Obstetric emergencies

For more than 60 years the Confidential Enquiries into Maternal Deaths triennial reports have helped build a picture of maternity care within the UK, highlighting not only our successes but failures in caring for women within the puerperal period. Despite most obstetric emergencies being well described and having clear management strategies and guidance, there continues to be substandard management with poor outcomes recorded and this has been highlighted within the latest triennium report. This article describes some common obstetric emergencies with which the anaesthetist will become involved. It emphasizes management related to some deficiencies identified in the Centre for Maternal and Child Enquiries report as well as highlighting a multidisciplinary approach throughout. Good communication between team members is paramount in all aspects of medical care, but this approach should be fostered routinely to ensure that rapid and appropriate decisions are made in a safe and timely manner.     

应用健康信念模式改善急诊病人焦虑心理的临床研究

目的探讨应用健康信念模式进行健康教育对改善急诊病人入院后的焦虑状态的作用。方法将80例急诊病人按入院顺序分为观察组和对照组,各40例。观察组采用健康信念模式对病人进行针对性的健康教育。对照组采用常规的健康教育。采用Zung焦虑自评量表（SAS）测定病人的焦虑程度。并比较健康教育后两组病人的焦虑程度。结果健康教育后观察组病人的焦虑程度明显低于对照组,有显著性差异（P〈0.01）。结论应用健康信念模式进行健康教育可以有效改善急诊病人的焦虑心理。     

Variability of DKA Management Among Pediatric Emergency Room and Critical Care Providers: A Call for More Evidence-Based and Cost-Effective Care?

Management protocols have been shown to be effective in the pediatric emergency medicine (PEM) and pediatric critical care (PCC) settings. Treatment protocols define clear goals which are achieved with consistency in implementation. Over the last decade, many new recommendations have been proposed on managing diabetic ketoacidosis (DKA). Although no perfect set of guidelines exist, many institutions are developing DKA treatment protocols. We sought to determine the variability between institutions in implementation of these protocols.     

浅谈医院急抢救设备的合理分布和管理

介绍了急抢救设备的管理现状,针对医疗机构急抢救设备管理中普遍存在的问题,结合某院对急抢救设备的管理经验,探讨了急抢救设备合理分布和管理的方法,最大程度地发挥了急抢救设备的效能,进一步提高了危病重患者的存活率,降低了在院患者的死亡率,提升了医院急抢救的水平。     

积极心理学框架下急诊科护士职业幸福感的质性研究

目的从积极情感体验和积极的社会组织系统出发,探寻急诊科护士的职业幸福感。方法采用现象学研究方法,选择芜湖市4所医院的16名急诊科护士进行深入访谈,并用Colaizzi的7步分析法分析资料。结果愉悦感是急诊科护士职业幸福感的直接表现,归属感是急诊科护士职业幸福感的内在感受。结论急诊科护士拥有职业幸福感,但需要采用素质拓展、团队辅导咨询等积极心理干预方法,进一步激发和提升。
