外源性压迫致腓总神经损伤的治疗

目的探讨外源性压迫因素所致腓总神经损伤的治疗方法。方法回顾分析了我院1995年8月～2005年8月收治的单纯外源性压迫所致腓总神经损伤病人32例的临床资料,其外源性压迫因素有石膏边缘卡压、小夹板及压垫压迫、牵引架布托卡压和下肢被动长时间外旋压伤腓骨颈处等。27例行保守治疗,方法包括电针灸配合应用药物熏洗、理疗、按摩、药物等;手术治疗5例,方法包括切开腓管减压、彻底松解神经周围粘连较重的组织,甚至将神经外膜松解。结果随访1～18个月,平均10个月,疗效优24例,良4例,可2例,差2例。2例差者均为病程超过6个月者,最终1例行肌腱移位,1例行踝关节融合术。结论外源性压迫所致腓总神经损伤的治疗越早效果越好,早期应以保守治疗为主,保守治疗超过3个月神经功能恢复停止者应行神经松解术治疗。     

辣椒碱表面活性剂泡囊处方设计

目的 研究辣椒碱表面活性剂泡囊的处方设计.方法 采用注入法制备辣椒碱表面活性剂泡囊,以包封率为评价指标,评价不同组成材料制备的辣椒碱表面活性剂泡囊.结果 得到9种辣椒碱表面活性剂泡囊处方,其平均包封率均≥75%,粒径分布为5～30 μm,平均粒径均≤10 μm.结论 本文筛选得到9种包封率较高、粒径大小理想的辣椒碱表面活性剂泡囊处方,为进一步的研究奠定了基础.     

Metastasis size in pathologic femoral fractures

Radiographic assessment was made of metastases in 69 consecutive patients with pathologic femoral fractures. The following variables were considered: width of metastasis, ratio between width of metastasis and bone, axial length of cortical destruction, and proportion of cortical bone destroyed of the circumference. Differences in size of the metastases were not related to location in the femur or histologic type. In 62 of 69 fractures the ratio between width of the metastasis and bone was ≥ 0.60, axial cortical destruction in the neck was ≥ 13 mm and in other parts of the femur ≥ 30 mm, and cortical destruction of the circumference ≥ 50 percent. Prophylactic internal fixation need not be considered for femur metastatic lesions that do not reach these limits.     

Recurrent ulnar nerve entrapment at the elbow: Correlation of surgical findings and 3-Tesla magnetic resonance neurography

The authors describe the correlation between 3-Tesla magnetic resonance neurography (MRN) and surgical findings in two patients who underwent multiple previous failed ulnar nerve surgeries. MRN correctly localized the site of the abnormality. Prospectively observed MRN findings of perineural fibrosis, ulnar nerve re-entrapment abnormalities, medial antebrachial cutaneous neuroma and additional median nerve entrapment were confirmed surgically.     

Treatment of meralgia paraesthetica by laparoscopic pre-peritoneal approach

Summary. Meralgic paraesthetica can be successfully treated laparoscopically.     

Vascular entrapment of a multipolar basket catheter (OrionTM) during catheter ablation

The IntellaMap Orion^TM (Boston Scientific) is a 64-electrode basket catheter allowing for ultrahigh-density mapping of complex cardiac arrhythmias. We report the case of a basket catheter vascular entrapment, requiring surgical removal.
