目的:对照研究连续硬膜外麻醉(CEA)和全身麻醉下人工全膝关节置换术(TKA)后深静脉血栓(DVT)的发生率,并探析其各自的临床意义。方法:分析2013年4月~2015年5月在我院接受治疗的68例患者的临床资料。将入选者随机分成 CEA 组和全麻组两组,各34例。比较两组患者的一般资料、术前术后的相关指标以及术前凝血、血小板相关指标以及 DVT 的发生率。结果:两组患者的一般资料无统计学差异。两组患者术中、术后的相关指标间差异均不明显。两组患者术前凝血与血小板比较无统计学差异。DVT 的总发生率为19.1%(13/68),其中单纯远端血栓有12例,近端血栓有1例,无肺栓塞及髂静脉血栓患者。CEA 组的 DVT 发生率明显低于全麻组。结论:CEA 可以显著降低 TKA 后 DVT 的发生率,疗效显著。 相似文献

Bilateral proximal deep vein thrombosis (DVT) in the lower extremities of young patients should raise suspicion over pro‐thrombotic conditions and venous anatomical abnormalities, even in the presence of other precipitating factors, such as viral infection. 目的:探讨小儿烧伤并发惊厥的原因及防治措施。方法:回顾性分析26例烧伤并发惊厥患儿的临床资料,并进行总结。结果:本组25例治愈,1例死亡。惊厥时间在1 min内控制者15例,在2-4min控制者7例,3例惊厥反复发作6-11h,随烧伤病情好转症状消失,1例死于败血症所致中毒性脑炎。结论:在正确处理烧伤创面的同时,及时抗休克、抗感染治疗,预防高热,积极纠正水、电解质、酸碱失衡,可有效防治小儿烧伤惊厥。 相似文献

目的 观察烧伤小鼠干扰素诱导表达蛋白IFIT1的表达变化,初步探究其表达变化的原因.方法 实验小鼠致TBSA 15%Ⅱ度烧伤,1 d及7 d后处死,取肝、肺及脾组织.脂多糖掺入小鼠腹腔巨噬细胞株RAW264.7的培养基中,至终浓度0.1~1.0μg/ml,作用6 h.提取组织及细胞RNA,进行半定量逆转录-聚合酶链式反应,提取组织及细胞裂解上清,进行免疫印迹检测.结果 小鼠肝、肺、脾组织IFIT1的转录在烧伤后1 d升高,伤后7 d恢复至伤前水平;肝、肺、脾IFIT1的蛋白水平在伤后1 d一致升高,伤后7 d仍能检出升高.体外实验,细菌脂多糖显著激活RAW264.7细胞的IF-IT1转录及蛋白表达.结论 烧伤小鼠伤后早期IFIT1表达迅速升高,此变化与同期的内毒素血症引起的细胞应激有关. 相似文献

Titanium alloys have high specific strength and excellent corrosion resistance and have been applied in deep-sea engineering fields. However, stress corrosion cracking may become one of the biggest threats to the service safety of a high-strength titanium alloy, as well as its weldment. In this work, stress corrosion cracking of a gas-tungsten-arc-welded Ti-6Al-3Nb-2Zr-1Mo (Ti6321) alloy influenced by the applied potentials in simulated deep-sea and shallow-sea environments was investigated by combining slow strain rate testing with electrochemical measurements. The results showed that the service environment and applied potential have a substantial effect on the stress corrosion cracking behavior of the Ti6321 welded joint. The Ti6321 welded joint exhibited higher stress corrosion susceptibility in a simulated deep-sea environment and at a strong polarization level owing to the diminishing protection of the passive film under passivation inhibition and the enhancement of the hydrogen effect. The fracture of a Ti6321 welded joint in the weld material could be attributed to the softening effect of the thick secondary α within the coarse-grained martensite. The electrochemical evaluation model of stress corrosion cracking susceptibility of a Ti6321 welded joint in a simulated marine environment was established by adding the criterion in the passivation region based on the literature model, and four potential regions corresponding to different stress corrosion cracking mechanisms were classified and discussed. Our study provides useful guidance for the deep-sea engineering applications of Ti6321 alloys and a rapid assessment method of stress corrosion risk. 相似文献

BackgroundThe coronavirus disease 2019 (COVID‐19) is a newly recognized illness that has spread rapidly all over the world. More and more reports highlight the risk of venous thromboembolism (VTE) in COVID‐19. Our study aims to identify in‐hospital VTE risk and bleeding risk in COVID‐19 patients.MethodsWe retrospectively studied 138 consecutively enrolled patients with COVID‐19 and identified in‐hospital VTE and bleeding risk by Padua Prediction Score and Improve bleed risk assessment model. The clinical data and features were analyzed in VTE patients.ResultsOur findings identified that 23 (16.7%) patients with COVID‐19 were at high risk for VTE according to Padua prediction score and 9 (6.5%) patients were at high risk of bleeding for VTE prophylaxis according to Improve prediction score. Fifteen critically ill patients faced double high risk from thrombosis (Padua score more than 4 points in all 15 [100%] patients) and hemorrhage (Improve score more than 7 points in 9 [60.0%] patients). Thrombotic events were identified in four patients (2.9%) of all COVID‐19 patients. All of them were diagnosed with deep vein thrombosis by ultrasound 3 to 18 days after admission. Three (75.0%) were critically ill patients, which means that the incidence of VTE among critically ill patients was 20%. One major hemorrhage happened in critically ill patients during VTE treatment.ConclusionCritically ill patients with COVID‐19 suffered both a high risk of thrombosis and bleeding risks. More effective VTE prevention strategies based on an individual assessment of bleeding risks were necessary for critically ill patients with COVID‐19. 相似文献
