A RETROSPECTIVE STUDY OF ARYTENOID DISLOCATION AFTER ENDOTRACHEAL INTUBATION

Five cases of arytendid dislocation after intuhation under general anesthesia are reported in this article.The anatomical and anesthetic causes of this complication were studied restrospectively, and the methods of treatment were discussed. The result showed that the incidence of arytenoid dislocation was about 0. 09% (5/5 826). Hoarseness, dysphonia and difficulty in feeding were the prevalent symptoms, and poor mobility of the vceal fold was the most common sign. The treatment included closed reduction of the displaced arytenoid under local anesthesia, steroid and physical therapy. The possible causes of arytenold dislocation were summarized as follows, (1) trauma produced by the blade tip of laryngoscope, (2) the use of styler,(3) overstretching of aryepiglottic fold and vocal cord during laryngoscopy and intubation, and (4) coughing and movemaent of the endotracheal tube on the larynx during the endotracheal tuba in place. It was suggested that correct technique and gentle manipulation during intubation could reduce the incidence of the complication. Timely postoperative follow-up and early reduction of arytenold cartilage should be emphasized in the diagnosis and treatment of the complication.     

Value of multidetector spiral CT in diagnosis of acute thoracolumbar spinal fracture and fracture-dislocation

Objective: To investigate the diagnostic value of multidetector spiral CT (MSCT) in acute thoracolumbar spinal fracture and fracture-dislocation. Methods: CT imaging files of 152 consecutive traumatic patients with thoracolumbar fractures were retrospectively reviewed. MSCT scannings were performed with a collimation of 3-5 mm and a pitch of 5.5. The postprocessing included sagittal and coronal multiplannar reconstruction, and 3-D reconstruction. Results: There were 88 cases of compression fracture, 54 cases of burst fracture and 10 cases of fracture-dislocation. Transverse images of MSCT could visualize all fractures directly and determine whether spinal canal was intact. Postprocessing image was helpful in depicting the displacement of fragment and orientation of dislocation. Conclusions. MSCT plays an important role in diagnosis and management of acute thoracolumhar spinal fracture and fracture-dislocation.     

Crouzon综合征一例

患者，男 44岁 因双眼外斜，视力差，2006年6月来院行残疾鉴定，患者自幼即双眼外斜，突眼，并逐渐加重，视力不佳，现突眼严重，穿脱套头衫时易使眼球脱臼，眼睑闭合不全常双眼发红。     

胃镜检查致环杓关节脱臼一例

患者男，52岁，主因上腹部疼痛1个月接受胃镜检查，常规用利多卡因胶浆口服咽部表面麻醉，应用富士能450型胃镜进行检查，插镜及检查过程顺利，出镜后患者声音嘶哑，喉部有不适感，经局部雾化治疗3天后仍声音嘶哑，胃镜下观察发现左侧声带松驰，闭合差，左杓状软骨活动受限，高于右侧（图1）。经耳鼻喉科医师会诊考虑环杓关节脱臼，先后进行2次纤维喉镜下拨动复位及1次间接喉镜下拨动复位，于脱臼后3周声音嘶哑症状消失，随诊半年病情无反复。     

老年跌倒研究概况与进展

跌倒是指突发、不自主、非故意的体位改变，倒在地面或比初始位置更低的平面上；按照国际疾病分类（ICD-10）对跌倒的分类，包括以下两类：①从一个平面至另一个平面的跌落；②同一平面的跌倒。跌倒是老年人常见健康问题之一。据报道，65岁以上老年人每年约有1／3的人跌倒1次以上，而且比例随着年龄增长而增加，80岁以上老年人跌倒的年发生率可高达50％。跌倒不仅使患者感到恐惧和焦虑，而且跌倒后约5％～15％会造成脑部损伤、软组织损伤、骨折、脱臼等并发症。     

醉酒后，胳膊受伤了

凌晨6点不到，急诊科打来电话说，有个病人酒后睡了一觉，醒来时右侧整个胳膊突然动不了了，照X光片没发现有关节脱臼或骨折，要求骨科会诊。

双乌透骨酒 配方：生川乌、生草乌、透骨草、伸筋草、祁艾叶、山奈各20g，西红花、桃仁、冰片（或樟脑）、细辛、桂枝各10g，乳香40g，95％乙醇2500mL。
