BACKGROUND: Concerns about possible adverse outcomes for children conceived using ICSI were highlighted in 1998 when 1-year-old ICSI children were found to be at increased risk (relative risk = 9.2) of delayed mental development compared with children conceived naturally or using IVF. As the findings were biologically plausible, it was considered important to reassess child development when a more accurate measure of long-term cognitive ability could be obtained. METHODS: The mental development of 97 ICSI, 80 IVF and 110 naturally conceived (NC) children at 5 years of age was assessed using intelligence quotients (IQ) obtained from the Wechsler Preschool and Primary Scales of Intelligence. RESULTS: The mean full-scale IQ was 110 +/- 18 for ICSI, 111 +/- 13 for IVF and 114 +/- 13 for NC children (P = 0.21, non-significant). ICSI children were not at increased risk for delayed (full-scale IQ <85) cognitive development (ICSI 5.2%, IVF 2.5%, NC 0.9%; P = 0.18, non-significant). The only significant independent predictor of below-average full-scale IQ on multivariate analysis was lower maternal education level. CONCLUSIONS: These findings suggest that the genetic influence of parental cognitive ability is more important than the mode of conception in determining the long-term intellectual ability of children conceived using ICSI.  相似文献

目的探讨总责任护士主导的医护一体化临床护理模式在创伤骨科的应用效果。 方法选择2016年7月至2017年6月在南京大学医学院附属鼓楼医院创伤骨科行手术治疗的骨折患者200例,按照入院先后顺序分为对照组90例(实施创伤骨科常规护理)和观察组110例(实施医护一体化工作模式,包括成立医护合作小组、实施总责任护士主导的医护一体化查房、个体化诊疗方案的共同决策、疼痛管理、早期出院计划与延续护理)。比较2组患者的术后护理效果和医护合作满意度。对数据行t检验和χ2检验。 结果总责任护士主导的医护一体化模式下,观察组患者术后72 h内数字评分法(NRS)评分为(4.09±1.64)分,疼痛干预有效率为77.57%,对照组术后72 h内NRS评分为(5.15±1.69)分,疼痛干预有效率为57.95%,2组比较差异均有统计学意义(P值均小于0.05);观察组下肢深静脉血栓、泌尿系统感染、便秘的发生例数分别为2、1、3,对照组下肢深静脉血栓、泌尿系统感染、便秘的发生例数分别为9、7、11,两组比较差异均有统计学意义(χ2=4.486、4.296、6.812,P=0.027、0.036、0.009)。观察组术后康复开始时间为(21.48±6.41)h,术后住院时间为(11.08±2.19)d;对照组术后康复开始时间为(36.48±10.75)h,术后住院时间为(13.95±2.93)d,2组比较差异均有统计学意义(t=11.670、-7.616,P值均小于0.05)。2组医护合作满意度结果显示,观察组患者信息的交流得分为(3.78±0.85)分,共同参与治疗或护理决策得分为(3.86±0.87)分,医师和护士的关系得分为(4.00±0.84)分,总分为(3.87±0.67)分;对照组患者信息的交流得分为(3.32±1.00)分,共同参与治疗或护理决策得分为(3.23±0.93)分,医师和护士的关系得分为(3.45±0.96)分,总分为(3.33±0.84)分,2组比较差异均有统计学意义(P值均小于0.05)。 结论总责任护士主导的医护一体化护理模式的应用,以亚专科为核心,重新整合医护资源,共同参与制订患者的诊疗护理方案,共同聚焦患者安全与质量,加强了医护间的配合,提高了医疗护理服务质量,提升医护合作满意度。  相似文献

Summary Training-induced adaptations in the endocrine system and strength development were investigated in nine male strength athletes during two separate 3-week intensive strength training periods. The overall amount of training in the periods was maintained at the same level. In both cases the training in the first 2 weeks was very intensive: this was followed by a 3rd week when the overall amount of training was greatly decreased. The two training periods differed only in that training period I included one daily session, while during the first 2 weeks of period II the same amount of training was divided between two daily sessions. In general, only slight and statistically insignificant changes occurred during training period I in mean concentrations of serum hormones examined or sex hormone-binding globulin as well as in maximal isometric leg extensor force. However, during training period II after 2 weeks of intensive strength training a significant decrease (P<0.05) was observed in serum free testosterone concentration [from 98.4 (SD 24.5) to 83.8 (SD 14.7) pmol · l–1] during the subsequent week of reduced training. No change in the concentration of total testosterone was observed. This training phase was also accompanied by significant increases (P<0.05) in serum luteinizing hormone (LH) and cortisol concentrations. After 2 successive days of rest serum free testosterone and LH returned to (P<0.05) their basal concentrations. Training period II led also to a significant increase (P<0.05) [from 3942 (SD 767) to 4151 (SD 926) N] in maximal force. These findings suggest that in male strength athletes dividing the amount of training into smaller units may create more effective training stimuli leading to further strength development.  相似文献

