Metastatic tumors of the hand: a report of six cases.

Six metastatic tumors of the hand of mammary, renal, pulmonary, colonic, and humeral origins are described. The areas involved were the distal phalanges, the metacarpal bones, a carpal bone, the nail bed, and the soft tissue of the dorsum of the hand. All six patients died within a few months after the metastatic lesions were diagnosed correctly. Metastases to the bone can cause destruction, pain, and swelling and may be misdiagnosed before operation as osteomyelitis. Metastases to the soft tissues tend to be asymptomatic and can closely mimic benign soft tissue tumors.     

Study of function and maturation of monocytes in morbidly obese individuals

In recent years there has been an intense interest in understanding the mechanisms involved in immune modulation associated with dietary restriction. In this study we have investigated maturation of peripheral blood monocytes in morbidly obese patients using an in vitro culture system. At the end of the culture, the matured monocytes (macrophages) were quantitated. Though there was no detectable functional defect observed in the matured monocytes in vitro, there was a quantitative difference in maturation among the monocytes plated. The number of monocytes that matured into macrophages in obese patients was 3.29 ± 2.41 × 10⁴ cells per milliliter of blood, as compared to 8.24 ± 3.14 × 10⁴ per milliliter of blood in normal volunteers. This difference was found to be statistically significant at P < 0.001. These results indicate a possible development of an intrinsic defect among morbidly obese individuals.     

Primary omental leiomyosarcoma masquerading as hemorrhagic ascites

An 85-year-old man with unexplained hemorrhagic pseudoascites of four years' duration was discovered to have a primary omental leiomyosarcoma. Neoplasms arising in the greater omentum are rare. To date, only three primary omental leiomyosarcomas have been described. Clinical and pathologic findings of this case are presented and the topic of primary omental neoplasms is reviewed.     

Pathologic features of the lung in the immunocompromised host

Acute pulmonary disease is a major complication of immunodeficiency, and it has become increasingly important with the expanded use of immunosuppressive drugs. When routine clinical evaluation fails to identify a specific etiologic agent, a morphologic diagnosis is pursued by means of one or more invasive procedures. Interpretation of the material obtained by these procedures poses a challenge to pathologists. In this paper, the important histopathologic patterns of pulmonary disease likely to be encountered in this setting are reviewed, with emphasis on differential diagnosis. In addition, various diagnostic techniques are discussed and compared, with regard to interpretation of findings and diagnostic yields.     

Multiple primaries among gynecologic malignancies

Seventy-eight synchronous or metachronous tumors among 2362 patients followed by the Downstate Gynecologic Tumor Registry are reviewed. Significant synchronous tumor pairs include cervix (invasive and in situ)-ovary, cervix (in situ)-uterus, cervix (in situ)-kidney, endometrium-ovary, endometrium-rectosigmoid and ovary-breast. Significant metachronous pairs include cervix (invasive and in situ combined)-lung, cervix (invasive and in situ combined)-upper alimentary tract, and cervix (invasive)-rectosigmoid. In the case of in situ and invasive cervical cancer-lower genital tract, significance was determined for both synchronous and metachronous pairs. Long survival is an important factor in the appearance of a second tumor as demonstrated in patients with cervical carcinoma. Synchronous data prove to be valuable in assessing risk of second primaries in patients surviving for short periods. The roles of cigarette smoking, hormones, immunosuppression, radiotherapy, and screening are discussed.     

黄氏通关汤内服坐浴治疗慢性前列腺炎临床观察

目的观察黄氏通关汤内服、坐浴综合治疗慢性前列腺炎的临床疗效。方法56例慢性前列腺炎患者随机分为治疗组38例和对照组18例。治疗组采用中药内服、坐浴治疗;对照组采用口服氟哌酸加前列康治疗。2组均以1月为1个疗程,2个疗程后统计疗效。结果总有效率治疗组为94.74%,对照组为72.22%,两组比较有显著性差异(P<0.01)。结论中药内服、坐浴治疗慢性前列腺炎疗效确切。
