EndocrinologyHormonal profile during the follicular phase in cycles stimulated with a combination of human menopausal gonadotrophin and gonadotrophin-releasing hormone antagonist (Cetrorelix)

A third-generation gonadotrophin-releasing hormone antagonist(Cetrorelix) was used during ovarian stimulation in 32 patientsundergoing assisted reproduction, in order to prevent the prematureluteinizing hormone (LH) surge. In all patients, ovarian stimulationwas carried out with two or three ampoules of human menopausalgonadotrophin (HMG), starting on day 2 of the menstrual cycle.In addition, 0.5 mg of Cetrorelix was administered daily fromday 6 of HMG treatment until the day of ovulation inductionby human chorionic gonadotrophin (HCG). A significant drop inplasma LH concentration was observed within a few hours of thefirst administration of Cetrorelix (P<0.005). Moreover, noLH surge was detected at any point in the treatment period inany of the 32 patients. A mean oestradiol concentration of 2122±935ng/1 was observed on the day of the HCG administration, indicatingnormal folliculogenesis. Like LH, progesterone concentrationalso dropped within a few hours of the first administrationof Cetrorelix (P< 0.005). A 0.5 mg daily dose of Cetrorelixprevented a premature LH surge in all the 32 patients treated.     

彩色多普勒超声检查对卵巢肿瘤的诊断价值

①目的 探讨彩色多普勒超声检查对卵巢肿瘤的诊断价值。②方法 对67例疑诊为卵巢肿瘤的病人术前行彩色多普勒超声检查及血流动力学分析，测其动脉血流收缩期峰值流速(PSV)和阻力指数(RI)，并与手术及病理结果对照。③结果 67例卵巢肿瘤经手术及病理证实良性49例，恶性18例，经彩色多普勒超声诊断为良性41例，恶性15例，超声诊断符合率分别为83．7％和83．3％。④结论 彩色多普勒超声对卵巢肿瘤的良恶性判断具有较高价值。     

人卵泡液中干细胞因子水平及其对卵泡发育的影响

目的 :探讨人卵泡液中是否存在干细胞因子 (SCF)及其与卵泡发育的关系 .方法 :采用夹心ELISA方法检测 3 0例进行体外受精 胚胎移植 (IVF ET)患者超促排卵前、取卵时血清与卵泡液中SCF的水平 .结果 :①取卵时卵泡液中SCF水平 (5 14± 67)ng·L 1,显著高于同期血清水平 (3 95± 4 9)ng·L 1,差别有显著性 (P <0 .0 5 ) ;②取卵时血清中SCF水平与超促排卵前血清中SCF水平相比 ,差别无显著性 [(3 95± 4 9)ng·L 1,(3 74± 3 6)ng·L 1,P >0 .0 5 ];③高、中反应型组卵泡中SCF水平 [(5 64± 64)ng·L 1,(5 3 2± 5 5 )ng·L 1]与低反应型 (3 5 2± 78)ng·L 1相比 ,差别有显著性 (P <0 .0 1) ,且与卵泡发育数目之间呈正相关 (r =0 .4 12 ,P <0 .0 5 ) .结论 :卵泡液中SCF可能来自于卵泡本身 ,而非血浆渗透 .SCF参与人卵泡发育的调控     

卵巢癌细胞多种细胞因子基因表达的研究

本文应用RT-PCR方法,检测5例刚分离的晚期上皮性卵巢癌患者肿瘤细胞和3例卵巢痛患者腹水中肿瘤细胞IL-2、IL-2R、TNF-a,IL-6、TGF-p、IL-10等细胞因子基因的表达,用免疫学方法检测卵巢癌细胞上清液中IL-6活性。结果发现:卵巢癌肿瘤细胞表达IL-6mRNA和抑制性细胞因子TGF-p、IL-10。腹水中存在较多量lL-6可能来自肿瘤细胞。     

A case of serous cystadenoma of the pancreas with focal malignant changes

Summary We present a serous cystadenoma of the pancreas with focal malignant changes, and describe its characteristic histological features. On gross examination, a tumor was present on the anterior surface of the body of the pancreas and measured approx 25×25⋻20, mm. Microscopically, most tumor cells showed the typical histological features of serous cystadenoma, characterized by a microcystic architecture and glycogenrich cells with a uniform and bland appearance. However, in some areas, a tendency to papillary structures with fibrovascular cores was noted. These papillary lesions were composed mainly of nonmucinous, glycogen-poor epithelial cells, the nuclei of which showed a mild atypia. In addition, vascular and perivascular invasion was focally observed. However, there was no clinical evidence of local or distant metastasis. From these findings, we diagnosed this lesion as a serous cystadenoma of the pancreas with focal malignant changes rather than a serous cystadenocarcinoma of the pancreas.     

Alagille syndrome and optic pit

Background Optic disc anomalies are frequently associated with Alagille syndrome. We report here the first case of a patient with Alagille syndrome combined with optic pit and serous macular detachment. Methods A male patient with Alagille syndrome was referred from the pediatric hepatology department for investigation of visual loss in the right eye. Anterior segment examination showed evidence of posterior embryotoxon, confirmed by gonioscopic evaluation. Dilated fundus examination revealed serous macular detachment and optic pit. Results OCT 3 scans confirmed the clinical findings. Conclusions Optic pit is uncommon and should prompt a search for genetic disorders or development abnormalities. The association between the two syndromes in this patient might be due to failure of correct neuroectodermal development, for which a common genetic pathway is likely.     

彩色多普勒血流显像对肺良、恶性肿瘤鉴别诊断的应用

本文应用彩色多普勒血流显像技术对４０例肺肿瘤患者进行研究。全部病例均经手术及病理证实。结果表明：肺恶性肿瘤的内部及周边血流丰富，搏动指数（ＰＩ）和阻力指数（ＲＩ）低于肺良性肿瘤。因此，通过观察肺肿瘤的血流丰富程度及ＰＩ（界值＜１），ＲＩ（界值＜０．５）．可做为辨别肺恶性肿瘤的参考条件。     

Peritoneal Desmoplastic Small Round Cell Tumors with Divergent Differentiation: A Review

Peritoneal desmoplastic small round cell tumors with divergent differentiation are recently described highly aggressive neoplasms with characteristic clinical, morphologic, and immunohistochemical features. This review covers 38 cases that have been reported in the literature. The average age of patients is 18.4 years, and males are affected twice as frequently as females. Tumors generally present as multiple peritoneal nodules without obvious organ involvement. Histology shows islands of small cells set in dense desmoplastic stroma. Immunohistochemical stains are usually positive for cytokeratins, epithelial membrane antigen, desmin, and vimentin. Many cases also stain for neuron-specific enolase but rarely for other neuroepithelial markers. Ultrastructural appearances range from undifferentiated small cells to larger epithelial elements. Paranuclear aggregates of intermediate filaments are characteristic. Dense-core granules and other neuroendocrine features have been described in a minority of cases. Some tumors respond to chemotherapy, but most patients die within months to a few years. The histogenesis of these tumors is uncertain.     

纵隔软组织肿瘤23例临床病理分析

本科手术治疗纵隔软组织肿瘤２３例，占同期手术治疗纵隔肿瘤３９０例的５．９％，年龄３５岁以下者１７例，占７３．９％。男女之比为４．８：１（１９／４），本病的临床及Ｘ线表现缺乏特异性，确诊有赖于术后病理学检查。本组中良性肿瘤１５例（６５．２％），恶性肿瘤８例（３４．８％），手术治疗２４例次，肿瘤切除率为９１．７％（２２／２４），手术为首选的治疗方法。