During training to relax the frontalis muscle, continuous biofeedback (BF) was compared to discrete verbal feedback (VF) delivered immediately after each trial. Both feedback modalities were based on frontalis electromyographic (EMG) activity. Training consisted of 3 consecutive daily session-each comprised of 3 baseline (nonfeedback) trials followed by 10 training trials of 128 see. The presence or absence of the two informationally positive feedback modalities were combined factorially to define four training conditions: BF + VF, NO BF + VF, BF + NO VF, and NO BF + NO VF. Results indicated that while VF alone facilitated muscle relaxation, BF was clearly prepotent ill effecting consistent decreases in EMG activity both across trials and days of training. Additionally, the facilitating effect of BF transferred to nonfeedback trials while VF did not affect performance on nonfeedback trials. Finally, accuracy of self-evaluations of performance on a trial by trial basis was markedly improved by BF, while VF improved accuracy only for trials having a very large absolute difference between levels of EMG activity. Ss receiving no feedback neither reduced muscle tension during training not were able to evaluate their performance accurately even when large absolute differences occurred between trials in frontalis EMG activity.  相似文献

The effect of aerobic training on parasympathetic reactivity to mental stress is unclear. Thus, the parasympathetic response, as assessed by time series analysis of heart period variability (HPVts), of 10 trained male runners (trained group), 10 inherently low resting heart rate untrained men (low HR group), and 10 normal resting HR men (control group) at rest and to two mental stressors was examined. Participants completed a mental arithmetic and Stroop task. Resting HPVts at high and medium frequencies was significantly greater for the trained and the low HR groups than for the control group. Significantly greater decreases in HPVts at the medium frequency during arithmetic recovery were measured for the trained and low HR groups compared with the response of the control group. Significantly greater decreases in HPVts at both the the high and medium frequencies during the first two epochs of the Stroop occurred only for the trained and low HR groups. These results suggest that the greater HPVts at rest and decline in HPVts during and after mental challenge is influenced by both aerobic training and genetic inheritance.  相似文献

Fifteen male physical education students were studied. The subjects trained for 4–6 weeks, 2–3 days per week, on a mechanically braked bicycle ergometer. A training session consisted of repeated 30-s ‘all-out’ sprints on a Wingate bicycle ergometer, on which the brake band of the flywheel was loaded with 75 g kg-1 body wt, with rest periods of 15–20 min between consecutive sprints. Thigh muscle biopsies were taken before and after the training period and were analysed for fibre types using a myofibrillar ATPase stain. The proportion of type I fibres decreased from 57 to 48% (P < 0.05) and type IIA fibres increased from 32 to 38% (P < 0.05). This study indicates that it is possible to achieve a fibre type transformation with high-intensity training. The effect of two-legged ‘sprint’ training on muscle fibre type composition may be related to a changed pattern of muscle fibre activation (e.g. an increased stimulation frequency). A change in fibre activation frequency may induce an increased synthesis of type II fibre myosin (fast myosin). Hormonal influences such as enhanced adrenergic stimulation of the muscle fibres cannot be excluded as a contributing factor, however.  相似文献

The fibre-type specific expression patterns of fast and slow isoforms of essential (alkali) myosin light chains (ELC) was analysed in trained, untrained and pathological human muscles. Biopsies from m. vastus lateralis of moderately trained and untrained persons, as well as highly trained endurance and strength athletes were analysed, by in situ hybridization, for the expression of the `fast' ELC 1f/3f and the `slow' ELC 1sb. We wanted to investigate if changes in the fibre-type specific ELC mRNA pattern could be used as markers for training adaptation, especially, if the mRNA of the slow ELC 1sb isoform would appear in type IIA fibres as a result of endurance training (Baumann et al. 1987). We found the fast/slow ELC expression patterns in the fibre types to be remarkably stable. Physiological stress, even high training loads, did not affect it. No IIA fibres expressing ELC 1sb mRNA were found. They could be detected, however, in pathological muscle samples, where fast/slow ELC patterns not found in normal muscles were frequent. Our data suggest that in healthy muscles, only a subset of the theoretically possible combinations of myosin heavy and light chain isoforms are expressed at the level of their mRNAs.  相似文献

