柔性化管理在护理质量督导中的应用

目的探讨柔性化管理在护理质量督导中的应用效果。方法更新护理质量督导理念,在护理质量标准的制订、一级质量控制人员的培训、护理质量检查结果反馈方式等方面,采用柔性化管理的护理督导模式,对全院护理质量进行柔性化督导管理。结果护理质量各项评价指标(除核心制度提问和特级、一级护理)和护理综合满意度均提高(P<0.01)。结论柔性化管理适应现代护理质量督导的特点,满足了新形势下患者需求的变化和护理服务内涵的更新,有利于护理质量的提高。

Brokering Community Engagement: Proactive Strategies for Supporting Indigenous Australians with Mental Health Problems

This qualitative study explored the experiences of mental health employees working with Indigenous clients living with mental illness. Interviews were conducted with 20 mental health workers to identify strategies they adopt to facilitate community engagement with Indigenous clients. Using a thematic analysis approach, ‘Brokering community engagement’ was the umbrella theme from which two subthemes related to community engagement for the service and clients emerged (1) enabling connections –community and family; and (2) recovery and reconnecting with community. Participant insights enabled a deeper understanding of the role of community in the recovery process for Indigenous clients and highlight the importance of community engagement as a primary, yet multifaceted strategy used by mental health workers in the communities they serve.     

The Role of Occupational Therapists in Home Modification Programs at an Area Agency on Aging

The Housing Department of the Philadelphia Corporation for Aging (PCA) has administered home repair and modification programs for seniors since 1980. PCA now provides services to 1500 people annually through six programs. Currently funded at approximately four million dollars per year, PCA's Housing Department is the largest in any Area Agency on Aging.

In all these programs, PCA contracts with Occupational Therapist Consultants to assist the consumers and the programs to develop the most useful and appropriate modifications. In one program, serving Philadel-phians of all ages, PCA has recently initiated a follow-up inspection by the therapist. These inspections are not only of benefit to the particular consumer, but also yield useful information on construction specifications and on the modification process.     

Safeguarding the Nonprofit Agency: The Role of the Board of Directors in Risk Management

SUMMARY

The board of directors of human service agencies has a pivotal role in overseeing the policies, procedures and operations of the service organization. This article explores the role of boards of directors in safeguarding nonprofit organizations through the management of risk and application of overall good judgment. Safeguarding refers to the oversight and accountability function of boards in relation to how business is conducted. Included are considerations pertaining to protection of consumer interests, assurances of quality services, positive and documented outcomes, and full compliance with laws, regulations, and professional standards pertaining to how nonprofit organizations are expected to operate on behalf of those served. The role of nonprofit boards in risk management is reviewed, with particular reference to the implementation of this role in nonprofit residential treatment settings. Strategies to enhance board functioning in the risk management arena are detailed and recommendations are offered about overall organizational preparation and practices for managing risk.     

The role of informal caregivers during cancer patients’ recovery from chemotherapy

Scand J Caring Sci; 2013; 27; 147–155 The role of informal caregivers during cancer patients’ recovery from chemotherapy Aims: (i) To investigate the availability, perceived importance and roles of informal caregivers in the recovery of patients treated at day oncology centres and (ii) identify differences between patients with and without informal caregivers in the extent to which they experienced symptoms, and the level of bother symptoms caused. Method: Patients from three Australian hospitals (n = 122) were recruited during cycles 1 or 2 of adjuvant chemotherapy. Participants completed a modified version of the Rotterdam Symptom Checklist (RSCL) each day for 5 days after chemotherapy. A telephone interview conducted 10 days post‐treatment explored the availability, importance and roles of caregivers. During the interview, participants also completed the modified RSCL in which they were asked about the extent to which they experienced, and were bothered by, each symptom over the prior 5 days combined. Results: Overall, 71% of participants had an informal caregiver in the 5 days postchemotherapy, commonly a partner. More women (71%) than men (48%) had a caregiver (p < 0.05). Caregivers were perceived to be highly important; they were more important for women than men (U = 213.50, p < 0.01). The most common assistance caregivers provided was meal preparation and emotional support and companionship. On days 6–10 postchemotherapy, the extent to which overall physical symptoms were experienced was higher for patients with a caregiver than without (U = 987.50, p < 0.05); similarly their symptoms caused more bother (U = 966.00, p < 0.01). Conclusions: These findings highlighted the importance of informal caregivers to patients postchemotherapy. It is imperative that patients are informed of the importance of this support so a caregiver can be arranged, if possible. Understanding the needs of patients following chemotherapy would enable health professionals to advise patients, with or without caregivers, how to best prepare for and manage their recovery at home.
