有效利用人力资源应对门诊护理人员不足的探讨

目的从工作实践中总结管理体会，有效应对当前形势下门诊护理人员缺编现象。方法统计我院门诊9个岗位三年内完成的工作量、人均接待的门诊量、岗位设置及对应人数的配置、护理人员各种休假总天数、人均各种休假天数，做自身比对。结果人员配备增长速度缓慢于工作量增长速度，人力资源不足时，有向护理人员的休假做透支性“借贷”倾向。结论护理人员缺编，会给医院顺利完成日常工作、确保医疗安全带来负面影响，护士长要有灵活多变的管理思路，弹性利用人力，巧妙应对门诊护理人员不足。     

Role of viral RNA and lipid in the adverse events associated with the 2010 Southern Hemisphere trivalent influenza vaccine

In Australia, during the 2010 Southern Hemisphere (SH) influenza season, there was an unexpected increase in post-marketing adverse event reports of febrile seizures (FS) in children under 5 years of age shortly after vaccination with the CSL 2010 SH trivalent influenza vaccine (CSL 2010 SH TIV) compared to previous CSL TIVs and other licensed 2010 SH TIVs. In an accompanying study, we described the contribution to these adverse events of the 2010 SH influenza strains as expressed in the CSL 2010 SH TIV using in vitro cytokine/chemokine secretion from whole blood cells and induction of NF-κB activation in HEK293 reporter cells. The aim of the present study was to identify the root cause components that elicited the elevated cytokine/chemokine and NF-κB signature. Our studies demonstrated that the pyrogenic signal was associated with a heat-labile, viral-derived component(s) in the CSL 2010 SH TIV. Further, it was found that viral lipid-mediated delivery of short, fragmented viral RNA was the key trigger for the increased cytokine/chemokine secretion and NF-κB activation. It is likely that the FS reported in children <5 years were due to a combination of the new influenza strains included in the 2010 SH TIV and the CSL standard method of manufacture preserving strain-specific viral components of the new influenza strains (particularly B/Brisbane/60/2008 and to a lesser extent H1N1 A/California/07/2009). These combined to heighten immune activation of innate immune cells, which in a small proportion of children <5 years of age is associated with the occurrence of FS. The data also demonstrates that CSL TIVs formulated with increased levels of splitting agent (TDOC) for the B/Brisbane/60/2008 strain can attenuate the pro-inflammatory signals in vitro, identifying a potential path forward for generating a CSL TIV indicated for use in children <5 years.     

PDCA循环在输液室规范化管理中的应用

目的研究PDCA(计划—实施—检查—处理)循环管理模式在医院输液室规范化管理中的应用效果。方法分析影响输液室护理服务质量的原因,成立护理质量管理小组,提出切实可行的整改措施,并组织落实。每月科会讨论实施情况,每季小结一次,未解决的问题进入下一轮PDCA。结果比较输液室在实施PDCA循环前后,护患纠纷发生率及不良事件发生率比较,差异有高度统计学意义(P<0.01);满意度及护理部对输液室质量评分差异有统计学意义(P<0.05)。结论 PDCA循环是输液室规范化管理中的一项行之有效的管理模式。     

In vivo and in vitro effects of 42-hydroxy-palytoxin on mouse skeletal muscle: Structural and functional impairment

Palytoxins (PLTXs) are known seafood contaminants and their entrance into the food chain raises concern about possible effects on human health. The increasing number of analogs being identified in edible marine organisms complicates the estimation of the real hazard associated with the presence of PLTX-like compounds. So far, 42-OH-PLTX is one of the few congeners available, and the study of its toxicity represents an important step toward a better comprehension of the mechanism of action of this family of compounds. From this perspective, the aim of this work was to investigate the in vivo and in vitro effect of 42-OH-PLTX on skeletal muscle, one of the most sensitive targets for PLTXs. Our results demonstrate that 42-OH-PLTX causes damage at the skeletal muscle level with a cytotoxic potency similar to that of PLTX. 42-OH-PLTX induces cytotoxicity and cell swelling in a Na⁺-dependent manner similar to the parent compound. However, the limited Ca²⁺-dependence of the toxic insult induced by 42-OH-PLTX suggests a specific mechanism of action for this analog. Our results also suggest an impaired response to the physiological agonist acetylcholine and altered cell elasticity.

Fetal echocardiography for planning perinatal and delivery room care of neonates with congenital heart disease

Fetal echocardiography facilitates the prenatal diagnosis of infants with congenital heart disease (CHD) and through sequential examinations, allows assessment of fetal hemodynamics and cardiovascular status from the time of diagnosis to delivery. Fetal cardiologists have created diagnostic protocols aimed at risk stratifying severity and potential postnatal compromise in fetuses with CHD, and identifying those who may require special intervention at birth or within the first days of life. In this article, we review fetal cardiovascular physiology, the progression of CHD in utero and fetal echocardiographic findings used for risk stratification of newborns with CHD, as well as some of the basic principles of planning for the neonatal resuscitation and initial transitional care of these complex newborns.     

护士“三定”管理在手术室的实施

目的 探讨“三定”（即定员管理、定向培养、定能分级）管理为核心的新型护理能级管理模式在手术室的实施,以加快专科护士培养进程,提升护士职业发展空间,深化护理内涵质量,推进优质护理服务。方法 根据手术室专科建设规范,按照定员管理原则合理配置护理人员。对规范化培训护士、准专科护士、专科护士三个阶段进行手术室专科护理人才的定向培养,并根据定能分级实现对护士能力的分层次使用及绩效分配。结果 手术室实施“三定”管理后,护士的工作能力得以提升,医生的工作满意度明显提高。结论 “三定”管理方案在手术室切实可行,提高了护理质量,促进了护理队伍的内涵建设和护理专科化发展,激发了护理人员的工作热情和创新能力,可为患者提供高质量的围手术期优质护理服务。

物品标签是影响无菌物品质量的一个重要因素,目前,绝大部分医院供应室物品标签都是由人工书写或应用计算机打印纸质标签,内容往往不完整、不规范,其劳动强度大,花费时间多,且手工书写有时不能准确填写灭菌时间,容易出现错误的标签标识。
