目的 采用HRCT和MRI对耳鸣患者颞骨进行检查,探讨其异常发生率及最佳扫描方案.方法 回顾性分析1015例耳鸣患者的HRCT和MRI资料.搏动性耳鸣145例,142例行HRCT,7例行MRI,其中4例同时行HRCT和MRI,仅行平扫HRCT者67例,仅行增强HRCT者71例.非搏动性耳鸣870例,650例行HRCT,267例行MRI,其中47例同时行HRCT和MRI,仅行平扫HRCT者598例,仅行增强HRCT者5例.分析颞骨异常阳性率及构成比,比较不同影像学检查方法和序列对颞骨异常的显示效果.采用χ2检验对数据进行组间分析.结果 1015例中显示病变者767例(75.57%).常见颞骨异常发生率分别为:颈静脉窝高位414例(40.79%);乙状窦异常387例(38.13%);中耳乳突炎148例(14.58%);中颅窝低位70例(6.90%).平扫HRCT对颈静脉窝高位显示阳性率最高(365/665,54.89%);增强HRCT对乙状窦异常显示的阳性率最高(56/76,73.68%).在搏动性耳鸣患者中,乙状窦异常发生率最高(96/145,66.21%),且其发生率明显高于非搏动性耳鸣患者(291/870,33.45%;χ2=56.537,P<0.01).真稳态进动快速成像(FIESTA)序列显示内耳道内血管袢影最佳(42/42,100%).结论 颈静脉窝高位和乙状窦异常是耳鸣患者最常见的颢骨影像异常.搏动性耳鸣首选增强HRCT,非搏动性耳鸣首选平扫HRCT,内耳道内血管显示首选MR FIESTA序列.
Abstract:
Objective To study high resolution CT (HRCT) and MRI findings of temporal bone anomaly in patients with tinnitus and identify the optimal examination method in the detection of the anomaly. Methods The HRCT and MRI data were analyzed retrospectively in 1015 patients including 145 patients with pulsatile tinnitus (PT) and 870 patients with nonpulsatile tinnitus (NPT). The positive rates of HRCT and MRI in the identification of temporal bone anomaly were analyzed and the efficiency of various examination methods was compared in revealing the anomaly. Data were tested by Chi-square test analysis. Results Among 1015 patients, anomaly was seen in 767 cases (75.57%). High jugular bulb was found in 414 patients, accounting for 40. 79%. Sigmoid sinus anomaly was detected in 387 patients (38. 13%), while otitis media was found in 148 cases (14. 58%), and low middle cranial fossa in 70 cases (6. 90%). The positive rate of HRCT in the detection of high jugular bulb was 54. 89% (365/665), which was significantly higher than those of other methods (P < 0. 05). The positive rate of enhanced HRCT in showing sigmoid sinus anomaly was 73.68% (56/76), which was significantly higher than those of other methods(P <0. 05). Sigmoid sinus anomaly was the most frequent finding in patients with PT, accounting for 66. 21% (96/145). The incidence of sigmoid sinus anomaly was higher in PT than in NPT (291/870,33.45% ;χ2 =56. 537 ,P <0. 01). The fast imaging employing steady-state acquisition (FIESTA) sequence was the best examination method in displaying the vessel within the internal auditory canal (42/42,100%).Conclusions High jugular bulb and sigmoid sinus anomaly were the most frequent abnormal findings of temporal bone in patients with tinnitus. Enhanced HRCT was the choice of modality in patients with PT.Plain HRCT was recommended for NPT. FIESTA sequence was the best in the evaluation of the vessel within the internal auditory canal.  相似文献

Penetrating neck injuries constitute 5-10% of all trauma cases. These injuries may cause life-threatening suppurative or vascular complications, but the severity and extent of damage depends upon the inflicting object and the involved structures. If significant complications are not expected, then it is best to leave the foreign body embedded and avoid surgical risks. We present a rare case of a foreign body embedded in the neck causing tinnitus and foreign body sensation.  相似文献

