The long-term effects of disease and treatment on electrophysiologicalmeasures of neurocognitive function were studied in childrenwho had survived acute lymphoblastic leukemia (ALL) for at least4 years and were currently in remission. We report here changesin cognitive processing time as shown by the latency of theP3 wave of the auditory event-related EEG potential (ERP). P3latency was significantly prolonged in long-term ALL surivors,as well as in patients successfully trreated for solid tumors(ST)outside the CNS who received similar chemotherapy but did notreceive prophylactic treatment to the CNS. P3 latencies werestrongly correlated with measures of school performance andIQ in these individuals. The similarity in P3 latency betweenthe ALL and ST groups suggests that the treatments used on thesepateints produce changes in electrophysiological responses thatare associated with mild, but significant, cognitive deficits.  相似文献

目的:探讨dl-3-正丁基苯酞软胶囊(恩必普软胶囊)治疗慢性脑供血不足的临床疗效及对慢性脑供血不足病人认知功能改善的影响。方法:将60例慢性脑供血不足病人随机分为治疗组和对照组,每组30例,治疗组口服dl-3-正丁基苯酞软胶囊,每次200毫克,每日3次;对照组口服复方丹参片,每次4片,每日3次,疗程为8周,治疗前后进行疗效评价和P300数据采样,同时给予同步MMSE、HDS量表评分。结果:治疗组总有效率为70%,对照组总有效率为53.3%,2组治疗后比较差异有显著意义(P<0.05);治疗组治疗前、后MMSE和HDS量表得分情况比较,均有明显改善(P<0.01),与对照组治疗后比较差异有显著意义(P<0.05);治疗组治疗前、后P300波潜伏期及P300波幅差异均有显著意义(P<0.05),与对照组治疗后比较均有显著差异(P<0.05)。结论:dl-3-正丁基苯酞软胶囊对慢性脑供血不足病人安全有效。  相似文献

目的 观察尼莫地平对Alzheimer病(AD)患者认知功能和听觉事件相关电位P300的影响.方法 将65例AD患者随机分成两组,尼莫地平治疗组(尼莫地平组,32例):尼莫地平注射液治疗;吡拉西坦对照组(吡拉西坦组,33例):吡拉西坦注射液治疗.应用简易精神状态检查量表(MMSE)评定两组患者治疗前后认知功能状况,并进行治疗前后P300检查.同时记录药物不良反应.结果 两组各30例进入结果分析.治疗前两组MMSE评分、P300的潜伏期及波幅差异无显著性(均P>0.05).治疗14 d、30 d时,两组MMSE评分显著提高(均P<0.01),P300潜伏期均有缩短,波幅均有提高(尼莫地平组P<0.01,吡拉西坦组P<0.05);治疗90 d时,尼莫地平组MMSE评分显著提高、P300潜伏期显著缩短、波幅显著提高(均P<0.01).治疗后14 d时认知功能改善尼莫地平组明显优于吡拉西坦组(总有效率分别为76.67%,70%)(P<0.05).两组治疗期间无严重不良反应.结论 尼莫地平注射液能够改善AD患者的认知功能,比吡拉西坦更有效;这可能与尼莫地平的脑保护作用有关.  相似文献

目的 探讨老年期抑郁症病人的认知功能状况。方法 按CCMD-3中心境障碍抑郁发作诊断标准,收集33例60岁以上首次抑郁发作的病人(抑郁组),32名健康老年人(对照组)。使用HAMD、HAMA和MMSE量表对所有入组者进行评定,并作P300测定。结果 抑郁组MMSE总分低于对照组。抑郁组N2和P3潜伏期比对照组延长;抑郁组的P300潜伏期N2和P3与HAMD的迟缓因子分呈正相关,与MMSE总分呈负相关。结论 老年期抑郁症病人存在认知功能障碍。  相似文献

本文应用P300地形图的方法,对25例帕金森病患者立体定向术前后进行观察,结果显示:手术前靶刺激P_3波波幅在额区降低,术后P_3波波幅较术前进一步降低,差异非常显著(F_z:P0.02,F_3:P<0.005,F_4:P<0.05)。提示术后近期内可出现脑高级认知功能障碍,并就其产生的原因进行了讨论。  相似文献

