This paper compares the immediate effects of dorsal, volar and no splints in reducing hypertonicity in the wrist flexors of spastic hemiplegic subjects. The measures include: (i) passive range of motion; (ii) angle of point of stretch reflex; (iii) resistance to passive wrist extension; and (iv) force of spontaneous wrist flexion. Results of this study indicate a significant reduction in hypertonicity following both dorsal and volar splint application on the passive range of motion and resistance to passive extension measures (P < .05), and a significant reduction in hypertonicity as measured by spontaneous flexion following two hours of dorsal splint wearing (P < .05). No significant reductions in hypertonicity were noted on the angle of point of stretch reflex measure, and on the force of spontaneous flexion measure. Possible explanations for results and suggestions for further resarch are discussed.  相似文献

BACKGROUND: Electrogymnastics can offer a range of proprioceptive, motor, and cutaneous sensation impulses to the central nerve system. The center receives perception of the paralyzed muscle through the aid of these impulses. During this process, functional reorganization of connecting network between segment and intersegment takes place. OBJECTIVE: To observe the therapeutic efficacy of acupoint electrogymnastics and traditional electroacupuncture on stroke hemiplegia. DESIGN, TIME AND SETTING: A multicenter, randomized, controlled, blinded, clinical study was performed at the College of Acu-moxibustion and Massage in Shanghai University of Traditional Chinese Medicine from May 2004 to September 2006. PARTICIPANTS: A total of 153 patients suffering from stroke hemiplegia, comprising 83 males and 70 females, aged 63-70 years, were admitted to outpatient and inpatient at LongHua Hospital Affiliated Shanghai University of Traditional Chinese medicine, Putuo District Traditional Chinese Medicine Hospital and Changqiao Street Community Health Service Center of Shanghai. METHODS: The patients were randomly divided into treatment (n = 77) and control (n = 76) groups. They were treated with acupoint electrogymnastics and traditional electroacupuncture, respectively. In the treatment group, two pairs of positive and negative JD-2008 type electrodes from a hemiplegia treatment apparatus were directly pasted on the Shousanli (LI 10) and Waiguan (SJ5) acupoints of the upper limb, as well as the Zusanli (S36) and Yanglingquan (GB34) acupoints of the lower limb, respectively. In the control group, needles were consecutively inserted into the above acupoints. Using the method of lifting-inserting and twisting-rotating, the needle was manipulated with small amplitude of 5-7 mm and a fast frequency of 80-120 times/min when the needle was inserted to a suitable depth. 目的:观察月国绳肌练习对改善脑卒中患者膝关节过伸及日常生活活动能力(ADL)的疗效。方法:38例患者随机分为治疗组(20例)和对照组(18例)。治疗组除了给予常规康复治疗(中级偏瘫医疗体操;神经肌肉促进技术;步态分解练习;ADL训练),还给予针对性月国绳肌练习(月国绳肌最大向心性等张-膝关节末端等长抗阻练习;月国绳肌肌电生物反馈治疗);对照组只给予常规康复治疗。治疗时间60d。比较治疗前后LKSS膝关节功能评定、上田敏12级评定、Barthel指数等各项指标的变化。结果:治疗组各项指数的改变均明显优于对照组(P<0.01)。结论:月国绳肌练习对脑卒中偏瘫患者的膝关节不稳有明显疗效,并能改善患者的日常生活活动能力。  相似文献

OBJECTIVE: This article discusses the pathophysiology and implications for treatment of hemiplegic migraine within a case study presentation. BACKGROUND: We evaluated a 31-year-old white woman for hemiplegia in her 36th week of pregnancy. She initially presented with severe headache, dysarthria, lethargy, and left-sided numbness and weakness. Hemiplegic migraine remains a diagnosis made by exclusion; neurologic examination of these patients is localizing, but nonspecific. DESIGN: Magnetic resonance imaging and single photon emission computed tomography scanning were performed on this patient during an exacerbation of headache associated with dense hemiplegia. RESULTS: Magnetic resonance imaging showed a superficial cerebral hemispheric signal abnormality with enhancement. Single photon emission computed tomography scanning confirmed hyperperfusion of that hemisphere. CONCLUSIONS: We believe the imaging evidence in our patient suggests that hemiplegia was caused and sustained by hyperperfusion. This case lends supportive evidence to a primarily vasodilatory mechanism and hyperperfusion as an etiology of the paralysis in such headaches and perhaps migraine with aura.  相似文献

目的 :探讨脑卒中偏瘫后早期行头针治疗的临床意义。方法 :脑卒中偏瘫患者 5 7例分为早治组 32例和晚治组 2 5例 ,分别于偏瘫后第 3~ 14天和第 31~ 6 4天进行药物和头针治疗。采用Fugl Meyer运动功能积分评定法评测 2组治疗前后的疗效。结果 :治疗前 2组患者上下肢运动功能Fugl Meyer评分比较差异无显著性(P >0 .0 5 ) ;治疗后 2组评分与治疗前比较均明显升高 (P <0 .0 1) ,早治组与晚治组比较 ,上肢运动功能改善更显著(P <0 .0 5 )。结论 :脑卒中偏瘫后早期配合头针治疗可明显提高康复疗效。  相似文献

