ENDOSCOPIC PANCREATIC STENTING IN CHRONIC PANCREATITIS WITH DUCTAL DILATION PROXIMAL TO A STRICTURE: A SAFE and EFFECTIVE PROTOCOL

After removal of intraductal stones, a 10‐Fr or 7‐Fr pancreatic stent was placed in 16 patients with upstream ductal dilation proximal to a stricture of the main pancreatic duct. Stents were removed after a mean duration of 52.5 days. Nine patients underwent repeated stenting. About one year after removal of the initial stent, when the remaining upstream ductal dilation was found on follow‐up pancreatograms, the next stent was replaced. Repeated stenting improved outflow of pancreatic juice more effectively than one‐time stenting. Correlation between long‐term pain relief without recurrence of intraductal stones and reduction of duct diameter was also shown. Stent occlusion was observed in 14 of 30 stents. Stent occlusion was frequently associated with recurrence of pancreatitis and intraductal stones, and was also associated with morphologic changes in the pancreatic ductal system. Although there were no significant differences between stent patency of the initial stents and that of the next stents, stent patency of 10‐Fr stents was superior to that of 7‐Fr stents. 10‐Fr stents should be removed within 8 weeks and 7‐Fr stents should be removed within 4 weeks for the prevention of stent occlusion. Repeated stenting with short‐term stenting is therefore considered a safe and effective protocol of endoscopic pancreatic stenting.     

胸骨"T"形锯开微创切口保留后叶装置二尖瓣置换术临床探讨

目的 介绍胸骨中下段“T”行锯开微创切口、保留后叶装置、连续缝合法人工机械瓣二尖瓣置换术的临床观察结果。方法 本组42例患应用胸骨中下部“T”形锯开微创切口行保留二尖瓣后叶装置人工机械瓣置换术，其中男性18例，女24例，年龄31～56岁。结果 应用该手术技术的患，手术顺利，无术后并发症及死亡。结论 本术式具有安全、创伤小、术后心脏功能恢复快、并发症少等优点。     

自发性食管破裂的诊断和外科治疗

目的:探讨自发性食管破裂的诊断和外科治疗。方法 :对 14例自发性食管破裂患者的诊断、治疗方法及治疗结果进行回顾性分析。结果:本组确诊时间 2 4 h以内者仅 2例 ,确诊时间大于 2 4 h者 12例 ;一期食管修补、带蒂胸膜覆盖术 3例 ,治愈 1例 ,治疗失败 2例 (其中 1例死亡 ,1例延期行结肠代食管术后治愈 ) ;行破裂段食管切除、胃 -食管吻合术的 5例全部治愈 ;保守治疗 6例 ,治愈及死亡各 3例。结论:早期诊断自发性食管破裂是提高疗效、降低病死率的关键。发病 2 4 h以上者 ,如全身情况允许 ,破裂段食管切除、胃 -食管吻合术的疗效较好。     

Pineal atrophy and other neuroendocrine and circumventricular features of the naked mole-rat,heterocephalusglaber (Rüppell), a fossorial,equatorial rodent

Summary Quantitative aspects of the microanatomy of the pineal gland and other neuroendocrine and circumventricular structures were studied in a small, reproductively suppressed, female Naked Mole-rat from central Kenya, Africa. The atrophic pineal is the smallest in absolute size (0.002135 mm³) of any so far described in a species of rodent, and in size relative to body weight is second only to that of another tropical species. The subcommissural organ and posterior collicular recess are also relatively small and less well differentiated than those in most other examined rodent species. In contrast, the subfornical organ, OVLT and median eminence are large and well vascularized. It is concluded that the pineal in this species follows the previously described trend among rodents of relatively smaller size in species whose centers of distribution are in lower latitudes. Although the pineal is atrophic, the Naked Mole-rat still exhibits 24-hour and seasonally timed patterns of behavior and seasonal reproduction. However, in this species these events are probably cued by moisture, temperature and social factors rather than by photic information.

重症心脏瓣膜置换术的体外循环管理

目的:回顾性总结重症心脏瓣膜病置换术体外循环经验.方法:105例重症心脏瓣膜病患者,使用stockerⅢ型心肺机及膜式氧合器,体外循环中采用中度低温,预充白蛋白并附加人工肾超滤,心肌保护用高钾含血停搏液灌注.结果:105例患者均顺利脱机,无1例死亡.体外循环时间60～180 min,主动脉阻断时间60～120 min,所有患者均顺利脱离体外循环,心脏自动复跳86例,电击19例.结论:手术中加强心肌保护,可减低手术后的功能衰竭和并发症,并可有效提高手术成功率.     

Behavioral models in mice. implication of the alpha noradrenergic system

1. The mechanism of action of drugs might change according to the test used. Several noradrenergic drugs were tested in order to understand their implication in the mobility tests.

2. It was found that clonidine, an Alpha 2 agonist, acted differently according to the tast used. It provoked sedation in spontaneous activity test, and anti-immobility effects in the other tests.

3. Tall suspension test is able to show the double acting of clonidine.

4. Idazoxan might act either as an alpha 2 antagonist or as partial alpha 2 agonist. TST shown the unexpected partial alpha agonist effect of the molecule.

5. Forced swimming test is more specific for predicting antidepressant activity than tail suspension test which is close to a spontaneous activity model.     

TDP局部照射用于治疗儿童髋关节一过性滑膜炎的疗效观察

目的:探讨加速儿童髋关节一过性滑膜炎疾病的愈合.方法:随机性分析2005年1月～2006年4月共100例髋关节一过性滑膜炎患儿的治疗效果,把病人随机分为治疗组及对照组.治疗组按常规方法治疗加TDP灯局部照射,对照组仅按常规方法治疗.结果:治疗1周后,治疗组B超检查增厚滑膜厚度变化差值、关节腔间隙宽度变化值、关节腔积液好转率、"4"字试验阴性率均明显优于对照组(P＜0.05).结论:TDP灯用于治疗儿童髋关节一过性滑膜炎能加速疾病愈合,提高治疗效果.
