The treatment of severe trauma, especially multiple injuries, requires multidisciplinary collaboration. The current study aims to highlight the challenges of consultation mode for severe trauma in general hospitals and emphasizes the need to create a new temporary-sustainable team. It suggests developing an information consultation mode and enforcing the fine management to improve the quality and safety of the medical treatment. The management mode of a temporary-sustainable team will reduce the cost and improve the treatment efficiency. Overall, a temporary-sustainable team has significant advantages over a traditional multidisciplinary team for severe trauma treatment.  相似文献

目的 构建并实施以个案管理师为主导的肺康复门诊,为慢性阻塞性肺疾病患者全程肺康复管理提供借鉴。 方法 构建由个案管理师,呼吸科、心理科、营养科、康复科医生组成的多学科团队,依托肺康复门诊,对106例慢性阻塞性肺疾病患者实施以个案管理师为主导的全程肺康复模式。评价入院时、出院前和出院后3个月患者的肺功能指标及入院时、出院前的健康心理控制源倾向。 结果 患者入院时、出院前和出院后3个月的肺功能指标比较,差异有统计学意义(均P<0.01);出院前患者健康心理控制源内控评分显著高于入院时,他控和机遇控评分显著低于入院时(均P<0.01)。 结论 依托肺康复门诊,实施以个案管理师为主导的全程肺康复模式,规范推进慢性阻塞性肺疾病患者肺康复开展,可以改善患者肺功能指标,提高患者健康心理控制能力,从而改善照护结局,提高患者生活质量。  相似文献

Objectives

Collegiality is considered to be any extra-role behaviour that is discretionary, not recognised by a formal reward system and that promotes the effective functioning of the organisation. Although there is much literature on the concept of collegiality, there are few studies examining collegiality in the medical profession and none looking at collegiality among emergency physicians (EPs). The aim of the present study is to explore the perceptions of different ED healthcare professionals on the meaning of collegiality among EPs, the benefits of collegiality and behaviours they identify as indicative of collegiality.

Methods

This was a qualitative study using grounded theory. Data collection was via focus group interviews of three to four participants per group. Participants included EPs, emergency medicine trainees, senior emergency nurses and nurse practitioners. Three questions were explored: ‘what does collegiality mean to you?’; ‘what are the benefits of collegiality?’; and ‘what specific behaviours do you see as part of collegiality?’

Results

Ten focus group interviews involving a total of 33 participants were conducted. Several themes were identified for both the meaning of collegiality and the benefits of collegiality among EPs. Eight themes regarding collegial behaviour were identified: (i) mutual respect and trust; (ii) mutual support; (iii) attitude; (iv) work ethic; (v) staff welfare; (vi) patient management; (vii) handover; and (viii) education.

Conclusion

The present study identified distinct themes and behaviours indicative of collegiality among EPs. Promoting these behaviours could improve staff well-being, ED efficiency, patient safety and productivity.  相似文献

目的了解衡阳县乡镇慢性病防治管理员(下称慢病管理员)专业水平及高血压患者健康管理服务工作开展情况,为促进慢性病防控、加强慢性病健康管理服务能力提供依据。方法对医务人员采用不记名的问卷调查,按容量调查法随机抽取1 493名高血压病人及家属采取集中和上门相结合的现场调查方法。结果慢病管理员问卷知识调查合格率为70%,与乡镇卫生院门诊、住院医生(临床医生)91.8%的合格率相比,P0.01;与乡村医生(下称村医)、防保员61.52%的合格率比较无差异,P0.05。42.85%的村开展了高血压防治专项健康教育,乡镇卫生院慢病管理员有、无专业技术资质与高血压健康服务工作开展有非常统计学差异。结论引进或聘用有专业技术的人才从事慢病防治管理工作,提高乡镇卫生院负责人的思想认识,加大农村村民健康教育工作力度,指导乡镇慢病管理员和村医把健康知识传播到每一位慢性病患者及患者家庭成员,实现国家对高血压病人健康管理服务目标。  相似文献

Background and objective

While multidisciplinary team (MDT) care in lung cancer is widely practiced, there are few guidelines for MDT on best data collection strategies. MDT meetings need ready access to information for the provision of optimal treatment recommendations (the primary purpose of the meeting), audit of team performance and benchmarking. This study aimed to develop a practical data set designed for these goals through a recognized consensus process with health professionals who participate in formal MDT settings.

Methods

A modified Delphi process with three iterations (two surveys and one consensus conference) was carried out involving over 100 Australian lung cancer MDT health professionals.

Results

In total, 122 lung cancer MDT health professionals responded to the Round 1 survey from over 350 invitees. Of the 122, 98 were available for invitation to Round 2. Of 98, 52 (53%) invitees responded to the Round 2 survey. After two rounds, 51 data elements across 8 domains (patient demographics, risk factors, biopsy data, staging, timeliness, treatment, follow‐up and patient selection) achieved consensus, defined as 80% agreement. For Round 3, 33 MDT lead clinicians were invited to participate in a consensus conference. Of 33, 14 (42%) invitees distilled the 47 data elements into 23 elements across 8 domains to address the study objectives.

Conclusion

A practical data set for lung cancer MDT to use for optimal treatment recommendations and to evaluate team performance was developed through recognized consensus methodology. Access to streamlined, relevant and feasible data collection strategies may improve MDT decision‐making, audit of team performance and facilitate benchmarking.
