Fracture-avulsion de l’insertion tibiale du ligament croisé postérieur chez un enfant. À propos d’un cas et revue de la littérature

The isolated lesion of the posterior cruciate ligament (PCL) and in particular the tibial avulsion is a rarely reported entity on immature skeleton. We report a case of isolated avulsion fracture of the tibial insertion of PCL in a young athlete of 14 years old, treated surgically with good functional results 2 years later.

With most procedures of rehabilitation following reconstruction of the anterior cruciate ligament that was treated though the technique of ligamentoplasty using gracilis and semi-tendinous, the hamstring can’t resume activity until four to six weeks after the operation. Comparing the engraftment of post-traumatic muscle damage, the described rehabilitation procedure is an accelerated procedure that uses early eccentric strengthening of the hamstring starting on day 8. This is achieved first by manual resistance until day 21 then on a hamstring chair from day 21 to day 45. The aim of such a treatment is not to resume sporting activity more quickly, but to ensure that the conditions for recovery are optimized. The described treatment also presents some limitations and should therefore be applied with caution so as not to jeopardize the ultimate recovery of the ligament.     

Cancer of the floor of the mouth in a ninety-four year old female patient: a record and its justification

INTRODUCTION: Patient's age is known to have no influence on either treatment or prognosis of oral squamous cell carcinoma. OBSERVATION: A 94-year old female patient, with a squamous cell carcinoma of the floor of the mouth, stage pT1pN0cM0, was classically treated by surgical tumor resection, selective neck dissection and immediate surgical reconstruction with a vascularized radial flap. DISCUSSION: This clinical case, which could be a record taking into account the patient's advanced age, confirms that treatment decision of oral squamous cell carcinoma must not be influenced by age. Only some age-related comorbidities must be considered.
