We examined the number of days spent in hospital due to a relapse of schizophrenia and the associated costs for patients treated
with olanzapine or haloperidol. Twenty-one German psychiatric hospitals participated in this retrospective study. Data on
the last hospitalisation following a relapse of schizophrenia were documented for equal numbers of patients on olanzapine
and haloperidol. Matching for time since diagnosis and severity of symptoms was performed. Data were collected on 136 matched
pairs. Total length of time spent in hospital was the same on average for patients in both groups (median about 5 weeks),
but olanzapine patients spent nearly 1 week less in the in-patient setting than haloperidol patients, resulting in a saving
of Euro 411 per patient. Our findings are consistent with those of randomised clinical trials in concluding that olanzapine
is preferable to haloperidol in terms of the direct cost of treating schizophrenia.

The weighted rank pairwise correlation (WRPC) statistic has been proposed as a robust test of genetic linkage, particularly adapted to the analysis of large and complex pedigrees and for age-dependent and heterogeneous diseases. In this paper a simulation study is presented. Validity and power of the WRPC test are studied and compared to the Haseman-Elston sibpair method for various types of problems. The power of the WRPC test is slightly lower than the Haseman-Elston method for analyzing a large number of small randomly chosen pedigrees. It is higher however in presence of genetic heterogeneity or for analyzing large individual pedigrees. Recently, evidence of linkage of Alzheimer's disease with a locus on chromosome 14, D14s43, has been obtained by the Lod-score method. We reanalyze these data using the WRPC test, essentially confirming the results of the Lod-score method. The WRPC test statistic is higher than the equivalent Lod-score statistic for the two pedigrees which show strong evidence of linkage with the two methods. The global WRPC test statistic is slightly lower than the Lod-score test statistic. The WRPC test, however, makes no hypothesis of a specific genetic transmission model and can be computed very quickly; in addition, an exact P-value can be computed by simulation for individual pedigrees. © 1994 Wiley-Liss, Inc. 相似文献

Platelet aggregation and thrombosis play an important role in the onset of acute coronary events. Regardless of the stimulus for activation, platelet thrombus formation is ultimately regulated through the IIb/IIIa receptor complex. The effects of oral administration of xemilofiban, a non-peptide mimetic of the RGDF sequence of the IIb/IIIa receptor complex, on thrombus formation were evaluated in a canine model. Xemilofiban significantly reduced platelet deposition on severely damaged arterial wall. Platelet deposition was reduced at both low (13 ± 1 from 56 ± 18 × 106 platelets cm−2 ; P < 0.05) and high (23 ± 2 from 111 ± 21 × 106 platelets cm−2 ; P < 0.01) shear rates. Platelet deposition was reduced to a monolayer as seen by electron microscopy (platelet–vessel wall interaction). Therefore, the availability of an orally active IIb/IIIa antagonist for chronic use may have significant value in preventing thrombus formation in those clinical situations associated with severe arterial injury, such as atherosclerotic plaque disruption. 相似文献

The HACCP (hazard analysis critical control point) concept is now widely applied in the food industry and provides a structured and critical approach to the identification and control of hazards that may affect food safety. It has shifted the emphasis for control from retrospective end-product testing to the effective control of raw materials and key processing operations.
This paper discusses how the HACCP approach can be applied to enteral feeding. This involves each unit assembling a multidisciplinary team of personnel involved in enteral feeding. This team will then carry out a detailed analysis of the process from selection of ingredients and feeding systems through to consumption of the feed by the patient by constructing a flow chart that relates specifically to each unit. They can then identify and assess the hazards associated with the handling of the product at each stage in the process. This will enable them to identify the points where control over an identified hazard can be achieved (critical control points, CCP) such as quality of ingredients, design of administration systems, preparation and distribution of the feeds and the procedures involved in the assembly and manipulation of the systems. Control and monitoring procedures can then be specified and implemented at relevant stages in the process. The major strengths of the HACCP procedure are that it entails a team effort from key personnel involved in the full range of activities associated with the product and each detailed analysis is specific to each unit's practices and resources and can be continually reviewed and modified in response to changing circumstances. 相似文献

The neuropathology of schizophrenia remains obscure despite the fact that many neuropathologists have investigated this area for over 100 years. While remarkable progress has been made in the neuropathological study of neurodegenerative diseases including Alzheimer's disease, progress in studying the neuropathological entity of schizophrenia has not kept pace; the phrase “schizophrenia is the graveyard of neuropathologists” has been stated in the field. Since the 1980s, the morphological or functional abnormalities in the brains of schizophrenia patients have been reported by means of CT or MRI and with advanced functional brain image technology such as positron emission tomography or single photon emission computed tomography. Results from such imaging studies have led to neuropathological examination of the post mortem brains of schizophrenia patients being undertaken again. These neuroimaging studies have influenced the neuropathological investigation of the schizophrenic brain. Not only the classical microscopic observation of neuropathology, but also measurement and statistical analysis using computer imaging software or using immunohistological techniques has been performed. Based on the neuropathological studies of schizophrenia over the last 20 years, it is clear that schizophrenia is not a pure functional disease without organic factors. Reports of neuropathological abnormalities in the post mortem schizophrenic brain indicated they were found in almost all areas of the brain, but there are more reports describing the temporal lobe and frontal lobe compared to those describing other areas of the brain. These observed neuropathological abnormalities are explained rationally by the hypothesis of a neurodevelopmental disorder in this disease. In recent molecular biology studies, several putative candidate genes were reported, and some of these genes might have the function of neurodevelopment or making neuronal networks. It is important to consider together these findings with morphometric studies in neuropathological observation, neuroimaging studies and genome studies to pursue the etiology of schizophrenia from various perspectives. 相似文献

