Acute drug toxicity related to drinking herbal tea in a kidney transplant recipient

Calcineurin and mTOR inhibitors are commonly used immunosuppressive agents with narrow therapeutic range. As the drugs are mainly metabolized by the P450 cytochrome system, the interaction between food and herbs are also commonly seen and affect the drug levels. We present a case of a kidney transplant recipient with toxic therapeutic levels of cyclosporine A and sirolimus due to interaction between the immunosuppressive agents and Chinese herbal tea. Ingredients within the herbal tea were reported to have inhibitory effect on cytochrome CYP3A4 in-vitro studies. Transplant recipients should be alert that there may be potent interaction between the immunosuppressive drugs and herbs resulting in adverse effect on allograft function.     

驻景丸加减治疗甲状腺功能减退性心脏病心肾阳虚型20例

目的评价驻景丸加减治疗甲状腺功能减退性心脏病心肾阳虚型的临床疗效。方法选取甲状腺功能减退性心脏病心肾阳虚型患者40例，随机分为治疗组和对照组。治疗组给予驻景丸加减联合左甲状腺素，对照组单纯给予左甲状腺素，观察临床疗效。结果治疗组总有效率为95．00％，对照组总有效率为80．00％，治疗组优于对照组（P〈0．01）。结论驻景丸加减联合左甲状腺素疗法优于单纯西药治疗。

Ursolic acid induces neural regeneration after sciatic nerve injury

In this study,we aimed to explore the role of ursolic acid in the neural regeneration of the injured sciatic nerve.BALB/c mice were used to establish models of sciatic nerve injury through unilateral sciatic nerve complete transection and microscopic anastomosis at 0.5 cm below the ischial tuberosity.The successfully generated model mice were treated with 10,5,or 2.5 mg/kg ursolic acid via intraperitoneal injection.Enzyme-linked immunosorbent assay results showed that serum S100 protein expression level gradually increased at 1-4 weeks after sciatic nerve injury,and significantly decreased at 8 weeks.As such,ursolic acid has the capacity to significantly increase S100 protein expression levels.Real-time quantitative PCR showed that S100 mRNA expression in the L4-6 segments on the injury side was increased after ursolic acid treatment.In addition,the muscular mass index in the soleus muscle was also increased in mice treated with ursolic acid.Toluidine blue staining revealed that the quantity and average diameter of myelinated nerve fibers in the injured sciatic nerve were significantly increased after treatment with ursolic acid.10 and 5 mg/kg of ursolic acid produced stronger effects than 2.5 mg/kg of ursolic acid.Our findings indicate that ursolic acid can dose-dependently increase S100 expression and promote neural regeneration in BALB/c mice following sciatic nerve injury.     

成年大鼠延髓薄片制备的改良方法

目的:建立一种适用于膜片钳记录研究的成年大鼠延髓薄片制备方法。方法:用改制的注射器将延髓和上颈髓段从离断的椎管中直接吹出;采用水平切的方式制备延髓薄片;记录三叉神经脊束核尾侧亚核(Vc)神经元的自发放电和诱发放电活动。结果:分离得到的延髓和上颈髓标本光滑完整;水平切的延髓薄片较好地保持了Vc的形态学结构和神经元活性,可较好地记录到Vc神经元自发的兴奋性突触后电流和诱发的双脉冲抑制活动。结论:本方法操作简单,取材快速,延髓薄片外形完整且能保持神经元活性,适用于成年大鼠Vc的膜片钳研究。     

The Meaning of Embodiment

There is substantial disagreement among philosophers of embodied cognitive science about the meaning of embodiment. In what follows, I describe three different views that can be found in the current literature. I show how this debate centers around the question of whether the science of embodied cognition can retain the computer theory of mind. One view, which I will label body functionalism, takes the body to play the functional role of linking external resources for problem solving with internal biological machinery. Embodiment is thus understood in terms of the role the body plays in supporting the computational circuits that realize cognition. Body enactivism argues by contrast that no computational account of cognition can account for the role of commonsense knowledge in our everyday practical engagement with the world. I will attempt a reconciliation of these seemingly opposed views.     

基于扎根理论的中成药上市后再评价专家访谈资料分析＊

目的 开展更全面的中成药临床应用资料收集，并对非定量化资料进行定性处理。方法 运用专家深度访谈的方法，以某中成药的临床应用为例，对临床上使用过该中成药的专家进行半结构式访谈，记录并整理访谈资料，引入扎根理论方法进行访谈资料分析。结果 本研究共访谈了10位专家，经扎根理论分析编码，将该中成药的临床应用编码为该药的背景、有效性、安全性、经济便宜性、依从性、临床定位6个主范畴，最终提炼出一个核心类属，并围绕这几个方面的具体信息进行详细分析。结论 通过收集、整理和分析专家经验性信息，丰富了中成药上市后再评价的临床证据集，明确了已上市中成药的临床应用范围，可为后续的临床和科研研究提供证据支持，同时，专家深度访谈和扎根理论分析是对专家经验的挖掘和利用，还可应用于中医经验传承、医者诊疗思维研究等领域。本研究进行了方法探讨和实例验证，以期为后续更高阶的研究奠定基础。

目的探讨改变中医医院输血科现状，建设独立输血科。方法分析某中医医院输血科（血库）发展的历史和现状以及建立独立输血科的条件。结果中医医院成立独立的输血科，能够充分发挥输血科真正的职能及临床作用，使临床用血更加科学合理、安全有效。结论中医医院输血科的建设，能反映中医医院临床用血、医疗水平和输血医学的发展，也能为患者提供合理治疗。
