herein was prompted by a report in 1992 of possible reproductive and developmental health
concerns among female workers in a Russian nickel refinery. Objective: The primary goal was to ascertain whether medical, statistical, and occupational data bases could be accessed for information
about the pregnancy histories, occupational histories, and life-style factors of the women affected. Methods: The project was facilitated by construction of a registry of all births in three towns with a nickel refinery and verification
of its contents against patients' records obtained from hospital delivery and gynecology departments and community polyclinics.
Municipal Registration Board, Regional Health Statistics Board, and nickel company records were also reviewed. Results: Reproductive/developmental outcome information and workplace histories were acceptable. Sample-size calculations indicated
that a cohort or cross-sectional study would be amenable and suitable for the detection of an excess risk for spontaneous
abortion with adequate statistical significance and power. Such investigations would need to be supplemented by workplace
environmental/biological monitoring assessments for evaluation of exposure to occupational hazardous factors and a worker's
questionnaire to obtain information about life-style factors. A case-control design is recommended for the study of congenital
defects. Conclusions: A well-designed, comprehensive epidemiology study is technically feasible because of the availability of a favorable pool
of study subjects, reproductive/developmental outcome data, information to control for major confounders, and suitable occupational
records.

PURPOSE: Determine whether standard or high-dose chemotherapy leads to changes in fatigue, hemoglobin (Hb), mental health, muscle and joint pain, and menopausal status from pre- to post-treatment and to evaluate whether fatigue is associated with these factors in disease-free breast cancer patients. PATIENTS AND METHODS: Eight hundred eighty-five patients were randomly assigned between two chemotherapy regimens both followed by radiotherapy and tamoxifen. Fatigue was assessed using vitality scale (score < or = 46 defined as fatigue), poor mental health using mental health scale (score < or = 56 defined as poor mental health) both of Short-Form 36, muscle and joint pain with Rotterdam Symptom Checklist, and Hb levels were assessed before and 1, 2, and 3 years after chemotherapy. RESULTS: Fatigue was reported in 20% of 430 assessable patients (202 standard-dose, 228 high-dose) with at least a 3-year follow-up, without change over time or difference between treatment arms. Mean Hb levels were lower following high-dose chemotherapy. Only 5% of patients experienced fatigue and anemia. Mental health score was the strongest fatigue predictor at all assessment moments. Menopausal status had no effect on fatigue. Linear mixed effect models showed that the higher the Hb level (P = .0006) and mental health score (P < .0001), the less fatigue was experienced. Joint (P < .0001) and muscle pain (P = .0283) were associated with more fatigue. CONCLUSION: In 3 years after treatment, no significant differences in fatigue were found between standard and high-dose chemotherapy. Fatigue did not change over time. Objective
Investigate the effects of disease management program (DMP) implementation on physical activity, smoking, and physical quality of life among chronically ill patients.Methods
This study used a mixed-methods approach involving qualitative (35 interviews with project managers) and quantitative (survey of patients from 18 DMPs) data collection. Questionnaire response rates were 51% (2010; 2619/5108) at T0 and 47% (2011; 2191/4693) at T1.Results
Physical activity and the percentage of smokers improved significantly over time, whereas physical quality of life declined. After adjusting for patients’ physical quality of life at T0, age, educational level, marital status, and gender, physical activity at T0 (p < 0.01), changes in physical activity (p < 0.001), and percentage of smokers at T0 (p < 0.05) predicted physical quality of life at T1. Project managers reported that DMPs improved patient–professional interaction. The ability to set more concrete targets improved patients’ health behaviors.Conclusions
DMPs appear to improve physical activity among chronically ill patients over time. Furthermore, (changes in) health behavior are important for the physical quality of life of chronically ill patients.Practice implications
Redesigning care systems and implementing DMPs based on the chronic care model may improve health behavior among chronically ill patients. 相似文献7.

