A one-year-old neutered female Labrador Retriever presented with twitches and subsequent generalised tonic–clonic seizure activity following ingestion of mouldy dog food. Further complicating factors were hyperthermia and metabolic acidosis. Initial emergency treatment involved seizure management, blood analysis, intravenous fluid therapy, active cooling and decontamination via gastric lavage, administration of activated charcoal and an intralipid infusion. The patient was maintained under general anaesthesia to manage the seizure activity. Post-anaesthetic management included treatment of aspiration pneumonia, physiotherapy, intravenous fluid therapy and management of an indwelling urinary catheter. The patient was discharged 48 h post-admission without further complication.  相似文献

ObjectiveBaby gates are one of the most widely used home safety products to protect children from home hazards. The objective was to describe the epidemiology of baby gate and barrier-associated injuries among children. It was hypothesized that injuries experienced by children ages ≤2 years and those >2 years were significantly different as a result of differences in gate interactions.MethodsA retrospective analysis was conducted by using nationally representative data from the National Electronic Injury Surveillance System. A total of 1188 actual cases were reviewed and national estimates generated.ResultsAn estimated 37,673 children were treated in emergency departments for injuries associated with gates, yielding an average of 1794 cases annually. The incidence of gate-related injuries increased significantly from 3.9 per 100,000 children in 1990 to 12.5 per 100,000 children in 2010 (P < .001). Patients were primarily boys (61.0%) and were <2 years of age (60.4%). Patients <2 years of age were most often injured by falls down stairs (odds ratio 6.72; 95% confidence interval 6.32–7.16) after the collapse of the gate. Patients aged 2 to 6 were most often injured by contact with the gate (odds ratio 2.03; 95% confidence interval 1.95–2.12), resulting in open wounds (55.4%) and soft-tissue injuries (24.2%).ConclusionsGiven the clear dichotomy between injury characteristics of patients aged <2 years and patients aged 2 to 6 years of age, as well as the prevalence of preventable injuries, greater efforts are needed to promote proper usage, ensure safety in product design, and increase awareness of age-related recommendations for use of gates.  相似文献

Objective

Traditionally, conventional antipsychotics, often administered intramuscularly, are used to reduce hostility and aggression in the emergency setting. This study investigated the efficacy of quetiapine, an oral atypical antipsychotic, in managing aggressive psychosis.

Methods

Adult hospitalized patients (n=36) with an Overt Aggression Scale (OAS) total score ≥1 and a Brief Psychiatric Rating Scale-derived Psychosis Index score ≥6 received 100–800 mg/day flexibly dosed quetiapine and were monitored daily for a maximum of 5 days. The OAS total score and Physical Aggression Against Others subscale score were primary efficacy assessments. Tolerability was assessed with the Udvalg for Kliniske Undersogelser Side Effects Rating Scale.

Results

An 83% decrease in the OAS Physical Aggression Against Others subscale score occurred from Day 1 (baseline) to Day 2, which was generally maintained to Day 5; a 39% reduction in OAS total score was observed by Day 2. The OAS total score decreased from a mean baseline of 3.3 to 1.5 at Day 5, a significant decrease for Days 2–4 (P?<?0.01) and of borderline significance on Day 5 (P=0.059). Adverse events were mostly mild to moderate with concentration difficulties and somnolence the most common.

Conclusion

These results from an uncontrolled trial suggest that quetiapine may be helpful for some aggressive patients in the emergency setting.  相似文献

目的 掌握广州市食物中毒突发公共卫生事件规律和特点,为提高其有效应对和防控提供科学依据。 方法 收集2006—2019年国家“突发公共卫生事件报告管理信息系统” 报告的广州市食物中毒突发公共卫生事件,应用描述性流行病学方法进行统计和分析。 结果 研究的14年间,广州市共报告食物中毒突发公共卫生事件89起,原因查明率为94.38%,中毒人数2 692人,死亡11人。69.66%的事件发生在5—9月。75.28%的事件致病因素是细菌,副溶血弧菌是细菌性食物中毒事件的首要致病菌,63.64%的死亡病例归因植物性食物中毒,78.65%的事件中毒场所在集体食堂。事件持续时间的中位数为13.50 h(范围为:0~135.33 h)。首例病例发生时间到接报的中位数是0.60 d(范围为:0.04~5.69 d),r=0.601,P<0.01。 结论 加强食物中毒事件和食品安全风险监测,尽早介入处置,加强针对性的宣传,落实重点场所防控督导巡查,将有助于减少食物中毒事件发生和危害影响。  相似文献
