Practical implementation – Essential elements resource tool

Hematopoietic stem cell transplantation (HSCT) is an established treatment for patients with severe congenital or acquired hemato-/oncological disorders. Despite major improvements, HSCT remains associated with substantial morbidity and mortality. Implementation of a quality management system has become standard practice not only for industries when their products or services are associated with significant risks to human safety but also in the healthcare sector. The use of a quality management system contributes to better products and services and improved patient’s outcome after medical interventions. Members of the Alliance for Harmonization of Cellular Therapy Accreditation prepared the document Essential Elements, which is intended to serve as a guide for establishing a quality program for new or developing HSCT programs. It is intended for use as a resource and does not contain the full requirements of all standards but seeks to provide clear examples of compliance to support basic quality system elements. The content is based on common elements found in already existing standards with a major focus on the establishment of a quality program that applies to the entire transplant program. Essential Elements is structured as an explanation helping to understand the intent of the element giving guidance what is needed, and examples showing how the element applies. Centers seeking accreditation are subjected to a detailed document review, on-site inspection and follow-up procedure. In conclusion, new and developing stem cell transplantation programs should focus on quality and safety and step on the path towards full accreditation. The HSCT community must continue its efforts to offer education and training to support developing programs to attain accreditation.     

Certification, accreditation, and specialization in the 1990s

Subspecialization was one of the inevitable consequences of merging medicine with science in the early years of this century. The differentiation of human endeavors increases exponentially with new knowledge, technology, and services. Our current patterns of accreditation, certification, and graduate medical education are being seriously challenged by new patterns of subspecialization. How realistically and creatively we respond to this challenge will be a key element in preserving the management of these functions primarily within the purview of the profession.     

实行财务委派制的实践与体会

为进一步加强医院财务会计管理工作,提高会计核算质量,促进医院健康发展,绍兴市卫生局对市直五家医院实行财务科长委派制。财务科长委派制的实施,能够促使医疗卫生单位的财务监管体系逐步建立和完善,促使医疗卫生单位的财务工作逐步标准化,促使医疗卫生单位的财务科长管理能力逐步提高。在制度实施的过程中,笔者还体会到应注重自律与他律相结合,注重刚性与人性相结合,注重培养大局意识,注重委派制的后续管理。     

美国高等教育认证制度的社会功能性价值对我国医学教育评估的启示

美国高等教育认证制度以其非政府、自愿性、专业性等独特评估特点,成为美国高等教育领域以及社会公众广泛认可并有效的教育质量保障体系。介绍了美国高等教育认证制度中的几点核心特征,并对美国教育认证制度进行社会功能性价值分析。结合目前我国医学教育评估工作的开展情况,从医学教育评估的主体价值观、运行机制及伦理建设等方面获得重要启示。     

Development and Testing of the Person-Centered Environment and Care Assessment Tool (PCECAT)

ABSTRACT

Objective: While person-centered principles provide an ideal foundation for developing service standards in residential aged and dementia care, there has been limited attention to developing tools which can measure service structures, processes and outcomes according to these principles. The Person-Centered Environment and Care Assessment Tool (PCECAT) was developed to assess and improve residential aged-care standards using person-centered principles, while also adhering to the Australian residential aged-care standards.

Methods: A mixed-methods approach was used to develop and validate the PCECAT in five sequential stages.

Results: Content, face, concurrent, and convergent validity were established and test-retest reliability was confirmed in 334 aged/dementia care units belonging to 131 Australian and New Zealand aged-care homes.

Conclusions: The PCECAT and its Guidelines have helped providers to improve service quality. While Australian residential aged-care standards were used as guidelines in developing the scale, there is potential for PCECAT adaptation in other countries and cultures.

《三级医院评审标准(2020 年版》的颁布是一个里程碑式的事件。其新增前置否决条款;强化定量指标条款;整合简化现场条款,体现了评审标准对医院管理“指挥棒”和“风向标”的作用。与《三级综合医院评审标准(2011 年版)》相比有显著差异,体现了与时俱进的特点。新标准不仅强调了医院的社会属性和公益责任,并且凸显了医院的法制意识和安全意识;也契合新时代要求,通过评审引导医院加快技术进步,深化内涵质量建设,提升可持续发展能力,从而助力医院进入高质量发展阶段。     

Assessing the ACGME General Competencies: General Considerations and Assessment Methods

The Accreditation Council for Graduate Medical Education's (ACGME's) general competency and outcome assessment initiative (i.e., the ACGME Outcome Project) is an effort to enhance residency education and accreditation effectiveness by increasing emphasis on educational outcomes. The Project is also a response to concerns about new graduates' ability to meet the demands of today's practice environment. The competencies emphasize learning in new domains (e.g., Practice-Based Learning and Improvement and Systems-Based Practice) and more traditional ones (e.g., Patient Care and Medical Knowledge). Outcome assessment will provide evidence of residency program educational effectiveness and information to guide improvement. This paper discusses the development and implementations of assessment methods appropriate to evaluate the performance of residents in each of the core competencies.
