ABSTRACT

Human immunodeficiency virus (HIV) and Tuberculosis (TB) are two main global public health threats that dent development in low and middle-income countries. This study evaluated the HIV/TB co-infection rate among HIV-1 infected individuals in old Cross River State, Nigeria. A total of 417 HIV-infected individuals participated in this study, 241 (57.8%) from Calabar, Cross River State, Nigeria and 176 (42.2%) from Uyo, Akwa-Ibom State, Nigeria. The age range of the 417 HIV-1 positive individuals who participated in the study was 4–72 years with an average age of 39.1 years. Plasma samples were analyzed for HIV and TB using fourth-generation Enzyme-Linked immunosorbent Assay. The CD4 count was enumerated using the Partec CyFlow® Counter. Plasma viral loads (PVL) were determined using the Abbott Real-Time HIV-1 assay. Results showed that 230 (55.2%) of the participants were in the 31–45 years age range. The majority (67.4%) of the HIV-1 infected individuals were females and 32.6% were males. An overall prevalence of HIV/TB coinfection in Old Cross River State, Nigeria was 1.4%, with Akwa Ibom State (0.6%) and Cross River State (1.2%). A higher prevalence of HIV/TB coinfection was observed among females (1.8%) than in males (0.7%). Higher prevalences of HIV/TB coinfections was observed in patients above 45 years of age (2.2%), married (2.3%), tertiary education (1.8%) followed by those with secondary education (1.4%), traders and civil servants (3.1%), patients with CD4 counts 200–349 and ≥500 cells/μl (1.9%), and those with viral load <40 copies/mL (2.7%). This study confirmed the presence of HIV/TB co-infection in old Cross River State, Nigeria. Although the prevalence rate of HIV/TB coinfection was low, its presence alone among HIV-1 infected individuals makes it a major source of concern. This finding highlights the need for a well-structured approach to the management of co-infection, and this includes both the social and medical aspects of the problem.  相似文献

The skeletal muscle relaxant properties of an aqueous extract of Portulaca oleracea were examined on the twitch and tetanus tension evoked by electrical stimulation using the rat phrenic nerve-hemidiaphragm and frog sciatic nerve-sartorius muscle preparations and on contractures induced by nicotinic agonists using the rat rectus abdominis muscle preparation. The extract (5-50 X 10(-4) g/ml) produced a dose-dependent initial enhancement, followed by a longer lasting depression of twitch tension as induced by indirect electrical stimulation (NS) as well as direct stimulation (MS) of the diaphragm and sartorius muscle preparations. The augmentation of twitch amplitude (MS) produced by the extract was not significantly antagonised or potentiated by d-tubocurarine (5-50 X 10(-8) M) or physostigmine (5-50 X 10(-8) M) at concentrations which blocked or potentiated NS-induced twitch contractions. Furthermore, tetrodotoxin (5 X 10(-7) g/ml) alone or in combination with d-tubocurarine did not significantly attenuate extract-induced augmentation of twitch contractions to MS. In addition, the depression of twitch tension due to NS was not antagonised by physostigmine but was reversed and/or abolished by Ca2+ (2-5 X 10(-3) M) or potassium thiocyanate (1 X 10(-3) M). Contractures induced by K+ (80 mM) or by tetanic stimulation (20-60 Hz) were significantly reduced by the extract whereas, contracture induced by caffeine (2.5-6.0 mM) was not affected. Ca2+-free Tyrode's solution and EDTA (1.25 X 10(-3) g/ml) potentiated extract-induced depression of twitch tension to MS. On the rectus abdominis, contractures induced by nicotinic agonists (acetylcholine, 3-500 X 10(-7) M; carbachol, 5-500 X 10(-7) M and nicotine, 5-500 X 10(-8) M) were significantly attenuated and/or abolished by the extract (5-10 X 10(-3) g/ml). These observations indicate that the aqueous extract possesses unique skeletal muscle relaxant properties which do not appear to involve interference with cholinoceptor mechanism(s). It appears that the mechanism of action of the extract may involve interference with Ca2+ mobilization in skeletal muscle.  相似文献

This article is a concise summary of intra-abdominal hypertension (IAH) and abdominal compartment syndrome (ACS) with an emphasis on factors relevant to their occurrence in children. It discusses the limitations in the direct application of the current World Society of Abdominal Compartment Syndrome consensus definitions and extrapolation of management practices derived from studying adult patients to the pediatric age group. Techniques that may be used for measuring intra-abdominal pressure (IAP) in children, normal IAP ranges, risk factors for developing ACS as well as current medical and surgical management options in children are discussed.  相似文献

ObjectiveTo determine the prevalence, sex-age related intensity of urinary schistosomiasis and to compare such parameters among rural school children and rural farmers in selected communities in Obollo-Eke located in Southeast, Nigeria.MethodsA cross-sectional survey involving 1 337 school children and farmers was conducted in Obollo-Eke community between September 2006 and July 2007. Demographic data of subjects was collected using a questionnaire prepared for this purpose. Urine samples were collected and examined for haematuria and ova of Schistosoma haematobium (S. haematobium) using Medi-test Combi 9 and sedimentation technique respectively.ResultsThe prevalence of urinary schistosomiasis based on microscopic examination of the urine sediment for the ova of S. haematobium was 17.5% while the prevalence of haematuria was 15.6%. Infection intensity varied from light to heavy. In general, the prevalence was higher among males (20.8%) than females (14.6%; P>0.05) and was slightly higher among primary school children (18.0%; n=762) than farmers (16.9%; n=575; P>0.05). The age-specific prevalence of schistosomiasis among the study subjects ranged from 8.3% to 21.2% in 0-5 years and 11-15 years age groups respectively.ConclusionsHaematuria and mean egg/10 mL urine (r = 0.95; P<0.01) showed that both procedures are reliable for the diagnosis of the disease and can be used to ascertain the prevalence of the disease in any community. The comparative analysis of urinary bilharziasis among primary school children and rural farmers demonstrated that the infection is moderately high in these two risk population groups at Obollo-Eke. Detecting adolescents with hypertension and prehypertension will aid early intervention and reduce morbidity and mortality from the disorders.