There is a controversy in regards to the efficacy of photobiomodulation (PBM) in the management of tinnitus. The aim was to systematically review randomized controlled trials (RCTs) that assessed the efficacy of PBM (low-level laser therapy) in the management of tinnitus. The focused question was “Is PBM effective in the management of tinnitus?”. Indexed databases were searched up to and including June 2020 using different combinations of the following key words: (a) laser; (b) diode; (c) low-level laser therapy; (d) photobiomodulation; (e) tinnitus; (f) medium-level laser; (g) photo-biomodulation; and (h) low-power laser; and RCTs performed on humans were included. Letters to the editor; case reports/series; commentaries; experimental studies and historic reviews were excluded. The risk of bias was assessed using the modified cochrane collaboration tool. The format of the current systematic review was personalized to summarize the appropriate information. Ten RCTs (2 single-blinded and 8 double-blinded) were included. One study reported 30% and 100% resolution of tinnitus using diode and Neodymium-doped Yttrium Aluminum Garnet lasers; respectively. One study reported that PBM was effective in relieving tinnitus for up to 3 months. Eight studies reported that PBM was ineffective in the management of chronic tinnitus. The risk of bias was high; medium and low in 4; 5 and 1 studies; respectively. The effectiveness of PBM in the management of tinnitus remains debatable. Further power-adjusted and well-designed RCTs with long-term follow-up are needed.  相似文献

Hearing loss is a serious condition affecting more than 1.5 billion people globally. Many affected people benefit from the use of devices, such as hearing aids, but these do not restore natural hearing, and many users still struggle to follow speech in the presence of background noise. Consequently, there is rapid growth in work to discover therapeutics to address this need. Our analysis of the therapeutic pipeline for inner ear and central processing disorders identified 23 assets in clinical trials and 56 in preclinical development, of which 25% have entered the pipeline in the past three years. The innovative potential of this pipeline is encouraging, but there are translational hurdles to be overcome. We highlight challenges for the pipeline and comment on opportunities to support and strengthen it.  相似文献

Background

Tinnitus is a common disease in public. It is not only associated with neuronal, muscular, vascular pathologies, but also with related psychological parameters. In this study, we aimed to investigate the relationship between tinnitus, anxiety and depression in patients undergoing tympanoplasty.

Methods

Patients with tinnitus and operated for chronic otitis media were included in our study. Before and after the operation, tinnitus handicap inventory, beck anxiety and depression scales were filled and pre- and postoperative values were compared. In addition, our patients were divided into two groups as tinnitus improve and did not improve and differences between them were investigated.

Results

148 patients were included in our study. Of 148 patients, 60 were male and 88 were female. There was no significant difference between the patients with and without tinnitus when the dermografic features, hearing levels and physical examination findings were compared.After the operation, the patients who did not improve tinnitus had higher levels of depression and depression than others (<0.001).

Conclusion

As a result of our study, anxiety and depression scales of the patients whose tinnitus did not improve were found to be higher than the patients whose tinnitus improved.  相似文献

Objective

This study was conducted to investigate the effectiveness of anodal and cathodal methods in reducing the intensity of tinnitus and to compare them with the control.

Methodology

This randomized double-blind clinical trial with case and control groups was conducted in Al-Zahra Hospital in Isfahan between 2015 and 2016. In this trial, 51 patients with tinnitus, for at least one year, were selected among those outpatients visiting the throat, nose and ear clinic within this period. Inclusion criteria were patients on electrical stimulation prohibition, with Ménière's disease, otosclerosis, chronic headache, and pulsatile tinnitus. Patients were randomly divided in three equal-sized groups: anodal stimulation group, cathodal stimulation group, and control group. The subjects received 20-min current stimulation (2?mA). Five subjects were selected from those with a significant difference between the stimulated states (anodal or cathodal) and/or control. They received weekly transcranial electrical stimulation for two months, and their long-term recovery from tinnitus was investigated. Data analysis was done with SPSS20.

Results

Findings showed no significant between-groups difference in mean scores of tinnitus before the intervention (p?=?.68); whereas, this difference was significant immediately after the intervention (p?=?.02) and 1?h after it (p?=?.03). The mean score of tinnitus in the anodal stimulation group was significantly lower than the control; whereas, no significant difference was observed between the anodal and cathodal stimulation groups, and between the cathodal and control groups (p?<?.05). Findings also showed that the mean scores of tinnitus in two cathodal stimulation groups (p?=?.24) and control group (p?=?.62) were not significantly different at three different points of time; whereas, this score was significantly different in the anodal group at these time points (p?=?.01).

