目的 通过监测脑血流速度、颈静脉球部血氧饱和度(SjvO2)、血压、心率(HR)和不良反应发生率等,综合评价艾司洛尔用于控制开颅手术苏醒期患者脑过度灌注的效果.方法 选择择期在全身麻醉下行开颅肿瘤切除术的患者,随机分为艾司洛尔组(E组,20例)和对照组(C组,20例).E组患者从拔管即刻起给予艾司洛尔0.6 mg·kg1·h-1静脉持续滴注15 min C组患者从拔管即刻起给予0.9%氯化钠溶液静脉持续滴注.分别监测两组患者术前、术后拔管即刻及拔管后15、30、45、60 min时的大脑中动脉平均血流速度(MCAVm)、SivO2平均动脉压(MAP)及HR,并记录不良反应发生例数.结果 E组术后15和30 min的MCA Vm、sjvO2及HR均显著低于C组(P值均<0.01),术后15 Min的MAP显著低于C组(P<0.05).无一例患者发生与艾司洛尔相关的药物不良反应.结论 艾司洛尔是控制术后脑过度灌注的理想药物.  相似文献

Clinically, phenol is used often as a neurolytic agent to treat pain and spasticity. The purpose of this study was to examine the time course of denervation and recovery in several hindlimb muscles following application of a 5% aqueous solution of phenol to the sciatic nerve. Phenol was applied to the sciatic nerve of adult female rats either by intraneural or perineural injection. Axonal degeneration was evident within the sciatic nerve 2 days following phenol application, although variable amounts of damage were observed. By 2 weeks, the soleus and tibialis anterior had atrophied to 63% and 51% of control. Reinnervation of hindlimb muscles occurred between 2 and 4 weeks following the nerve block. Following denervation, the soleus became slower in that all of the fibers expressed the slow myosin heavy chain (MHC). At 5 months, maximum tension of the soleus was 74% of control and the muscle consisted of more fast fibers on average, some of which expressed IIx MHC. These data suggest that 5% phenol causes an injury to the nerve that is more severe than a crush injury, and that reinnervation of denervated muscles may be by motoneurons other than those that originally innervated the muscles. © 1996 John Wiley & Sons, Inc.  相似文献

A method for rapidly producing velocity images is presented. This sequence combines a modified bipolar gradient pulse to magnitude encode the velocity with the rotating ultra-fast imaging sequence (RUFIS) to image the encoded spins. Velocity encoding is done in 3 msec, and RUFIS acquires 32 projections in 8 msec. The method is applied to turbulent jets associated with a 75% stenosis in a 15-mm inner diameter glass pipe. Data is acquired upstream and downstream from the stenosis for Reynolds numbers from 560 to 3750. In addition, a robust method of reconstructing the unobserved short time region of a free induction decay is presented and incorporated into the image processing.  相似文献

Summary: Despite numerous deficiencies in some antihypertensive drug treatment trials, and some questionable selections of studies for inclusion in several meta-analyses, undoubtedly such trials have shown treatment benefits from reducing hypertension. Complications that can be corrected or prevented include malignant hypertension, hypertensive heart failure, stroke and coronary artery morbidity. the all-cause mortality has been lowered in several trials. the benefits have been seen in subjects aged over 60 years, in women and men, and in patients with isolated systolic hypertension. the benefits have been achieved using a wide range of drugs, not only with beta-blockers or diuretics. Non-pharmacological means of lowering blood pressure have not been evaluated in relation to morbidity.  相似文献

The aim of this study was to investigate the reactivity of the nasal mucosa of patients with pollen allergy compared to healthy controls, when challenged with histamine outside the pollen season. Assessments were made with symptom score, acoustic rhinometry, nasal peak expiratory and inspiratory flow (NPEF and NPIF) and rhinomanometry in order to find the most sensitive method for the purpose. Twenty-one patients with seasonal allergic rhinitis and 20 healthy controls were challenged with histamine dihydrochloride in increasing concentrations (0.01, 0.1, 1.0 mg/ml) locally in the nose. Our results show no difference in mucosal reactivity between the patients and controls regardless of the method used. When comparing the methods we find that NPIF and NPEF are more sensitive to mucosal changes than the other methods we have studied.  相似文献

通过对临床37例肾移植的观察,发现术中移植肾开放血流后肾血管痉挛现象与当时血压明显相关。收缩压大于24kPa者,移植肾血管痉挛率为53.33%,收缩压低于24kPa者,移植肾血管痉挛率为9.09%。两组有显著性差异(P<0.01).发生血管痉挛的肾脏排尿时间较未痉挛者明显延长,分别为23±6.35min.3.8±0.86min,两组差异极显著(P<0.01).从而认为肾移植术中提升血压应适度,并针对移植肾血管痉挛的原因进行了讨论。  相似文献

Summary Intracranial pressure, arterial blood pressure, and middle cerebral artery blood velocity were monitored for periods from 1 to 10 days in 30 neurosurgical intensive care patients. The recordings revealed rapid changes in the cerebral perfusion and gave insight into individual cerebral haemodynamic states.Twenty patients consistently showed CO2 reactivity within normal limits, between 2.5 and 5% per mm Hg (19–38% per kPa). Severely impaired CO2 reactivity, considerably below 1% per mm Hg (7.5% per kPa) was observed in four patients. Three of these patients died, while the fourth patient survived in a persistent vegetative state.Seven patients demonstrated pressure-passive blood velocity changes throughout the observed CPP range. The four patients with severely impaired CO2 reactivity all belonged to this group. The recordings from three of the remaining 23 patients showed signs of MCA blood velocity autoregulation with a lower regulatory limit of about 40–45 mm Hg. This observation is in keeping with findings from electromagnetic flowmetry on brain arteries in the neurosurgical operating field, and supports blood velocity measurements as a relevant index of brain perfusion in clinical neurosurgery and neuro-intensive care settings.  相似文献

A set-up for D.C. recordings of slow ocular potentials such as the c-wave of the electroretinogram (ERG) as well as the fast oscillation (FO), the light peak (LP) and the dark trough (DT) in both clinical and experimental work is described. It includes matched calomel half-cells connected by saline-agar bridges to a corneal contact lens on the eye and a reference chamber on the forehead, a low-drift differential-input D.C. amplifier, an A/D converter, a computer, a thermoprinter, a flexible disc memory, a plotter, and a device for light stimulation controlled by the computer.Examples of the usefulness of the set-up in clinical work are shown in the form of D.C. c-wave ERGs of normal subjects as well as of patients with vitelliform macular degeneration, choriocapillaris atrophy, and retinitis pigmentosa. The direct corneal recording of the FO and LP is demonstrated as well. The different origins of the standing potential (SP) of the eye, the ERG c-wave, the FO and the LP are reviewed briefly.  相似文献
