MR appearance of parasymphyseal insufficiency fractures of the os pubis

 Objective. To clarify the MRI features of parasymphyseal insufficiency fractures of the os pubis. Design and patients. MRI was performed in four postmenopausal women with parasymphyseal insufficiency fractures. The diagnosis was confirmed with plain films in every patient. T1-weighted and T2-weighted images were obtained in four patients using a 1.5-T unit. Postcontrast T1-weighted imaging was also done in three patients. Results and conclusions. MRI of pubic parasymphyseal insufficiency fracture characteristically demonstrates a hyperintense mass lesion with a hypointense rim on T2-weighted imaging, showing peripheral and septal enhancement after contrast administration. It is important to have this entity in mind in patients with osteoporosis, especially in patients with a history of pelvic irradiation for malignant disease, so as not to misinterpret it as a chondroid tumor or bone metastasis.     

反映手术间利用效率的统计指标与方法探讨

目的 探索反映手术室工作状态的指标和方法，为了解科室需求、合理利用手术室资源奠定基础。方法 提取医院局域网中有关手术安排、手术过程、麻醉等信息，通过常用统计指标、统计图表，反映手术室、手术科室、手术间的工作状态。结果 工作日手术问日均工作6．34小时，节假日日均工作0．6l小时。工作日13点后有近l／3的空台，14点后近l／2空台。个别手术间利用率偏低。结论 1．由在用手术间时点统计结果可见，14点后接台手术尚有一定潜力，节假日安排手术潜力更大；2．工作日间均工作时数已近满负荷，若要进一步提高手术间的利用，需加强麻醉科和手术室力量。     

手术室护生发生差错的原因分析及对策

目的防范护生在手术室实习期间发生护理差错。方法对护生发生差错的常见原因进行分析,制定相应的防范措施。结果通过认真分析差错原因,制定相应防范措施,两年来避免了护理教学差错发生,使手术室的护理教学取得明显效果。     

Brain abscess in Saudi Arabia

There are significant variations among countries in the incidence of brain abscess. We report here 26 cases of brain abscess treated at the Neurosurgery Department of King Faisal University and Dammam Central Hospital Saudi Arabia over a six year period (1982–1988). This is 2.3% of total admissions to the two neurosurgery departments serving a population of approximately 1.2 million in the same period.Young males were most often affected (M/F ratio 3.3:1; 31% were less than 15 years old, 46% aged between 15–39 years, and 23% older than 40 years). Streptococcus was found to be the most common microorganism (38.4%). Mixed infection was seen in 15.3%, and sterile abscesses were found in 11.5% of the patients after aerobic and anaerobic cultures of the pus. Chronic otitis media and paranasal sinusitis predisposed the patients to abscess formation in 57.6% of the cases. The temporo-parietal area was the commonest site. Epilepsy was a complication in 30.7% of our patients, and the mortality rate was 15.3%.     

我国正常中老年人心率变异分析

心率变异分析（ＨｅａｒｔｒａｔｅｖａｒｉａｂｉｌｉｔｙＨＲＶ）目前被认为是一项预测心性猝死，特别是预测心肌梗死后病人猝死危险性的独立、敏感的指示。目前尚缺乏大样本ＨＲＶ时相分析正常值范围。本研究采用英国ＯＸＦＯＲＤ公司ＭｅｄｉｌｏｇＥＸＣＥＬ长程心电图分析系统，分别用标准差法（Ｓｔａｎｄａｒｄｄｅｖｉａｔｉｏｎ，ＳＤ）和ＨＲＶ指数法（ＨＲＶＩｎｄｅｘ）对１２０名（男：女＝９７：２３）４０一７０岁（平均５７岁，＜６０岁者６６人，≥６０岁者５４人）健康人的２４小时ＨＲＶ值进行测算。其结果为，ＳＤ：１１８．１５±１５．３７ｍｓ．ＨＲＶＩｎｄｅｘ１８．６２±２．３１；中年组（＜６０岁）平均ＳＤ：；１２２．０３ｍｓ．平均ＨＲＶＩｎｄｅｘ：２０．１６；老年组（≥６０岁）平均ＳＤ：１０７。９２ｍｓ．平均ＨＲＶＩｎｄｅｘ：１６．４１。两种方法的结果均显示。ＨＲＶ日间低于夜间，中年人高于老年人，３小时９时ＨＲＶ最低，与晨间冠心病急性发作事件高峰相吻合。本文并对ＨＲＶ的昼夜变化规律及其意义进行了初步探讨。

苹果常温保鲜技术

采用AF型气氛保鲜纸对苹果进行常温保鲜效果试验,并与其他保鲜方法进行了对比试验.该法具有简单易行、投资少、见效快等特点,可用于苹果常温贮藏保鲜.结果表明,用AF型气氛保鲜纸包裹苹果,可使苹果在常温下贮藏保鲜期稳定在140 d以上,失重率降低66.9%,皱皮率降低90.6%,腐烂率降低87.3%,硬度和可溶性固形物含量基本不变.     

Integrated home monitoring predicts lead failure in a pacemaker dependent 4-year-old girl.

A 4-year-old girl with post-surgical complete atrioventricular block received an epicardial dual chamber pacemaker system. During further growth intermittent exit block occurred, first misinterpreted as neurological seizures. The epicardial lead was replaced using a transvenous approach, and a pacemaker with an integrated home monitoring facility was implanted. After her discharge, a rise in the pacing threshold automatically initiated an event message. On the basis of this information, the patient was called in and imminent dislodgement of the ventricular lead was diagnosed by x-ray. The lead was repositioned and was found stable over 1-year follow-up.     

STANDARD IMAGING TECHNIQUES OF ENDOSCOPIC ULTRASOUND‐GUIDED FINE‐NEEDLE ASPIRATION USING A CURVED LINEAR ARRAY ECHOENDOSCOPE

Standard imaging techniques using a curved linear array echoendocope are summarized to facilitate the attainment of expertise in endoscopic ultrasonography and endoscopic ultrasound‐guided fine needle aspiration, and to promote the widespread use of this diagnostic and therapeutic tool. Typical images of the mediastinal organs, the bilio‐pancreatic systems and neighboring organs by scanning from the esophagus, stomach, duodenal bulb, and descending portion of the duodenum, are shown in a sequential manner. The basic techniques of endoscopic ultrasound‐guided fine needle aspiration are also presented.