Conclusion

In conclusion, anodal stimulation was more effective than the cathodal and control stimulation in reducing the intensity of tinnitus in the short term.  相似文献

Objective

This study analyzed the characteristics of tinnitus identified in anemia patients with cohort- and population-based studies in a single institute and suggests a management algorithm.

Methods

Fifty patients who were treated for anemia and referred for tinnitus treatment were included in a single institute retrospective study. Characteristics of tinnitus were investigated in a correlation analysis with demographic and audiologic parameters. For the population-based study, data from the Korea National Health and Nutrition Examination Survey collected between 2010 and 2011 were analyzed. The study population consisted of 11,402 individuals aged 20–97 years with complete tinnitus-related data. The prevalence of tinnitus in anemia patients was investigated using the questionnaire, and associations between tinnitus and blood/urine parameters were evaluated by binary logistic regression analysis.

Results

In a single-institute study, patients with non-pulsatile tinnitus were significantly older and their initial hemoglobin was higher than those with pulsatile tinnitus (p = 0.001, 0.008, respectively). In pulsatile tinnitus, age and difference between initial and post-treatment hemoglobin were significantly associated with a subjective improvement in tinnitus (p = 0.002, 0.016, respectively). There were no significant audiologic or hematologic parameters associated with the improvement of non-pulsatile tinnitus. In the population-based study, there was no significant correlation between anemia and tinnitus (p = 0.064). In a multivariate analysis, age was the only parameter associated with tinnitus in participants with anemia.

Conclusion

The therapeutic strategy and prognosis of tinnitus in anemia patients differ according to the characteristics of tinnitus and the severity of anemia.  相似文献

目的通过分析耳鸣与听力下降的关系,总结耳鸣与不同类型听力下降的关联性,以及不同类型听力下降所伴有耳鸣的治疗效果。方法分析2016年1月~2017年6月因“耳鸣”或者因“听力下降伴有耳鸣”就诊的急性耳鸣患者188例,其中男102例,女86例;年龄23~62岁,平均年龄44岁。双侧耳鸣者65例,单侧耳鸣者123例。所有患者入院时均进行电测听及耳鸣检查,根据听力曲线类型对耳鸣患者进行分组,其中听力正常者12例,低频听力下降者36例,高频听力下降者84例,平坦听力下降者42例,全聋型14例。入院后所有患者予以行营养神经、改善循环、激素冲击等治疗,并于入院当日、第3天、第5天及第10天行听力检查及耳鸣检查,比较各组患者耳鸣治疗的疗效。结果不同听力下降类型的耳鸣患者构成比比较,差异具有统计学意义(P<0.05)。听力正常者耳鸣治疗有效率为91.67%(11/12),低频听力下降组耳鸣治疗有效率为83.33%(30/36),高频听力下降组耳鸣治疗有效率为54.76%(46/84),平坦型听力下降组耳鸣治疗有效率为69.05%(29/42),全聋型听力下降组耳鸣治疗有效率为28.57%(4/14),对各组的耳鸣治疗有效率进行比较,差异具有统计学意义(P<0.05)。结论高频听力下降者耳鸣发生率最高,听力正常者耳鸣发生率最低。听力正常及低频听力下降者耳鸣治疗有效率最高,高频听力及全聋者耳鸣治疗效果较差。  相似文献

Introduction

Tinnitus and sound intolerance are frequent and subjective complaints that may have an impact on a patient's quality of life.

Objective

To present a review of the salient points including concepts, pathophysiology, diagnosis and approach of the patient with tinnitus and sensitivity to sounds.

Methods

Literature review with bibliographic survey in LILACS, SciELO, Pubmed and MEDLINE database. Articles and book chapters on tinnitus and sound sensitivity were selected. The several topics were discussed by a group of Brazilian professionals and the conclusions were described.

Results

The prevalence of tinnitus has increased over the years, often associated with hearing loss, metabolic factors and inadequate diet. Medical evaluation should be performed carefully to guide the request of subsidiary exams. Currently available treatments range from medications to the use of sounds with specific characteristics and meditation techniques, with variable results.

Conclusion

A review on tinnitus and auditory sensitivity was presented, allowing the reader a broad view of the approach to these patients, based on scientific evidence and national experience.  相似文献
