Summary The concept of an automated nursing documentation system has been subject to casual debate for a number of years. During this time, Hewlett-Packard developed a system that is both adequate and workable from many perspectives (i.e. the Staff Notes Subsystem of the Patient Data Management System — PDMS). This package has been used as the primary method to document patient care for six years in the Critical Care Units of Phoenix Baptist Hospital. With the aid of Baptist Hospitals and Health Systems (BHHS) Design Engineers, it continues to evolve predicated on a singular intent: To provide the type of documentation required by the medical staff.The objective of this paper is multifold. Foremost, generalized criteria are established to provide a vehicle for standardizing the design or evolution of a staff notes package. The generic structure of the PDMS notes subsystem is discussed in regard to its parity with these criteria. Finally, custom modifications and additions to this package will be presented, and recommendations for future enhancements will be outlined. 相似文献

This paper aims to present a discussion about intrinsic nursing knowledge. The paper stems from the author's study of knowledge claims enshrined in nursing journal articles, books and conference speeches. It is argued that claims by academic nurses have largely depended on principles drawn from continental and not Analytic (British-American) philosophy. Thus, claims are credible only insofar as they defer propositional logic. This is problematic inasmuch as nursing is a practice-based activity usually carried out in medical settings. Transpersonal nursing models are particularly criticizable in respect of their unworldly character as are also concepts based on shallow usages of physics or mathematics. I argue that sensible measurements of the 'real world' are possible--without endorsing positivism--and that nursing requires little recourse to logically unsustainable claims. The paper concludes with an analysis of a recent review of nursing knowledge, which analysis indicates the circularity that attends many discussions on the topic. 相似文献

Despite recognition of the importance of family in health-care and progress in family theory development, there has been limited transfer of family theory to acute care nursing practice. We argue that this family theory-practice gap results from a persistent lack of conceptual clarity in family nursing and other barriers. Lack of conceptual clarity takes the form of conceptual overlap and semantic inconsistency, as well as the complexity of language found in the family nursing literature. Barriers include practice contexts, relational problems, and knowledge types. Our exploration begins with a brief discussion of the intimate link between nursing theory and practice followed by an overview of some issues associated with the family nursing theory-practice gap. Based on a synthesis of family nursing literature, problems associated with conceptual clarity in family nursing theory are explored. We conclude with recommendations for family nursing research to develop concepts grounded in nursing practice. 相似文献

Aim. To develop an instrument to determine nurses’ perceptions of psychologically violent behaviours that they are exposed to in the workplace. Background. According to Leymann, psychological terror or mobbing in work life involves hostile and unethical communication, which is directed in a systematic way towards one individual who, due to mobbing, is pushed into a helpless and defenceless position, and being held there by means of continuing mobbing activities. Design. Survey. Methods. Because nurses who work in hospitals are generally the principle victims of physical, emotional and verbal violence due to the nature of their work environment, the research sample comprised 476 hospital nurses. Data were collected via self‐administered questionnaires. Results. The instrument to determine the perception of workplace psychologically violent behaviours contains 33 items and four factors (individual’s isolation from work, attack on professional status, attack on personality and direct attack). All items have shown statistically significant correlation (p < 0·01); the instrument’s total Cronbach’s α internal consistency coefficient was found to be 0·93. Conclusions. The findings show that the instrument’s validity and reliability are within the limits of an acceptable level and that it is an instrument that will encourage more studies on this subject. Relevance to practice. Defining the psychological pressure that nurses are exposed to in the workplace and determining its negative effects on the victim of workplace psychological pressure and on the institution will make it possible to protect individuals and the institution from psychological violence with both individual and institutional practices. 相似文献