Analysis of in vivo short TE 1H spectra is complicated by broad baseline signal contributions and resonance line-shape distortions. Although the assumptions of ideal metabolite resonance line-shapes and slowly varying baseline signals can be used to separate these signals, the presence of broad or asymmetric line-shapes can invalidate this model. More complex line-shape models are computationally expensive or difficult to constrain, particularly for the low signal-to-noise commonly found for in vivo MR spectroscopic imaging applications. In this study, two time-domain models for fitting variable spectral line-shapes are examined, one using B-splines and another using summed sinusoids. The methods were verified using both phantom and human data, and Monte Carlo simulations were used to evaluate variations in calculated metabolite amplitudes due to interactions between the baseline and line-shape estimations. Additional studies investigated the use of prior line-shape information, obtained from either a water MRSI measurement or calculations from B(0) maps, to determine parameter starting values or optimization constraints. Both line-shape models showed the ability to fit the variety of line-shapes present in both the phantom and human MRSI data, with similar or improved accuracy over a Gaussian line-shape model; however, this improvement resulted in only minor improvement for the high-SNR phantom data and moderate improvements in regions with asymmetry for the fitted in vivo metabolite images. The use of prior line-shape information was of most benefit when applied toward setting optimization constraints but was of limited benefit when used to define initial starting values. 相似文献

目的了解正常大鼠角膜内皮细胞在体外经γ-干扰素诱导后,主要组织相容性复合体(major histoeompatibility complex,MHC)-Ⅰ、Ⅱ类抗原异常表达的情况。并观察比较白细胞介素-2-绿脓杆菌外毒素(interleukin-2-pseudomonas exotoxin40。IL-2-PE40)、环胞霉素A(cyclosporine A,CsA)对角膜内皮细胞MHC-Ⅰ、Ⅱ类抗原异常表达的免疫抑制作用。方法采用ACAS-570黏附式细胞分析仪和免疫荧光技术,对体外原代培养经γ-干扰素诱导后的正常大鼠角膜内皮细胞分成A、B2组进行MHC-Ⅰ、Ⅱ类抗原表达的相对量测定,并在培养液中加入新型免疫抑制剂IL-2-PE40和CsA,进一步测定角膜内皮细胞MHC-Ⅰ、Ⅱ类抗原的表达量。结果未加入γ-干扰素前,MHC-Ⅰ类抗原的表达量为97.8±8.1,MHC-Ⅱ类抗原无表达;经γ-干扰素诱导后,MHC—Ⅰ类抗原的表达量为1006.3±13.2,MHC-Ⅱ类抗原表达量为406.5±10.5,γ-干扰素加入前后MHC-Ⅰ、Ⅱ类抗原比较均有统计学意义(P〈0.05)。IL-2.PE40组MHC-Ⅰ类抗原的表达量为618.2±13.5,MHC-Ⅱ类抗原表达量为204.5±7.8,CsA组MHC-Ⅰ类抗原的表达量为609.5±12.9,MHC-Ⅱ类抗原表达量为198.5±6.9,IL-2.PE40组、CsA组分别与注射用水比较,MHC—Ⅰ、Ⅱ类抗原间均有统计学意义(P〈0.05)。IL-2.PE40组与CsA组比较.MHC—Ⅰ、Ⅱ类抗原间差异无统计学意义(P〉0.05)。结论在体外,γ-干扰素可诱导角膜内皮细胞MHC-Ⅰ、Ⅱ类抗原异常表达;IL-2-PE40及CsA均能不同程度的抑制这种表达。[眼科新进展20ff7;27(3):170.172] 相似文献

Standardised epidemiological caries assessments used in oral health surveys have been shown to be poor at predicting whether a tooth surface will be treated restoratively when a patient visits a dentist. However, it has been argued that oral health surveys may be more relevant in determining needs at the level of an individual or groups of individuals. The objective of this study was to determine the discriminatory power of visual caries assessments at two thresholds (D1 & D3) in adolescents of average age 12.1 years to predict experience of dentinal caries 3 years later or the experience of restorative treatment (not re‐treatment) during the 3‐year period. The data was derived from a prospective 3‐year longitudinal study in which the dental care provided by 41 dentists for 403 adolescents was monitored. Dental caries experience was monitored by annual standardised assessments of caries undertaken by a single trained examiner. ROC analysis showed that caries assessed visually at the D1 threshold in 12‐year‐olds was a better predictor (P < 0.001) of experiencing some dentinal caries after 3 years (Az = 0.781) than was caries assessed visually at D3 threshold in 12‐year‐olds (Az = 0.670). Assessing caries visually at either the D1 or the D3 threshold had no discriminatory power for predicting whether an individual would experience some restorative treatment during the ensuing 3‐year period (Az for D1 = 0.507; Az for D3 = 0.518). 相似文献
