预防接种与自身免疫病的关系

感染与自身免疫性疾病之间的联系已引出一场争论 :疫苗是否也能激发自身免疫性疾病。有关这种风险有众多正反两方面的主张。在此复习感染诱导自身免疫病的相关机制 ,并对预防接种和自身免疫病之间的相对危险度进行评估。     

原发性肾紫癜诊治体会（附5例报告）

目的：探讨原发性肾紫癜的诊断和治疗。方法：总结1994～2001年收治经手术和病理证实的肾紫癜5例的临床资料，其中误诊肾盂肿瘤2例，肾结核1例，血尿查因疑肾紫癜2例。结果：5例均行肾脏切除，其中4例因短时内大量出血超过500ml而急诊行肾切除，包括疑肾紫癜还未用上激素治疗的2例。1例反复出血不能排除肿瘤而行肾切。结论：本病的术前诊断比较困难，应结合临床具体分析，尽可能早期用激素治疗以保存肾脏，只有在血尿严重，威胁生命或不排除恶性肿瘤时才考虑行肾切除。     

GraVes眼病炎性活动期与静止期血清sICAM-1水平的研究

目的:研究可溶性细胞间粘附因子-1(sICAM-1)在Graves眼病(GO)炎性活动期与静止期的水平差异及其与各项甲状腺功能指标的关系。方法:将GO炎性活动期与静止期患者,Graves病(GD)无眼病患者及正常对照,分为A、B、C、D四组,抽取空腹静脉全血,测血清sICAM-1,各组间进行比较。并分析各组sICAM-1与FT3、FT4、TG、TM的相关关系。结果:①A、B、C组患者血清sICAM-1均明显高于正常对照组(P<0.05)。A组患者sICAM-1水平最高,与B组及C组患者相比有统计学差异(P<0.01);②前三组各组内sICAM-1与FT3、FT4成直线相关关系,与TG、TM成直线相关关系;③GO炎性活动期患者sICAM-1与GO评分成正相关。结论:血清sICAM-1水平可作为反映GO活动性评判的免疫指标,长期监测sICAM-1可预示GD进展为临床活动性眼病的可能性。     

Measurement of Perceived Disruption During Rebuilding Following Hurricane Andrew

The purpose of this study was to develop and evaluate a measure of perceived disruption during rebuilding following a disaster. Two eight-item scales, which measured intensity of disruption during the entire repair phase (Intensity-RP) and intensity of disruption during the past month (Intensity-PM) were developed and administered to 135 survivors of Hurricane Andrew. At 9 to 12 months postdisaster, Intensity-RP and Intensity-PM were both significantly associated with scores on the Global Severity Index of the SCL-90-R, and with scores on the Impact of Event-Intrusion Scale; Intensity-PM alone was significantly associated with PTSD scores. Regression analyses indicated that each scale contributed significant unique variance in predicting mental health symptoms, even after controlling for relevant demographic and initial disaster exposure variables.     

Toll-like receptors in Borrelia burgdorferi-induced inflammation

Lyme arthritis, the most common manifestation of late Lyme disease, has been associated with the presence of Borellia burgdorferi in the joint. However, it is still unclear whether the pathogen itself is able to elicit such a sustained inflammatory response, or whether an aberrant immunological reaction of the host is the main driving force. Borrelia antigens, including lipoproteins, flagellin and DNA, are ligands of Toll-like receptors, and can thus elicit a strong stimulation of host cells, such as neutrophils, mononuclear cells and resident tissue cells. Understanding the molecular basis of the signalling events caused by Borrelia lipoproteins will lead to a greater understanding of inflammation in Lyme arthritis and, hopefully, new treatment strategies for chronic antibiotic-resistant disease.     

Testosterone not associated with violent dreams or REM sleep behavior disorder in men with Parkinson's.

We examined the relationship between testosterone levels, violent dreams, and REM sleep behavior disorder (RBD) in 31 men with Parkinson's disease (PD): 12 with clinical RBD and 19 without. All PD patients with clinical RBD experienced violent dreams, but none of the 19 non-RBD patients reported violent dreams. While dream content appears to be more aggressive in PD patients with clinical RBD, the presence of violent dreams or clinical RBD is not associated with testosterone levels in men with PD.     

大鼠坐骨神经慢性挤压性损伤后痛阈和血清IL-6的变化及相关性研究

目的:通过大鼠坐骨神经慢性挤压伤(CCI)神经性疼痛模型的热敏变化及血清中IL-6含量的变化,探讨血清IL-6在神经性疼痛形成中的作用及可能机制.方法:36只250～300g的健康雄性Wistar大鼠,在戊巴比妥钠麻醉下于大腿中部暴露坐骨神经并作结扎,取对侧大腿坐骨神经暴露作为模拟对照(B组).术后1,3,5,7,9,11,13,15 d测定大鼠(n=12)两侧后爪对热敏阈值的变化.于术后第15 d处死大鼠,取血清,ELISA法测定IL-6浓度.结果:大鼠双侧后爪(CCI和B)的收缩潜伏期在术后第3,5,7,9,11,13,15 d有显著差异;CCI组血清IL-6与对照组比较有显著差异.结论:IL-6与大鼠坐骨神经慢性挤压性损伤后出现的神经源性疼痛过敏有关.     

The prevalence of LRRK2 Gly2385Arg variant in Chinese Han population with Parkinson's disease.

We conducted a case-control study to determine the prevalence of the LRRK2 Gly2385Arg variant in patients with Parkinson's disease in Han population in mainland China. Heterozygous LRRK2 Gly2385Arg variant was identified in 14 of 235 patients with Parkinson's disease (5.69%), but not in 214 unrelated healthy controls. Multivariate analysis indicated the frequency of Gly2385Arg variant in the female patients with early age at onset is higher than their male counterparts. The founder haplotype analysis showed the variant carriers shared the same founder. Clinically, the LRRK2 Gly2385Arg carriers presented with classical Parkinson's disease symptoms. Our study indicates that the LRRK2 Gly2385Arg variant is a potential ethnic-specific genetic risk factor of Parkinson's disease within Chinese Han ethnicity.     

双相气道内正压呼吸机在老年慢性阻塞性肺疾病康复期和急性肺水肿患者中的临床应用

应用双相气道内正压呼吸机（ＢｉＰＡＰ）对９例慢性阻塞性肺疾病（ＣＯＰＤ）康复期和呼吸衰竭稳定期进行临床观察发现，治疗后动脉血氧分压与治疗前相比显著升高（Ｐ＜０．０１）；肺活量及用力肺活量轻度升高；１例３次发生急性左心衰、急性肺水肿致急性呼衰患者治疗后，病情很快改善；１例阻塞性夜间睡眠呼吸暂停综合征者应用效果满意。该机应用简便、安全，对心率血压无明显影响，为ＣＯＰＤ康复期、急性左心衰兼肺水肿的治疗提供了新途径。