目的研究选择性脊神经后根部分切断术对偏侧肢体痉挛性瘫痪患儿早期缓解下肢痉挛作用,以及利用核磁波谱分析患儿在选择性脊神经后根部分切断术后基底节区的小分子代谢变化,探索该术式的"超节段反应"的机制。方法对12~20岁年龄段偏侧痉挛性肢体瘫患者,在接受选择性脊神经后根部分切断术前后,对髋内收肌、腘绳肌和小腿三头肌行改良的Ashworth评级。行核磁波谱分析,利用双侧基底节区域作为兴趣区。分析该区域手术前后的小分子代谢图谱。结果手术后所有患者下肢肌群肌张力有不同程度降低(P0.01),其中4例患者称获得了类似"超节段反应"临床表现,3例患儿上肢协调性以及精细活动改善,1例患儿斜视改善。患儿对侧基底节区N-乙酰天门冬氨含量下降(P0.05),同侧各类物质无明显改变。结论选择性脊神经后根部分切除术能有效降低偏侧肢体痉挛,对侧基底节区N-乙酰天门冬氨减少,可能是出现"超节段反应"的机制之一。  相似文献

Data mining has the potential to provide information for improving clinical acupuncture strategies by uncovering hidden rules between acupuncture manipulation and therapeutic effects in a data set. In this study, we performed acupuncture on 30 patients with hemiplegia due to acute ischemic stroke. All participants were pre-screened to ensure that they exhibited immediate responses to acupuncture. We used a twirling reinforcing acupuncture manipulation at the speciifc lines between the bilateralBaihui(GV20) andTaiyang (EX-HN5). We collected neurologic deifcit score, simpliifed Fugl-Meyer assessment score, muscle strength of the proximal and distal hemiplegic limbs, ratio of the maximal H-relfex to the maximal M-wave (Hmax/Mmax), muscle tension at baseline and immediately after treatment, and the syndromes of traditional Chinese medicine at baseline. We then conducted data mining using an association algorithm and an artiifcial neural network backpropagation algorithm. We found that the twirling reinforcing manipulation had no obvious therapeutic difference in traditional Chinese medicine syndromes of “Deifciency and Excess”. The change in the muscle strength of the upper distal and lower proximal limbs was one of the main factors affecting the immediate change in Fugl-Meyer scores. Additionally, we found a positive correlation between the muscle tension change of the upper limb and Hmax/Mmax immediate change, and both positive and neg-ative correlations existed between the muscle tension change of the lower limb and immediate Hmax/Mmax change. Additionally, when the difference value of muscle tension for the upper and lower limbs was > 0 or < 0, the difference value of Hmax/Mmax was correspondingly positive or negative, indicating the scalp acupuncture has a bidirectional effect on muscle tension in hemiplegic limbs. Therefore, acu-puncture with twirling reinforcing manipulation has distinct effects on acute ischemic stroke patients with different symptoms or stages of disease. Improved muscle tension in the upper and lower limbs, relfected by the variation in the Hmax/Mmax ratio, is crucial for recovery of motor function from hemiplegia.  相似文献

目的检验功能前伸试验(FRT)在脑卒中偏瘫患者站立平衡功能评定中的效度和信度,为FRT在临床康复评定中的应用提供客观依据。方法随机选取50例恢复期脑卒中偏瘫患者,分为治疗组和对照组,并对两组在治疗前后分别进行FRT、Lindmark站立平衡功能评定、Fugl-Meyer站立平衡功能评定以及MAS运动功能评定。评定结果采用Spearman等级相关分析来检验FRT方法的效度;采用组内和组间相关系数和独立样本t检验来检验FRT方法的信度。结果FRT与Lindmark站立平衡功能评定、Fugl-Meyer站立平衡功能评定法之间呈高度相关(P<0.01),与MAS之间具有高度相关(P<0.01);FRT方法组内相关系数ICC=0.889—0.939,组间相关系数ICC=0.865—0.946;治疗组治疗后的评定分数较治疗前有明显提高,差异显著(P<0.01),而且治疗组治疗前后的疗效明显高于对照组,两组间具有显著性差异(P<0.01)。结论FRT在脑卒中偏瘫患者的平衡功能评定中具有良好的效度和信度,且能够反映动态的平衡功能变化过程,在临床康复评定中有推广应用价值。  相似文献

目的探讨早期康复护理对脑梗死偏瘫失语患者的康复效果的影响分析。方法将本院78例脑梗死偏瘫失语患者作为研究对象,随机分为对照组和观察组各39例,其中对照组患者采用常规护理,而观察组患者则在对照组的基础上采用早期康复护理。半年之后对2组患者语言功能以及运动功能和生活能力进行比较。结果护理前,2组患者的语言功能以及运动功能和生活能力无显著差异(P0.05);护理后,2组各项指标均明显比护理前好(P0.05),且观察组患者的3项指标的改善情况又明显好于对照组(P0.05)。结论脑梗死偏瘫患者实施早期护理干预,可以提高治疗效果,改善患者的运动功能以及语言功能及生活能力。  相似文献