The P300-amplitude evoked with an acoustic oddball-paradigm is considered the most stable late event-related potential (ERP). This amplitude-index has become a standard parameter in electrophysiology. Recently, a robust ERP-parameter (NoGo-anteriorization, NGA) has been introduced, which reflects spatial brain electrical changes in relation to execution and inhibition of a motor response elicited with a Continuous Performance Test (CPT). The current study refers to the stability of this new topographical ERP-parameter compared to the stability of the classical P300-amplitude. For that purpose, 12 healthy subjects were investigated with both paradigms during recording of a 21-channel EEG. Analysis of the resulting ERPs revealed a very high stability for both, topographical and amplitude index: In every single subject, the brain electrical fields were characterized by a more anterior location in the NoGo- compared to the Go-condition (=NGA) and by higher amplitudes after target compared to distractor condition. T-tests, analyses of the effect size and of the power revealed equivalent differences between the two contrasting conditions for the topographical compared to the amplitude index. These results indicate that the stability of the topographical ERP-parameters elicited with the CPT is sufficient for an electrophysiological standard-index. The possibility to elicit a robust and specific spatial brain activation with the CPT is an ideal completion to the classical P300 amplitude effect and, therefore, hopefully will be a useful expansion of the standard paradigms in electrophysiological laboratories.  相似文献

The study-test paradigm was used to investigate memory acquisition processes and the effects of repetition on long-term recognition memory. In this procedure, subjects are presented with a list of words (“targets”) to be memorized (Study series). They are later tested for recognition on a word list comprised of the target words mixed randomly with an equal number of new, distractor words (Test series). Both reaction time and the P300 component of the event-related brain potential were used as measures of processing time. During the Study series, large P300s were elicited despite a word category probability of 1.0. When the words from the Study series were divided on the basis of recognition performance, words that were subsequently recognized elicited P300s with shorter latencies than unrecognized words. P300 amplitude to words in the Study series increased with repetition while maintaining a constant latency. During the Test series, P300 latency and reaction time decreased with repetition for both target and distractor words. P300 amplitude to all words increased substantially over Test repetitions with target words eliciting larger P300s than distractor words. Words that were recognized more consistently during the Test series elicited larger and earlier P300s than words that were recognized less consistently. The P300 amplitude and latency results from both the Study and Test series are interpreted as reflecting the increased discriminability of the target words as the memory trace increases in strength.  相似文献

Somatosensory evoked potential (SEP) changes associated with selective attention were investigated. In 16 subjects, SEPs were recorded from five locations while they counted electrical stimuli to one of four randomly stimulated fingers. Sequential SEP events measured included peaks P30 (positivity at 30 msec). P45, N60, P100. N140. P190. N230, P400. Counting was associated with greater P45, P100. P190, N230, and P400 amplitudes; effects were not attributable to eye or tongue activity. Analyses designed to reveal changes associated with two conceptualized “channels” (finger class, hand) showed that the P45, P100, and P190 amplitude increases involved both channels. The P400 effect was limited to the target finger. Channel effects for N60 and N140 amplitudes resulted from decreases localized to the unattended element of one channel, suggesting “inhibition.” Latency effects involved mainly the hand channel; counted hand latencies were shorter for P30, P45, P100 and P190. The findings indicate modifications of both early and late electrocortical events with selective attention, and that changes can be of several kinds. They support the view that attention proceeds in more than one stage.  相似文献

In this study the organization of information processing in a selective search task was examined by analyzing event-related potentials. This task consisted of searching for target letters in a relevant (attended) color. The ERPs revealed two different effects of attention: an early occipital negativity (+/- 150 ms) reflecting feature-specific attention, and a later, central N2b component (+/- 240 ms) reflecting covert orienting of attention. A later, prolonged negativity (search-related negativity) (+/- 300 ms), maximal at Cz, was related to controlled search to letters in the attended color. Detection of relevant targets resulted in a parietal P3b component. Depending on stimulus presentation conditions an earlier response to both attended and unattended targets was found: an N2 component (+/- 250 ms). In these same conditions, C'3-C'4 asymmetries (Corrected Motor Asymmetries--CMA) suggested motor activation at +/- 300 ms, in the same time range as search-related negativity. It was argued that N2 and CMA suggest the existence of a preattentive target detection system, operating in parallel with a slower serial attentive system, as reflected by N2b and search negativity.  相似文献