The aim of this work was to delineate the effects of chronic ingestion of strontium 90 (90Sr) at low concentrations on the hematopoiesis and the bone physiology. A mouse model was used for that purpose. Parent animals ingested water containing 20 kBq l?1 of 90Sr two weeks before mating. Offspring were then continuously contaminated with 90Sr through placental transfer during fetal life, through lactation after birth and through drinking water after weaning. At various ages between birth and 20 weeks, animals were tested for hematopoietic parameters such as blood cell counts, colony forming cells in spleen and bone marrow and cytokine concentrations in the plasma. However, we did not find any modification in 90Sr ingesting animals as compared with control animals. By contrast, the analysis of bone physiology showed a modification of gene expression towards bone resorption. This was confirmed by an increase in C‐telopeptide of collagen in the plasma of 90Sr ingesting animals as compared with control animals. This modification in bone metabolism was not linked to a modification of the phosphocalcic homeostasis, as measured by calcium, phosphorus, vitamin D and parathyroid hormone in the blood. Overall these results suggest that the chronic ingestion of 90Sr at low concentration in the long term may induce modifications in bone metabolism but not in hematopoiesis. Dendritic cells (DCs) have the unique ability to initiate primary immune responses, and they can be conditioned for vaccinal purposes to present antigens after the engulfment of apoptotic cells. To recruit the rare antigen-specific naive T cells, DCs require a maturation step and subsequent transport toward lymph node (LN). To date, prostaglandin E2 (PGE2) is the best-characterized compound inducing this LN-directed migration in vitro, but PGE2 may skew the immune responses in a T(H)2 direction. We demonstrate here that on incubation with apoptotic tumor cells and tumor necrosis factor-alpha (TNF-alpha) or lipopolysaccharide (LPS), human monocyte-derived DCs become fully mature and acquire high migratory capacities toward LN-directing chemokines. The migration of TNF-alpha-treated DCs occurs only after cotreatment with apoptotic cells but not with necrotic cells. DC migration requires CD36 expression and incubation with apoptotic cells in the presence of heat-labile serum components. Moreover, on treatment with apoptotic cells and LPS, the migrating DCs are able to recruit naive T cells to generate T(H)1 immune responses. Our results show that the cotreatment of DCs with apoptotic tumor cells and inflammatory signals is promising for the design of an antitumoral DC-based vaccine. 相似文献

The effects of occupational exposure to organic solvents in pregnancy on foetal growth are still unclear. Our aim was to study whether live newborns to women employed in paid jobs with frequent exposure had a different risk of being born with low birth weight (LBW), compared to those of women in jobs without such exposure. The study population was all singleton newborns delivered in the industrial township of Mon?egorsk (N?=?26,415). Information about occupation and characteristics of the mothers and babies was obtained from the local population-based birth register, and registered job function was used to classify exposure. We observed an elevated risk of LBW among live, singleton newborns in the exposed group (adjusted odds ratio: 1.68 [95% CI: 1.18–2.41]), which predominantly consisted of painters. The adjusted odds of LBW in the exposed group were also higher among term-born neonates. In addition, a lower mean birth weight was observed among the exposed. 相似文献

The authors investigated the effect of residues from copper- and gold-mining on the Cree population of Oujé-Bougoumou, located 560 km north of Quebec City, Canada. Subjects (225) from Oujé-Bougoumou and a control population (100) completed a questionnaire on lifestyle and dietary habits and provided blood and urine samples for analysis. Geometric means of arsenic, lead, cadmium, and copper concentrations were not significantly different for subjects or controls 15 yr and older or children (8-14 yr old). However, blood zinc was higher and selenium was lower in Oujé-Bougoumou samples. Mean blood lead level was higher in children from Oujé-Bougoumou, but lower in adults aged 40 yr and older. For adults (15 yr and older) blood lead level increased with age and was higher in men, those who hunted, and consumed wild meat (R2 = 0.43). Blood cadmium increased with age and smoking (R2 = 0.61). No influence of mine residues was observed among residents of Oujé-Bougoumou, but lifestyle exposure associations were noted for both communities. 相似文献
