目的 :探讨地塞米松减轻曲马多术后镇痛时恶心呕吐的临床效果。方法 :选择术后自愿硬膜外镇痛(PCEA)的病例 6 0例 ,随机分为试验组 (地塞米松 曲马多 0 .15 %布比卡因 )和对照组 (格拉司琼 曲马多 0 .15 %布比卡因 )。结果 :两组病人术后疼痛评分 (VAS法 ) ,镇静评分及恶心呕吐的发生率无明显差异 (P >0 .0 5 )。结论 :地塞米松可以减轻曲马多术后镇痛时恶心呕吐的发生 ,效果良好 ,不增加副作用 ,价格便宜  相似文献

目的 观察羟丁酸钠对缺血缺氧性脑损伤新生大鼠大脑皮层神经细胞凋亡的影响。方法 新生7dSD大鼠随机分为假手术组(S组)、生理盐水对照组(C组)和羟丁酸钠组(71、坦和档组)。每组按Ⅲ(缺血缺氧)后不同时间点进一步分为1h、3h、1d、3d、7d5个亚组(n=6)。按Rice法制作缺血缺氧性脑损伤模型。C、γ1、γ2和γ3组HI后即腹腔分别注射生理盐水0.2ml/10g、羟丁酸钠50、100、200mg/kg,3次/日;S组术后吸空气2h,腹腔注射生理盐水0.2ml/10g,3次/日。TUNEL染色法检测HI后各时间点大脑皮层凋亡神经细胞。结果 HI后1h~7d,C、γ1、γ2、γ3组凋亡神经细胞数高于S组(P〈0.05),且于HI后1d达高峰;HI后3h-3d,C、γ3组凋亡神经细胞数多于γ1、γ2组(P〈0.05),而C组与γ3组之间凋亡神经细胞数差异无统计学意义(P〉0.05);HI后7d,γ1组凋亡神经细胞数多于让组(P〈0.05)。结论 50、100mg/kg羟丁酸钠能抑制缺血缺氧性脑损伤新生大鼠大脑皮层神经细胞凋亡,且100mg/kg效果较好。  相似文献

本研究用酶联免疫吸附术观察8只家兔在附睾注射鱼肝油酸钠后血清及精浆中的抗精子抗体。血清抗精子抗体阳性率为37.5~50.0%,最早检出率为术后2个月,6~7月达高峰。精浆抗精子抗体阳性率为25%,最早检出率为术后4个月,且此时己达高峰。与输精管结扎资料相比,发现血清抗精子抗体检出率偏低,而精浆相应抗体偏高,表明向附睾注射鱼肝油酸钠引起的免疫反应有别于结扎,以生殖道局部为主。  相似文献

