目的 描述系统性红斑狼疮患者症状群的构成,并运用网络分析探索群内症状之间的关系,为症状管理提供依据。方法 采取便利抽样原则选取201例系统性红斑狼疮患者为研究对象。采用一般资料调查表、系统性红斑狼疮症状清单对患者进行调查。症状群的提取采用探索性因子分析,以JASP软件绘制网络分析图及各症状中心指标图,分析群内症状之间的关系。结果 系统性红斑狼疮患者疲劳的发生率最高(64.7%),共提取5个症状群:疲劳相关症状群、体质量增加相关症状群、瘙痒-疼痛相关症状群、皮肤改变症状群、身体形象症状群。经过网络分析,对太阳光过敏在所有集群中的强度和紧密度最高,情绪改变的中介度最高。结论 系统性红斑狼疮患者存在症状群,各症状的影响强度不同。临床医护人员需密切关注患者症状,通过改善核心症状来改变群内与之相关的其他症状,进一步提高患者的生活质量。  相似文献

ObjectiveTo investigate the effects of hand–foot syndrome (HFS) and fatigue on disease progression and survival in patients treated with sorafenib followed by regorafenib for advanced hepatocellular carcinoma.MethodsA retrospective analysis of patients with advanced hepatocellular carcinoma treated with sorafenib in our hospital from 1 October 2018 to 31 October 2021 was performed, and clinical and pathological data and follow-up results were obtained. Patients were divided into groups according to the severity of HFS and fatigue. Survival analysis among the groups was performed using the Kaplan–Meier method, continuous variables were analyzed using the t-test, and factors associated with survival were evaluated using multivariate Cox regression analysis.ResultsThe study included 150 men and 23 women with a mean age of 60.77 years (range: 40–85 years). The median overall survival (OS), progression-free survival (PFS), and time to tumor progression (TTP) increased with increasing severity of HFS. Conversely, the median OS, PFS, and TTP decreased with increasing severity of fatigueConclusionHFS and fatigue were independent risk factors affecting TTP, PFS, and OS among patients treated with sorafenib followed by regorafenib for advanced hepatocellular carcinoma.  相似文献

Severe shot peening (SSP) was used on additive manufactured 316L by laser powder bed fusion. The effect of the post processing on the surface features of the material was analyzed through residual stress measurements, tensile testing, hardness-depth profiles, and fatigue testing by flexural bending. The results showed that SSP can be utilized to form residual stresses up to −400 MPa 200 μm below the surface. At the same time, a clear improvement on the surface hardness was achieved from 275 HV to near 650 HV. These together resulted in a clear improvement on material strength which was recorded at 10% improvement in ultimate tensile strength. Most significantly, the fatigue limit of the material was tripled from 200 MPa to over 600 MPa and the overall fatigue strength raised similarly from a low to high cycle regime.  相似文献

The Smith-Watson-Topper parameter (SWT) in its original form was designed to estimate the fatigue life of metal materials in a uniaxial load state (tension–compression) in the range up to fatigue crack initiation, with non-zero mean values. This parameter is based on the analysis of both stress and strain. Therefore, the stress–strain criterion is the focus, rather than the energy criterion. This paper presents the original SWT model and its numerous modifications. The first part presents different versions of this parameter defined by the normal parameters. Then, it presents versions defined through the tangent parameter and the most promising parameter defined through the tangent and normal parameters. It was noted that the final form of the equivalent value is defined either by stress or by an energy parameter. Therefore, the possible characteristics from which the fatigue life can be determined are also presented.  相似文献

The fatigue properties of neat polyimide and the “polyimide + 10 wt.% milled carbon fibers + 10 wt.% polytetrafluoroethylene” composite were investigated under various cyclic loading conditions. In contrast to most of the reported studies, constructing of hysteresis loops was performed through the strain assessment using the non-contact 2D Digital Image Correlation method. The accumulation of cyclic damage was analyzed by calculating parameters of mechanical hysteresis loops. They were: (i) the energy losses (hysteresis loop area), (ii) the dynamic modulus (proportional to the compliance/stiffness of the material) and (iii) the damping capacity (calculated through the dissipated and total mechanical energies). On average, the reduction in energy losses reached 10–18% at the onset of fracture, whereas the modulus variation did not exceed 2.5% of the nominal value. The energy losses decreased from 20 down to 18 J/m3 (10%) for the composite, whereas they reduced from 30 down to 25 J/m3 (17%) for neat PI in the low-cycle fatigue mode. For high-cycle fatigue, energy losses decreased from 10 to 9 J/m3 (10%) and from 17 to 14 J/m3 (18%) for neat PI and composite, respectively. For this reason, the changes of the energy losses due to hysteresis are of prospects for the characterization of both neat PI and the reinforced PI-based composites.  相似文献

