The relationships between the morphology and receptive fields of local and intersegmental interneurons that process mechanosensory information from a hindleg of the locust have been analysed. Sensory neurons from tactile hairs project to ventral areas of neuropil in the metathoracic ganglion where they form a 3-dimensional somatotopic map of a hindleg. By contrast, sensory neurons from a proprioceptor at the femoro-tibial joint (the femoral chordotonal organ) project to lateral and more intermediate areas of neuropil and have no branches in the most ventral regions of neuropil. Particular local and intersegmental interneurons respond to stimulation of specific arrays of hairs on a hindleg, or to movements of particular joints. Their receptive fields are defined, in part, by the patterns of excitatory, monosynaptic connections made by these afferents. Each interneuron has a characteristic receptive field and a characteristic morphology defined by its array of branches in the regions of neuropil containing the projections of the afferents that provide its monosynaptic inputs. Interneurons with inputs exclusively from tactile hairs have branches in the most ventral regions of neuropil, while those with exclusively proprioceptive inputs have branches only in more intermediate levels of neuropil. Interneurons with extensive receptive fields from tactile hairs also have extensive areas of branching within the ventral neuropil. Interneurons with receptive fields restricted to particular regions of the leg have branches restricted to the ventral region of neuropil containing the projections of afferents from that part of the leg. Thus, interneurons with inputs only from hairs on the tarsus have branches in the posterior region of neuropil corresponding to the projections of the tarsal afferents, while interneurons with receptive fields on the femur have branches in more anterior regions of neuropil corresponding to the projections of the femoral hair afferents. Peribulbar anaesthesia using a mixture of local anaesthetic and vecuronium   总被引:1,自引:0,他引:1  
The aim of this double-blind, randomised study was to assess the effects of the addition of 0.5 mg of vecuronium bromide to a standard local anaesthetic mixture used for peribulbar anaesthesia. We studied 60 patients undergoing regional anaesthesia for intra-ocular surgery and were primarily interested in the quality of globe and lid akinesia. All received a mixture of 5 ml 2% lignocaine with 1:200 000 adrenaline, 5 ml 0.75% bupivacaine and 150 IU hyaluronidase with either 0.9% saline 0.25 ml (group A, n  = 30) or vecuronium bromide 0.25 ml (0.5 mg) (group B, n  = 30). Eye movements assessed at both 5 and 10 min were significantly reduced in the vecuronium group (group B) (p < 0.05). We conclude that the addition of vecuronium at a dose of 0.5 mg to the standard local anaesthetic mixture improves the quality of globe and lid akinesia.  相似文献

Summary. The aim of this study was to investigate the possible relationship between variations in local skin and body temperatures, and a subcutaneous hyperaemia response during sleep in humans. Nocturnal subcutaneous blood flow (SBF) was measured in the lower legs of 9 subjects for 8–12 h under outpatient conditions. The core temperature was measured by a tympanic membrane temperature sensor, and the local skin temperature was measured by a skin temperature sensor. The 133Xe wash-out technique with portable CdTe(Cl) detectors was used for measurement of the SBF. The SBF increased significantly (P<0.0001) after 1 h sleep, with the hyperaemia persisting for 2 h. During the hyperaemic phase, the local skin temperature increased significantly (P<0.0001) and then decreased again in the post-hyperaemic phase (P<0.01). The core temperature decreased significantly during the measurement period (P<0.01). Separate ordinary linear regression analyses revealed no significant correlations between the measured temperature parameters and the blood flow levels during sleep. The results indicate no governing role of the local skin or body temperatures on the local SBF. The local skin temperature increased significantly secondary to the nocturnal subcutaneous hyperaemia (i.e. active vasodilatation), which is believed to be involved in a thermoregulatory effector mechanism.  相似文献

The role of innate immunity in allograft injury is just beginning to become clear, and complement is probably one of a number of factors that are activated very early in the course of transplantation. Kidney transplantation into complement-inhibited rats reduces subsequent inflammation of the graft, probably as a result of reduction of ischemia reperfusion damage as well as diminution of immune mediated damage. Closer analysis of the role of locally synthesised components in mice has suggested that regional synthesis of complement proteins, in particular by the renal tubule, may play a more important role than circulating components. A marked effect on the antidonor T cell response may be explained by the triggering of complement receptors present on antigen presenting cells or T cells infiltrating the graft, or by a more direct effect of complement on the liaison between proximal tubule cells and T cells. Therapeutic control is likely to require a shift to a more targeted approach, directed at complement components produced in the extravascular tissue compartment.  相似文献

We describe the appearance of complete atrioventricular block (CAVB) with sevoflurane and nitrous oxide during the slow induction in a ten-year-old male patient with hypertension, renal dysfunction, and impaired cardiac conduction. Sinus rhythm was restored following the washout of the anaesthetic gas. And CAVB recurred after the subcutaneous injection of lignocaine. The present report shows that sevoflurane should be treated with care like other inhalational anaesthetics as regards the effect on cardiac conduction.  相似文献
