大黄联合血液净化治疗马蜂蜇伤并多脏器功能障碍的疗效观察

目的:观察大黄联合血液净化治疗马蜂蛰伤并发多脏器功能障碍(MODS)的疗效。方法:选取病历资料完整的马蜂蜇伤并发MODS且急性肾损伤(AKI)Ⅱ期以上患者89例,按随机数字表法分为血液净化联合大黄组(治疗组)和单独血液净化组(对照组),观察两组患者治疗前后血尿素氮、血肌酐、N-乙酰-β-d氨基葡萄糖、丙氨酸氨基转移酶、天冬氨酸氨基转移酶、总胆红素、直接胆红素、肌酸激酶水平,心电图异常者例数,少尿期时间,住院时间,血液净化次数及疗效。结果:两组患者心电图异常者例数与治疗前比较,差异具有统计学意义(P>0.05);治疗组与对照组的少尿期时间、住院时间、血液净化次数及疗效比较,差异具有统计学意义(P<0.05)。结论:大黄联合血液净化治疗马蜂蛰伤并发MODS的疗效优于单独血液净化治疗。     

Skin problems from marine arthropods

While the majority of the phylum Arthropoda are terrestrial, some of the aquatic members can be harmful to human skin. While none of the aquatic arthropods are known to be venomous, traumatic injuries from claws or spines can cause penetrating and laceration injuries which may lead to secondary infection. Bite reactions, poisoning from ingestion, and contact reactions are also seen due to marine arthropods. Preventive measures and treatment are discussed.     

Uptake of dopamine by rat hepatocytes in vitro

The present results showed that uptake of dopamine (DA) by rat isolated hep-atocytes was mediated, in addition to simple diffusion, mainly by a transporter-involved process, with Km of 66. 8 μmol and Vmax of 52. 3 pmol·min-1/105 cells. The process was pH- and temperature-dependent and required an activation energy of 4. 12 kcal ·mol-1(Q10 = 1.25) in the range of 2.0-12.7 C and 13.0 kcal·mol -1(Q10 = 2. 0) in the range of 12.7 -39.0C. Cysteine residue having free thiol group was. unrelated to the activity of the transporter. Catecholamines, serotonin, and cocaine inhibited the DA transport , but tyramine (TA) and tryptamine, as well as benztropine and imipramine (which are potent inhibitors for hepatic TA transporter and neuronal DA transporter), had no inhibitory effect on the transport of DA in these cells. These results indicated that DA was taken up into hepatocytes by a distinct carrier. 不同剂量连续性静脉-静脉血液滤过联合血液灌流对蜂蛰伤并发急性肾损伤疗效的影响

目的探讨以不同置换液剂量行连续性静脉-静脉血液滤过（CVVH）联合血液灌流（HP）对蜂蛰伤并发急性肾损伤（AKI）疗效的影响。方法回顾性分析西南医院、新桥医院和解放军第452医院2003年1月至2009年11月行CVVH联合HP治疗的蜂蛰伤并发AKI患者45例的临床资料,按置换液剂量分为中小剂量组[35～45 mL/（kg.h）]、大剂量组[45～70 mL/（kg.h）]。结果两组患者年龄、体重指数、死亡率及血液净化治疗前肌酐、急性生理学和慢性健康状况评分Ⅱ（APACHEⅡ评分）无统计学差异（P〉0.05）;中小剂量组少尿期持续时间、平均连续血液滤过治疗时间明显长于大剂量组（P〈0.05）;两组经CVVH联合HP治疗24 h的APACHEⅡ评分无统计学差异（P〉0.05）,中小剂量组经CVVH联合HP治疗48 h的APACHEⅡ评分明显高于大剂量组（P〈0.05）。结论以大剂量行CVVH联合HP有助于缓解蜂蛰伤并发AKI的病情。
