Taken-for-granted assumptions about the clinical experience of newly graduated registered nurses from their pre-registration paid employment: A narrative inquiry

Paid employment within clinical setting, such as externships for undergraduate student, are used locally and globally to better prepare and retain new graduates for actual practice and facilitate their transition into becoming registered nurses. However, the influence of paid employment on the post-registration experience of such nurses remains unclear. Through the use of narrative inquiry, this study explores how the experience of pre-registration paid employment shapes the post-registration experience of newly graduated registered nurses. Repeated individual interviews were conducted with 18 new graduates, and focus group interviews were conducted with 11 preceptors and 10 stakeholders recruited from 8 public hospitals in Hong Kong. The data were subjected to narrative and paradigmatic analyses. Taken-for-granted assumptions about the knowledge and performance of graduates who worked in the same unit for their undergraduate paid work experience were uncovered. These assumptions affected the quantity and quality of support and time that other senior nurses provided to these graduates for their further development into competent nurses and patient advocates, which could have implications for patient safety. It is our hope that this narrative inquiry will heighten awareness of taken-for-granted assumptions, so as to help graduates transition to their new role and provide quality patient care.     

Foreign nurse importation and the supply of native nurses

The importation of foreign registered nurses has been used as a strategy to ease nursing shortages in the United States. The effectiveness of this policy depends critically on the long-run response of native nurses. We examine the effects of immigration of foreign-born registered nurses on the long-run employment and occupational choice of native nurses. Using a variety of empirical strategies that exploit the geographical distribution of immigrant nurses across US cities, we find evidence of large displacement effects – over a ten-year period, for every foreign nurse that migrates to a city, between 1 and 2 fewer native nurses are employed in the city. We find similar results using data on nursing board exam-takers at the state level – an increase in the flow of foreign nurses significantly reduces the number of natives sitting for licensure exams in more dependent states relative to less dependent states. Using data on self-reported workplace satisfaction among a sample of California nurses, we find suggestive evidence that part of the displacement effects could be driven by a decline in the perceived quality of the workplace environment.     

临床检验人员社会支持与职业自我概念的状况研究

目的：调查临床检验人员的职业自我概念与社会支持状况，以探讨两者之间的相关性。方法采用社会支持评定量表（SSRS）和职业自我概念量表（PSCS）对82名临床检验人员进行问卷调查。结果临床检验人员SSRS总分评分为（35.56±4.37）分，为一般水平，PSCS总分评分为（34.29±5.48）分，处于低水平。相关分析显示，职业自我概念与社会支持具有高度相关性（P＜0.01）。结论临床检验人员社会支持和职业自我概念状况不佳，社会支持是影响职业自我概念的因素。     

情景模拟教学法在低年资护士基础生命支持培训中的应用

目的探讨情景模拟教学法对急诊科低年资护士基础生命支持培训的实践效果和应用体会。方法应用情景模拟教学法对我科24名N0、N1级护理人员进行理论和操作培训,培训结束后予以考核。并对培训前后急诊临床教学质量及各级医护人员的满意度进行评价。结果自实行情景模拟教学法培训以来,护理人员的心肺复苏考核成绩较前有较大提升(P0.01);各级医护人员的满意度均有所提高,受训护士自身满意度与情景模拟教学培训前比较,差异有统计学意义(P0.05)。结论情景模拟教学因材施教,效果显著,增进了急诊科教学效果,提高了急诊教学满意度。