The effects of the Aconitum alkaloid 3-acetylaconitine on neuronal activity were investigated in the slice preparation and on cultivated neurons of rat hippocampus by extracellular and patch-clamp recordings, respectively. 3-Acetylaconitine (0.01–1 μM) diminished the orthodromic and antidromic population spike in a concentration-dependent manner. The inhibitory action of the drug was preceded by a transiently enhanced excitability. The latency of onset of the inhibition was accelerated by increased stimulation frequency, whereas recovery during washout of the alkaloid was accelerated by decreased stimulation frequency. Moreover, the inhibitory effect of 3-acetylaconitine was evaluated in two different models of epileptiform activity induced either by blockade of GABA receptors by bicuculline (10 μM) or by a nominal Mg2+-free bathing medium. In accordance with the activity-dependent mode of action, this compound abolished the synaptically evoked population spikes in the presence of bicuculline or nominal Mg2+-free bathing medium, respectively. Whole-cell patch-clamp recordings revealed an interaction of 3-acetylaconitine with the voltage-dependent sodium channel. At a concentration of 1 μM, 3-acetylaconitine did not affect the peak amplitude of the sodium current, but shifted the current-voltage relationship in the hyperpolarized direction such that sodium currents were already activated at the resting potential. Nephrocalcinosis (NC) detected by ultrasound is a recognized abnormality for some patients with X-linked hypophosphatemia (XLH) who received vitamin D2 and inorganic phosphate therapy, but is commonly observed in XLH patients treated with 1,25-dihydroxyvitamin D3 and inorganic phosphate supplementation. Nevertheless, long-term follow-up of kidney function in XLH patients with NC detected ultrasonographically has not been reported. We investigated two women with XLH, ages 31 (patient 1) and 39 (patient 2) years, each of whom had suffered at least one documented episode of vitamin D2-induced hypercalcemia and renal azotemia during childhood. Patient 2 had also been treated with inorganic phosphate. No medications for XLH had been taken during adulthood. Renal ultrasonography at our institution demonstrated marked bilateral medullary NC in both women. No other explanation was found for their NC that apparently occurred several decades earlier from medical therapy for XLH. Detailed studies (including creatinine clearance, β2-microglobulin excretion, and fasting urinary osmolality and acidification) revealed no impairment of kidney function in either patient. Our findings indicate that subradiographic medullary NC acquired during medical therapy for XLH may persist for decades, but with no adverse renal sequelae. Definitive (long-term) assessment of kidney function in the XLH population with NC, however, will be necessary to fully understand the risk of current medical treatment for this most common heritable form of rickets.  相似文献

In the past few years there have been numerous publications which have stressed the value of the dexamethasone suppression test (DST) as a diagnostic marker of endogenous depression. Our own studies in 333 psychiatric inpatients and 121 healthy subjects did not reveal a differential diagnostic use for the DST. This result is in good agreement with other results in the literature. Our data demonstrate that intervening variables such as severity of illness, weight loss, sleep disturbances, situational stress, drug and alcohol withdrawal, and the pharmacokinetics of dexamethasone have an important influence on DST results, regardless of the diagnostic classification.  相似文献

目的:探讨新生大鼠内毒素休克脑损伤时脑一氧化氮合成酶(NO6)三种亚型基因表达的变化及地塞米松(DEX)对其的调控作用。方法:在新生大鼠内毒素休克动物模型基础上,采用逆转录PCR及PCR技术,对脑组织中三型NOS mRNA及caspase—3 mRNA的表达进行半定量分析。结果:正常新生大鼠脑iNOS及eNOS mRNA无明显表达,nNOS mRNA、caspase—3 mRNA有一定程度表达。内毒素脂多糖(LPS)腹腔注射后2h,三种亚型NOS mRNA开始表达,于LPS6h达高峰,并持续至24h。caspase—3 mRNA于LPS腹腔注射后2h后表达逐渐增加,24h达高峰。DEX可抑制nNOS、iNOS及caspase—3 mRNA的表达,且以用药后2h最为明显,并持续至用药后24h。结论:内毒素休克脑损伤时,各型NOS均有表达,NO的产生是内毒素休克脑损伤时重要的病理生理机制之一。DEX通过抑制NOS、caspase—3 mRNA的表达部分实现其神经保护作用.  相似文献

It is well established that 2% Sodium Cromoglycate is an effective treatment for a number of allergic eye diseases. It has been shown to be non-toxic. It can be used longterm and in serious allergic problems it is a useful adjunctive therapy to steroids. The main problem with Sodium Cromoglycate is that the recommended dosage is a four times daily application and patient non-compliance is common. One of the main objectives of any therapy is to reduce the frequency of dosage and the current study has been designed to investigate the efficacy of a 4% solution of Sodium Cromoglycate, used twice daily, versus a 2% solution used four times daily in seasonal allergic conjunctivitis. A multicentre study, therefore, was carried out to assess the efficacy of both drugs and to assess any possibility of side effects. In addition, a unit dose was used, thus eliminating preservatives and it was used specifically in seasonal allergic conjunctivitis in the pollen season. This study showed that 4% Sodium Cromoglycate used twice daily was at least as affective as 2% Sodium Cromoglycate used four times daily.  相似文献
