Abstract

Background The Signposts for Building Better Behaviour program, developed by the Parenting Research Centre, Victoria, Australia, was conducted at a public hospital facility in Singapore.

Method More than 1,000 parents completed the program, and filled in questionnaires about their child's behaviours.

Results Parents rated themselves in the questionnaires as being significantly less hassled, stressed, depressed, and anxious after attending the program. They were more confident and satisfied with managing their child, and rated their children's behaviours as having improved. Effect sizes ranged from 0.12 to 0.59. The findings were maintained 3 months after completion of the program.

Conclusions The study provides evidence of the cross-cultural applicability of the principles underlying the Signposts program. As there are long-term repercussions when children's behaviour problems are not dealt with appropriately, such behaviour management programs should be made more available to parents and caregivers.  相似文献

Background

There is a need to determine the difference in response to mail, e-mail, and phone in clinical research surveys.

Methods

We enrolled 150 new and follow-up patients presenting to our hand and upper extremity department. Patients were assigned to complete a survey by mail, e-mail, or phone 3 months after enrollment, altering the follow-up method every 5 patients, until we had 3 groups of 50 patients. At initial enrollment and at 3 month follow-up (range 2–5 months), patients completed the short version of the Disabilities of the Arm, Shoulder, and Hand questionnaire (QuickDASH), the short version of the Patient Health Questionnaire (PHQ-2), the Pain Self-Efficacy Questionnaire (PSEQ), and rated their pain intensity.

Results

The percent of patients that completed the survey was 34 % for mail, 24 % for e-mail, and 80 % for phone. Factors associated with responding to the survey were older age, nonsmoking, and lower pain intensity. Working full-time was associated with not responding.

Conclusions

The response rate to survey by phone is significantly higher than by mail or e-mail. Younger age, smoking, higher pain intensity, and working full-time are associated with not responding.Type of study/level of evidence: Prognostic I  相似文献

Background

Evaluation of medical students during the surgical clerkship is controversial. Performance is often based on subjective scoring, whereas objective knowledge is based on written examinations. Whether these measures correspond or are relevant to assess student performance is unknown. We hypothesized that student evaluations correlate with performance on the National Board Of Medical Examiners (NBME) examination.

Methods

Data were collected from the 2011–2012 academic year. Medical students underwent a ward evaluation using a seven-point Likert scale assessing six educational competencies. Students also undertook the NBME examination, where performance was recorded as a percentile score adjusted to national standards.

Results

A total of 129 medical students were studied. Scores on the NBME ranged from the 52nd to the 96th percentile with an average in the 75th percentile (±9). Clerkship scores ranged from 3.2–7.0 with a mean of 5.7 (±0.8). There was a strong positive association between higher NBME scores and higher clerkship evaluations shown by a Pearson correlation coefficient of 0.47 (P < 0.001). Students clustered with below average ward evaluations (3.0–4.0) were in the 69.5th percentile of NBME scores, whereas students clustered with above average ward evaluations (6.0–7.0) were in the 79.2th percentile (P < 0.001).

Conclusions

A strong positive relationship exists between subjective ward evaluations and NBME performance. These data may afford some confidence to surgical faculty and surgical resident ability to accurately evaluate medical students during clinical clerkships. Understanding factors in student performance may help in improving the surgical clerkship experience.  相似文献

目的评价国产胶体金法检测全血铁蛋白的分析性能,探讨其在儿童缺铁性贫血中的临床应用价值。方法通过与免疫化学发光方法比较,根据EP文件从精密度、线性范围及抗干扰能力、可比性等方面评价胶体金法测定全血铁蛋白的分析性能;随机抽取重庆医科大学附属儿童医院50例缺铁性贫血患儿及重庆市妇幼保健院健康体检儿童50例,分别测定两组全血铁蛋白,分析其诊断效率。结果胶体金法测定全血铁蛋白批间精密度分别为7.2%和9.6%,线性为0~100ng/mL,溶血、黄疸、脂血无干扰,与雅培i2000SR血清铁蛋白检测结果比对差异无统计学意义(P0.05);与对照组相比,缺铁性贫血组全血铁蛋白明显降低。以20ng/mL作为诊断临界点,其灵敏度为94%,特异度为100%,诊断效率为97%。结论胶体金法分析性能良好,作为即时检测项目能满足临床,具有较高诊断效率,对儿童是否缺铁可作提示性诊断。  相似文献

Background

The Duke Activity Status Index (DASI) assesses the functional capacity of patients with cardiovascular disease (CVD), but there is no Portuguese version validated for CVD.

Objectives

To translate and adapt cross-culturally the DASI for the Portuguese-Brazil language, and to verify its psychometric properties in the assessment of functional capacity of patients with CVD.

Methods

The DASI was translated into Portuguese, then checked by back-translation into English and evaluated by an expert committee. The pre-test version was first evaluated in 30 subjects. The psychometric properties and correlation with exercise testing was performed in a second group of 67 subjects. An exploratory factor analyses was performed in all 97 subjects to verify the construct validity of the DASI.

Results

The intraclass correlation coefficient for test-retest reliability was 0.87 and for the inter-rater reliability was 0.84. Cronbach''s α for internal consistency was 0.93. The concurrent validity was verified by significant positive correlations of DASI scores with the VO2max (r = 0.51, p < 0.001). The factor analysis yielded two factors, which explained 54% of the total variance, with factor 1 accounting for 40% of the variance. Application of the DASI required between one and three and a half minutes per patient.

Conclusions

The Brazilian version of the DASI appears to be a valid, reliable, fast and easy to administer tool to assess functional capacity among patients with CVD.  相似文献

In this article, we explore the form of evaluation put forward by guidelines used in the health sciences for appraising qualitative research and we begin to articulate an alternative posture. Most guidelines are derivative of the modes of assessment developed by clinical epidemiologists as part of the promotion of evidence-based medicine (EBM). They are predominantly proceduralist in orientation, equating quality with the proper execution of research techniques. We argue that this form of judgment assumes a fixed relationship between research practice and knowledge generated, and tends to over-simplify and standardize the complex and non-formulaic nature of qualitative inquiry. A concern with methods as objects of judgment in and of themselves restricts the reader's field of vision to the research process and diverts attention away from the analytic content of the research. We propose an alternative 'substantive' perspective that focuses on the analysis put forward, and regards methods as resources for engaging with and understanding the substantive findings and topic of inquiry. An important challenge is to find a way to embody such a form of judgment in practical assessment tools.  相似文献
