A “HOLTER” for Parkinson’s disease: Validation of the ability to detect on-off states using the REMPARK system

The treatment of Parkinson's disease (PD) with levodopa is very effective. However, over time, motor complications (MCs) appear, restricting the patient from leading a normal life. One of the most disabling MCs is ON-OFF fluctuations. Gathering accurate information about the clinical status of the patient is essential for planning treatment and assessing its effect. Systems such as the REMPARK system, capable of accurately and reliably monitoring ON-OFF fluctuations, are of great interest.ObjectiveTo analyze the ability of the REMPARK System to detect ON-OFF fluctuations.MethodsForty-one patients with moderate to severe idiopathic PD were recruited according to the UK Parkinson’s Disease Society Brain Bank criteria. Patients with motor fluctuations, freezing of gait and/or dyskinesia and who were able to walk unassisted in the OFF phase, were included in the study. Patients wore the REMPARK System for 3 days and completed a diary of their motor state once every hour.ResultsThe record obtained by the REMPARK System, compared with patient-completed diaries, demonstrated 97% sensitivity in detecting OFF states and 88% specificity (i.e., accuracy in detecting ON states).ConclusionThe REMPARK System detects an accurate evaluation of ON-OFF fluctuations in PD; this technology paves the way for an optimisation of the symptomatic control of PD motor symptoms as well as an accurate assessment of medication efficacy.     

全自动毛细管电泳技术在筛查地中海贫血中的临床诊断价值

目的探讨全自动毛细管电泳技术在地中海贫血筛查中的临床诊断价值。方法选取2008年9月-2012年4月我院筛查的380例患者作为研究对象，分别采用琼脂糖凝胶电泳技术和全自动毛细管电泳技术对地中海贫血进行筛查，以通过PCR基因诊断技术进行确诊的结果为参照，比较两种筛查技术的检出率和血常规检查结果，以明确其临床价值。结果采用PCR基因诊断技术在380例患者中共有108例确诊为地中海贫血。对于α-地中海贫血，全自动毛细管电泳技术的检出率为90．91％，明显高于凝胶电泳技术的检出率65．91％；对于β-地中海贫血，毛细管电泳技术和凝胶电泳技术的检出率分别为94．23％和71．15％，全自动毛细管电泳技术检出率明显高于凝胶电泳技术；地中海贫血的MCV、CH均值相对于异常血红蛋白病HbE、HbCS值稍低，差异有统计学意义（P〈O．05）。结论全自动毛细管电泳技术在地中海贫血筛查中检出率高，具有显著的优越性，能够为我国地中海贫血诊断工作提供更有力的技术保障。     

Comparison of three devices for measuring blood pressure

Blood pressure measurement with two automatic devices, Dinamap 845 and Sentry, was compared with the standard mercury sphygmomanometer, by means of a 3-period crossover experiment. Both automatic devices read diastolic pressure considerably lower than the mercury sphygmomanometer (about 7 mmHg on average); agreement was better for systolic pressure. Device differences in individual patients were often much higher. To assess the patient's emotional pressor response during manual measurement, Dinamap was used to monitor blood pressure before, during and after measurement with the mercury sphygmomanometer. No rise in pressure was found. It is concluded that the three devices can often give different values, and that the physician's involvement cannot explain these differences.     

Determination of Degree of Substitution of Formyl Groups in Polyaldehyde Dextran by the Hydroxylamine Hydrochloride Method

Colorimetric or potentiometric titration of the aldehyde residues in polyaldehyde dextran by the hydroxylamine hydrochloride/sodium hydroxide method has been found to be a convenient and accurate method to determine formyl content. Nitrogen combustion analyses on the isolated oximes confirmed the titrametric results.

以自制的涂碳空白PVC膜电极为指示电极、四苯硼钠标准溶液为滴定剂,测试了26种药物的电位滴定曲线,进行了回收率和方法对比实验,获良好结果。本文还用三参数法处理了所有的电位滴定曲线,评价了不同滴定体系的灵敏度及其优劣次序。     

A sampling study of bedside nursing activity in a cardiac surgical intensive care unit. Part 2

For a selected group of 17 patients following cardiac surgery, 33 discrete elements of nursing workload have been defined, and the nurse's bedside activities logged at 1 min intervals throughout the 24 h immediately following the patient's return from the operating theatre. It is possible to identify three broad types of activity undertaken by the bedside nurse; technical nursing care (Type 1), intermittent nursing care (Typy 2), and the balance of the nurse's time, largely concerned with observation and liaison with other staff — learning activities (Type 3). The time devoted to technical nursing care reduces significantly over the patient's first 24 h in the ward. On the other hand the time devoted to intermittent nursing care and learning activities is shown to be shift dependent. The effect of the efficient utilisation of computer technology is estimated, and it is demonstrated that the implementation of such technology would result in the expansion of the time available for the nurse to observe the patient. The potential benefits and drawbacks of this are discussed.
