目的 针对半骨盆假体发展中的标准化问题,建立I+II+III型标准半骨盆假体模型,对受损骨盆进行骨盆环重建,并进行生物力学研究,以评价其力学响应特点。方法 使用有限元方法模拟人体骨盆在单腿站立时的受力情况。所使用的骨盆模型包含相关关节软骨和韧带,以模拟真实体内环境。结果 在单腿站立姿态载荷作用下,重建骨盆中应力分布与健康骨盆相似,应力均从S1面经由骶髂关节向下传递;骨与假体应力峰值低于材料的屈服极限。结论 Ⅰ+Ⅱ+Ⅲ型标准半骨盆假体能够可靠地恢复受损骨盆的功能,研究结果为该种假体在使用中的力学可靠性提供初步的理论依据。  相似文献

Symptoms of nasal, pharyngeal and ocular discomfort have been reported among workers in the wood surface-coating industry. Symptoms were reported more often by workers using ultraviolet radiation-curable acrylate coatings (UV coatings), which contain potential chemical sensitizers, than by those using acid-curing coatings. Furthermore, increased levels of eosinophil cationic protein (ECP) and albumin, but not tryptase, in nasal lavage from workers exposed to UV coatings have been observed. To further examine whether air contaminants present in the UV-coating industry are causing the observed increase in symptoms, the inflammatory process in the nasal mucosa of workers exposed to UV coatings was investigated. Clinical and biochemical endpoints were selected to distinguish between specific and non-specific hypersensitivity and to test the hypothesis that the symptoms were consistent with Type IV hypersensitivity. The nasal lavage and nasal biopsy were performed under local anesthetic at the workplace during working hours after a minimum of 2 h of work in both the exposed and control groups. Albumin and ECP, and the cytokines tumor necrosis factor- &#102 (TNF- &#102 ) and interleukin-8 (IL-8), were used as inflammatory markers. A multi-probe ribonuclease protection assay was used to attempt to detect cytokine variation in human nasal biopsies. The cytokine genes analyzed were TNF- &#102 , GM-CSF, interferon- &#110 , IL-2, IL-4 and IL-5. L32 and GAPDH were used as control genes for mRNA expression levels. Mucosal inflammation symptoms correlated with increased levels of albumin, but not with increased levels of ECP, secreted proinflammatory cytokines or cytokine gene mRNA expression. We conclude that the symptoms are non-specific and do not correlate with occupational exposure to UV coatings under the conditions of this investigation.  相似文献

Conclusion Although long-term data will be necessary for confirmation, the result of this preliminary study indicates that vitallium may be a good alternative material for ossicular replacement prostheses in the middle ear.

Objectives To investigate the biocompatibility of vitallium (Co–Cr–Mo) as ossicular reconstruction material in the rabbit middle ear, and to compare the results with those obtained with titanium, well known as a highly biocompatible material, and non-implanted control groups.

Material and methods Eighteen female New Zealand White rabbits were anesthetized. The tympanomeatal flap was elevated and 12 vitallium and 12 titanium implants were placed in the bulla away from the ossicles in 24 middle ears. Six rabbits were used as non-implanted controls. All animals were sacrificed under general anesthesia on the 180th day after implantation. The temporal bones were removed, fixed in 10% buffered paraformaldehyde and decalcified for a week in EDTA. Tissue samples were then prepared using an Autotechnicon and embedded in paraffin. Sections (30-μm thick) were cut with a microtome, stained with hematoxylin–eosin, von Gieson's stain and fibroblast growth factor (FGF) and examined under a light microscope. The numbers of lymphocytes, collagen fibers and FGF-positive cells were determined in all three groups.

Results There was no significant difference in the numbers of collagen fibers between the groups (p>0.05). No significant differences were found in the numbers of lymphocytes and FGF-positive cells between the titanium and vitallium groups (p>0.05). The differences in the numbers of lymphocytes and FGF-positive cells between the control and other groups were found to be significant (p<0.05).  相似文献

Nasal septum perforations or defects can have a variety of causes. Defects of the septum tend to occur in the antero-inferior region. We report two cases of congenital vomeral defect of the septum detected during nasal endoscopy. Neither of the patients in this report had any nasal surgery, trauma, infectious disease, or drug abuse. Our findings suggest that a developmental abnormality of the vomer during the prenatal period resulted in congenital defects of the vomer. Congenital defects of the nasal septum should be added to the etiology of septal perforation and defect.  相似文献

Conclusions. Radical ablative surgery and radial forearm free flap (RFFF) reconstruction provide promising oncologic and functional results in patients with oral or oropharyngeal cancer. Objectives. To assess the postoperative outcomes and the oncologic and functional results, with their main predictive factors, after radical ablative surgery and RFFF reconstruction for patients with oral or oropharyngeal cancer. Patients and methods. Between 2000 and 2006, we prospectively analyzed the postoperative, oncologic and functional outcomes of all previously untreated patients who underwent this type of surgery. Results. A total of 132 patients were enrolled in this study. There were three RFFF failures. The rate of surgical complications was 20%. The 5-year locoregional control and overall survival rates were 68% and 52%, respectively. Advanced age, high comorbidity index, elevated overall stage and tumoral involvement of the inner part of the cheek were correlated with a lower overall survival rate. A good functional result was obtained for oral diet, speech, mouth opening and aesthetic outcome in 87%, 80%, 86% and 88% of the patients, respectively. High comorbidity index, large flap surface, radiotherapy and tumoral involvement of the mobile tongue were significant predictors of poorer functional or aesthetic outcomes.  相似文献

ObjectiveThe aim of our study was to analyze the hearing results of ossicular chain reconstruction in incus long process defects in pediatric patients.MethodsThis retrospective study included 15 pediatric patients that had incus long process defect due to chronic otitis media or adhesive otitis, and repaired with glass ionomer cement between 2009 and 2015. The audiological tests (air conduction thresholds, bone conduction thresholds, air bone gap) obtained preoperatively and one year after surgery were compared. In addition, preoperative and postoperative air bone gap differences were estimated to determine hearing gain.ResultsMean air conduction and air bone gaps decreased significantly one year after surgery when compared to the preoperative values (p< 0.001 for both). Mean hearing gain was 20.33 ± 6.36 dB one year after surgery.ConclusionUse of glass ionomer cement to repair incus long process defects is a suitable method that improves hearing in pediatric patients. Further large studies that compare glass ionomer cement ossiculoplasty with other ossicular reconstruction methods are needed.  相似文献