Effects of a 24-week strength training performed twice weekly (24 ST) (combined with explosive exercises) followed by either a 3-week detraining (3 DT) and a 21-week re-strength-training (21 RST) (experiment A) or by a 24-week detraining (24 DT) (experiment B) on neural activation of the agonist and antagonist leg extensors, muscle cross-sectional area (CSA) of the quadriceps femoris, maximal isometric and one repetition maximum (1-RM) strength and jumping (J) and walking (W) performances were examined. A group of middle-aged (M, 37–44 years, n=12) and elderly (E, 62–77, n=10) and another group of M (35–45, n=7) and E (63–78, n=7) served as subjects. In experiment A, the 1-RM increased substantially during 24 ST in M (27%, P < 0.001) and E (29%, P < 0.001) and in experiment B in M (29%, P < 0.001) and E (23%, P < 0.01). During 21 RST the 1-RM was increased by 5% at week 48 (P < 0.01) in M and 3% at week 41 in E (n.s., but P < 0.05 at week 34). In experiment A the integrated electromyogram (IEMG) of the vastus muscles in the 1-RM increased during 24 ST in both M (P < 0.05) and E (P < 0.001) and during 21 RST in M for the right (P < 0.05) and in E for both legs (P < 0.05). The biceps femoris co-activation during the 1-RM leg extension decreased during the first 8-week training in M (from 29 ± 5% to 25 ± 3%, n.s.) and especially in E (from 41 ± 11% to 32 ± 9%, P < 0.05). The CSA increased by 7% in M (P < 0.05) and by 7% in E (P < 0.001), and by 7% (n.s.) in M and by 3% in E (n.s.) during 24 ST periods. Increases of 18% (P < 0.001) and 12% (P < 0.05) in M and 22% (P < 0.001) and 26% (P < 0.05) in E occurred in J. W speed increased (P < 0.05) in both age groups. The only decrease during 3 DT was in maximal isometric force in M by 6% (P < 0.05) and by 4% (n.s.) in E. During 24 DT the CSA decreased in both age groups (P < 0.01), the 1-RM decreased by 6% (P < 0.05) in M and by 4% (P < 0.05) in E and isometric force by 12% (P < 0.001) in M and by 9% (P < 0.05) in E, respectively, while J and W remained unaltered. The strength gains were accompanied by increased maximal voluntary neural activation of the agonists in both age groups with reduced antagonist co-activation in the elderly during the initial training phases. Neural adaptation seemed to play a greater role than muscle hypertrophy. Short-term detraining led to only minor changes, while prolonged detraining resulted in muscle atrophy and decreased voluntary strength, but explosive jumping and walking actions in both age groups appeared to remain elevated for quite a long time by compensatory types of physical activities when performed on a regular basis. During the training phase, 96 subjects were given one of four types of relaxation instructions (single instructions, repeated instructions, relaxation training, no instructions) and in addition either did or did not receive frontal EMG biofeedback training. Results indicated that each of the instructions and biofeedback procedures were equally effective in reducing frontal EMG, but that none of these procedures had any effect on subjective anxiety or autonomic indices of arousal (pulse rate, skin temperature, and finger pulse volume). During the generalization/stress phase, subjects were threatened with electric shock and were told to apply the relaxation techniques they learned during the training phase even though no additional instructions and/ or biofeedback training would be provided. To assess the effectiveness of the shock manipulation, a no-threat control group was included. Results indicated that: a) the shock manipulation was effective in increasing arousal, b) previous instructions and/or biofeedback were equally effective in reducing frontal EMG levels, but that c) only relaxation training was consistently effective in reducing subjective and autonomic indices of arousal. These findings: a) suggest that in stressful situations, relaxation training may be more effective than either EMG biofeedback or simple relaxation instructions in producing a general relaxation effect as opposed to a specific EMG effect; and b) indicate the importance of assessing the effectiveness of relaxation procedures during stressful situations during which subjects’ levels of arousal are elevated above resting baseline levels.  相似文献

Summary Differences between the effects of training at sea level and at simulated altitude on performance and muscle structural and biochemical properties were investigated in 8 competitive cyclists who trained for 3–4 weeks, 4–5 sessions/week, each session consisting of cycling for 60–90 min continuously and 45–60 min intermittently. Four subjects, the altitude group (AG), trained in a hypobaric chamber (574 torr=2300 m above sea level), and the other four at sea level (SLG). Before and after training work capacity was tested both at simulated altitude (574 torr) and at sea level, by an incremental cycle ergometer test until exhaustion. Work capacity was expressed as total amount of work performed. Venous blood samples were taken during the tests. Leg muscle biopsies were taken at rest before and after the training period. AG exhibited an increase of 33% in both sea level and altitude performance, while SLG increased 22% at sea level and 14% at altitude. Blood lactate concentration at a given submaximal load at altitude was significantly more reduced by training in AG than SLG. Muscle phosphofructokinase (PFK) activity decreased with training in AG but increased in SLG. All AG subjects showed increases in capillary density. In conclusion, work capacity at altitude was increased more by training at altitude than at sea level. Work capacity at sea level was at least as much improved by altitude as by sea level training. The improved work capacity by training at altitude was paralleled by decreased exercise blood lactate concentration, increased capillarization and decreased glycolytic capacity in leg muscle.  相似文献