This paper aims to investigate the fatigue cracking evolution of cold recycled asphalt mixtures with asphalt emulsion (CRME) under different curing times. The fatigue cracking model of CRME based on damage mechanics and fracture mechanics was analyzed according to the fatigue loading curve. Firstly, the fatigue cracking evolution of CRME was studied through an SCB strength test and SCB fatigue test. Then, the fatigue damage mechanics were used to establish a nonlinear fatigue cracking model, and the damage degree of CRME at the initial cracking point was determined. The Paris formula was used to characterize the law of fatigue crack propagation. Finally, the microstructure of CRME was observed by scanning electron microscopy (SEM) with the backscattering method. The results indicate that the initial cracking point appears at around 60% of the fatigue life according to the SCB fatigue test by means of image analysis. The damage variable was obtained through the cracking model, and the value of the damage variable was determined as 0.06–0.17 at the initial cracking point. In addition, the Paris formula showed that the crack growth of CRME can be reflected by the stress intensity factor and correlative parameters. Moreover, cement hydration products were mixed with the asphalt membrane to form a denser spatial structure during the curing process, which may provide higher fatigue performance of CRME. This research may provide a theoretical reference for studying the fatigue cracking behavior of CRME.  相似文献

Fracturing pumpheads are typical pressure vessels that experience frequent fatigue failure under the effect of notches in their cross-bore. To enhance the fatigue life of fracturing pumpheads, the study of the notch effect is indispensable and important to establish a reliable mathematical model to predict their fatigue life. In the present paper, two novel fatigue life prediction models are proposed for notched specimens. In these models, two new geometric fatigue failure regions are defined to improve the weight function. Finally, the elaborated novel stress-field intensity approach was applied to three different types of notched specimens. Experiment results indicate that the new SFI approach achieves 47.82%, 39.48%, and 31.85% higher prediction accuracy than the traditional SFI approach, respectively. It was found that the modified SFI approach provided better predictions than the traditional SFI approach and the TCD method. The II-th novel SFI approach had the highest accuracy, and the I-th novel SFI approach was more suitable for sharply notched specimens.  相似文献

目的 采用Meta分析的方法评价临床护士心理弹性与共情疲劳的相关性。方法 检索美国科学引文索引网络版数据库(Web of Science)、世界医学文献数据库(PubMed)、Ovid电子期刊全文数据库、EBSCO护理学全文学术期刊(CINAHL)、中国生物医学文献数据库、中国知网(CNKI)、万方数据库和维普全文电子期刊,检索时限为2001年1月—2021年10月,查找关于临床护士心理弹性与共情疲劳相关性的文献。由2名研究人员根据纳入、排除标准独立筛选、评价文献。提取资料后,使用RevMan 5.2进行Meta分析。结果 共计纳入4项研究,文献质量中等。Meta分析结果显示,护士心理弹性与共情疲劳合并相关系数为-0.22,亚组分析显示,ICU护士心理弹性与共情疲劳水平相关系数高于全院护士。结论 临床护士的心理弹性水平越高,其共情疲劳的程度越低,ICU护士心理弹性与共情疲劳水平相关性更高。建议护理管理者加大对于临床护士心理弹性建立的支持,减少共情疲劳对临床护士工作的影响。  相似文献

The very high cycle fatigue (VHCF) failure of in-service components is mainly caused by the vibration of thin-wall elements at a high frequency. In this work, a novel model of ultrasonic fatigue test was developed to test thin-wall material in bending up to VHCF with an accelerated frequency. The theoretical principle and finite element analysis were introduced for designing a sample that resonated at the frequency of 20 kHz in flexural vibration. In the advantage of the second-order flexural vibration, the gauge section of the sample was in the pure bending condition which prevented the intricate stress condition for thin-wall material as in the root of cantilever or the contact point of three points bending. Moreover, combining the constraint and the loading contact in one small section significantly reduced heating that originated from the friction at an ultrasonic frequency. Both strain gauge and deflection angle methods were applied to verify the controlling of stress amplitude. The fractography observation on Ti6Al4V samples indicated that the characterized fracture obtained from the novel model was the same as that from the conventional bending test.  相似文献
