An outbreak of Pneumocystis carinii pneumonia in children with malignancy

An outbreak of Pneumocystis carinii pneumonia (PCP) in three patients within a 6 week period was reported. Two patients had acute lymphoblastic leukaemia and one had brain-stem glioma. They shared common features of immuno-suppression and absence of cotrimoxazole prophylaxis and had been nursed in the same room. The severity of PCP and its response to treatment may be related to the degree of immunosuppression. Because of the morbidity and mortality of PCP, chemoprophylaxis should be given to all at-risk cases. Furthermore, isolation of patients with PCP should be considered in view of increasing evidence of nosocomial transmission.     

The progressive outer retinal necrosis syndrome

The progressive outer retinal necrosis (PORN) syndrome is a recently described clinical variant of necrotizing herpetic retinopathy in patients with the acquired immunodeficiency syndrome (AIDS). It is caused by varicellazoster virus infection of the retina. Its course and clinical features distinguish it from the acute retinal necrosis syndrome and CMV retinopathy. Early disease is characterized by multifocal deep retinal opacification. Lesions rapidly coalesce and progress to total retinal necrosis over a short period of time. Despite aggressive therapy with intravenous antivirial drugs, prognosis is poor; disease progression and/or recurrence is common, and the majority of patients develop no light perception vision. Total retinal detachments are common. Prophylaxis against retinal detachment using laser retinopexy has not been useful in most cases. PORN syndrome is an uncommon, but devastating complication of AIDS.     

广东省城市社区卫生服务机构功能完善情况调查

目的 调查广东省不同组织形式的社区卫生服务机构功能完善情况。方法 采用问卷和现场检查的方式。结果 广东省城市社区卫生服务机构功能完善情况总体较好，但对某些服务功能如上级医院向下转诊、计划免疫工作的开展、配合计划生育工作等在不同组织形式中完善程度差异很大。结论 在社区卫生服务中，要进一步加强和完善健康教育进入社区、上级医院向下转诊、机构内计划免疫服务、家庭健康档案的建立等工作。     

从中日沿海城市部分老年人生活满足感比较看老年社区护理的发展

目的 通过比较中日两围老年人生活满足感，探讨老年人社区护理的发展方向。方法 中国大连市选取60岁以上老年人，进行人户调查，日本滨松市选取俱乐部的65岁以上老年人，填写问卷。结果 除在与亲人、朋友交往及存钱以防万一等方面外，2国老年人在其他生活满足感方面比较，差异均有统计学意义，P〈0．05。结论 中日两国不同的经济基础导致了不同层次需求的差异。在中国发展老年人社区护理，一方面可以借鉴国外经验，从物质基础上改进和完善；另一方面，借助传统文化优势，从社会风气上进行促进。     

Gram-negative bacterial pneumonia with secondary aspergillosis in an AIDS patient

Summary A 40-year-old, HIV-infected female patient received antibiotic treatment for a urinary tract infection. After the initial success of therapy and a symptom-free period, she developed pneumonia with septic shock and adult respiratory distress syndrome (ARDS). In spite of intensive care and respirator therapy with positive end-expiratory pressure (PEEP), she died of infectious toxic shock. Autopsy findings showed relapsing, gramnegative, bacterial pneumonia (morphologically compatible with Klebsiella pneumonia) and secondary, invasive aspergillosis. 社区卫生服务信息系统功能建模研究

理顺功能关系、理清业务流程是构建社区卫生服务信息系统的前提和基础,功能建模是理顺功能关系的一个重要手段。在研究现有的各种功能分析方法的基础上,本文以IDEF0模型方法为基础,提出了社区卫生服务信息系统功能建模方法。通过建立功能、数据和约束之间的内在联系,为系统的功能设计和数据分析提供了基础。     

哈尔滨市社区卫生服务需求、利用及影响因素分析

目的：为了了解哈尔滨市居民对社区卫生服务的需求、利用情况以及影响因素。方法：采用分层整群抽样，对哈尔滨市2000名居民进行了入户调查。结果：被调查居民的两周患病率为34．3％，两周就诊率为27．7％，其未就诊率为30．0％。影响人们对卫生服务利用的因素有收入、患病次数、健康状况等因素。结论：社区卫生服务应不断加强和完善，从而真正成为实现卫生目标的有效途径。     

艾滋病相关卡波西肉瘤32例尸检材料病理学研究

目的：概况艾滋病相关卡产波西肉瘤（ＫＳ）病理特征，并探讨其两全是学分型、来源与性质。方法：所有病例均按常规尸检及制片，光镜观察，并复习资料。结果：ＫＳ累及皮肤２３例，呼吸系统２０例，消化道１７例，淋巴结１２例，２６例为播散性。肿瘤肉眼表现为斑型和结节型，镜下主要同增生的血管和梭形细胞构成。并常见胶原分、淋这扩张、嗜性小体、炎细胞浸浸、出血及含铁务黄素沉积等特征性病变。结论：ＫＳ是一种颇具特征的病变
