论AB类医疗设备使用率考核方法

对AB类医疗设备使用率的考核是了解设备使用情况和效益的有效方法。本文通过几年来对南京总医院AB类医疗设备使用率的年度考核，摸索总结出一套行之有效的考核方法，为今后新型医疗设备使用率考核提供了可循的依据。     

The estimation of the plasma free fraction of cyclosporine in rabbits and heart transplant patients: The application of a physiological model of renal clearance

We estimated the free fraction (f_u) of cyclosporine (CyA) in the plasma from concentrations of CyA in urine (C_u) and plasma (C_p), urine flow rate (UF), and glomerular filtration rate in rabbits and in heart transplant patients. Following intravenous administration of CyA (5–30 mg kg^?1) in ten NZW rabbits and oral administration of CyA (4.8–12.1 mg kg^?1) in nine heart transplant patients, CyA concentrations in urine and plasma were measured by HPLC. The ratios of C_u to C_p and UF data were fitted to a physiological model of renal clearance using NONMEM. The free fraction of cyclosporine in the rabbits and the heart transplant patients was 0.0122 and 0.14, respectively. Because of the relatively low permeability of CyA across the tubular epithelium, no apparent equilibrium between C_u and C_p at any urine flow rate was reached and, therefore, the C_u to C_p ratio will not be equal to f_u.

大豆皂甙和神经肽Y在下丘脑后核引起的心血管效应中的关系

微量注射大豆皂甙（Ｔｓ）到下丘脑后核（ＰＨＮ）能引起平均动脉压（ＭＡＰ）升高，心率加快，其效应与向ＰＨＮ内注入神经肽Ｙ（ＮＰＹ）相似。分别注入不能引起血压、心率发生明显变化的微量Ｔｓ、ＮＰＹ，如果合并注入ＰＨＮ则可引起血压、心率的显著变化，表明二者互有加强作用。Ａｔｒｏｐｉｎｅ可以对抗Ｔｓ、ＮＰＹ或Ｔｓ＋ＮＰＹ同时注入ＰＨＮ的升压效应，提示ＮＰＹ是通过激活脑内Ｍ受体而实现升压效应的，而Ｔｓ的心血管效应又可能是经过脑内ＮＰＹ介导和加强的。     

Biological exposure limits estimated from relations between occupational styrene exposure during a workweek and excretion of mandelic and phenylglyoxylic acids in urine

Summary Styrene exposure of 18 workers in fiber-glass reinforced plastic industries was measured for 30-min periods throughout each workday for a week. The styrene uptake was estimated using pulmonary ventilation measurements. All urine voidings were collected separately and the styrene metabolites, mandelic acid (MA) and phenylglyoxylic acid (PGA) were determined. The relationship between both exposure and uptake versus excretion of these metabolites was studied. Styrene metabolite concentrations and excretion rates (with 95% tolerance limits) were calculated to correspond to a constant 8-h exposure at the Swedish exposure limit level (25 ppm) or an uptake of an exposure limit related styrene dose (6.3 mmol). The tightest tolerance limits were obtained for excretion rate of MA + PGA per 24 h. The calculated biological exposure limit was 3.4 (± 0.7) mmol MA + PGA/24 h for a dose of 6.3 mmol styrene.     

中西医结合治疗肺心病心律失常40例疗效观察

为提高肺心病心律失常的临床疗效,采用中药清肺化痰活血剂配合抗菌素及氧疗法对40例肺心病并发心律失常患者进行了治疗观察,同时与单纯使用抗菌素及氧疗法治疗的40例患者进行对照。临床以心电图改善情况为观察指标,结果两组有效率分别为87.5％和67.5％。Ridit分析结果表明,治疗组疗效明显优于对照组,经显著性检验,两组差异显著(P<0.05)。     

医院感染及漏报率统计分析

目的　了解我院医院感染发生及漏报率。方法　将我院 2 0 0 3年 1～ 12月出院的所有病例进行回顾性调查。结果　医院感染率 7.3 2 %、漏报率 14 .93 %,均符合国家卫生部要求。结论　做好医院感染监测工作 ,严格执行各项工作制度 ,掌握医院感染与手术并发症的鉴别诊断 ,提高医院感染诊断的准确率     

导管射频消融治疗房室结折返性心动过速前后心率变异的改变

对２０例房室结折返性心动过速患者在射频消融前后进行心率变异频域和时域分析，以了解射频消融对心脏自主神经系统的影响。结果显示，心率变异的极低频段（ＵＬＦＰ）、低频段（ＬＦＰ）、高频段（ＨＦＰ）及总能谱（ＴＦＰ）均较射频消融前降低（Ｐ〈０．０５）。提示射频消融对心脏交感神经、融交感神经均有损害，而以副交感神经损害为主。
