Expanding Our Strategies to Study the Process of Change

The conceptual and methodological framework proposed by Doss (this issue) makes valuable suggestions for strategic choices in future research. This commentary addresses conceptual and terminological distinctions adopted by Doss, as well as his criticism of add-on/ dismantling studies. We also suggest research topics and methodological developments that could be integrated in Doss's framework to further expand understanding of therapeutic change.

钾离子通道的活性状态对蛛网膜下腔注射腺苷抗伤害性作用的影响

目的探讨ATP敏感型钾离子通道的活性状态对鞘内注射腺苷类似物R-phenylisopropyladenosine(R-PIA)抗伤害性作用的影响.方法雄性SD大鼠在苯巴比妥钠麻醉下,蛛网膜下腔留置PE-10导管,测定注药后鼠尾对光热刺激的反应(抗伤害作用).结果蛛网膜下腔注射R-PIA产生剂量依赖性的甩尾时间曲线上移(P＜0.05).蛛网膜下腔注药后10min起效,镇痛时间长达60min.ATP敏感型通道开放剂nicorandil和阻滞剂glibenclamide单独蛛网膜下腔应用无镇痛作用(P＞0.05),但nicorandil明显增强R-PIA的抗伤害性作用,相反glibenclamide明显减弱R-PIA的抗伤害性作用(P＜0.05).结论蛛网膜下腔注射R-PIA可产生明显的剂量依赖性抗伤害作用,此作用受ATP敏感型钾离子通道活性调节.     

Analgesic and antipyretic effects of Ecballium elaterium (L.) A. Richard. Extract in rodents

The dried (elaterium) or fresh juice from Ecballium elaterium fruit is used in the treatment of various inflammatory conditions in popular medicine. In this study the elaterium is examined for its analgesic and antipyretic effects in several animal models. The elaterium exhibits both analgesic and antipyretic activities.     

辽宁产东亚钳蝎毒的镇痛作用——离体海马脑片的细胞内电位记录

应用离体脑片技术及细胞内生物电记录方法，研究辽宁产东亚钳蝎毒及某些活性物质对海马ＣＡ１区痛敏神经元的影响，以了解蝎毒的作用机制，在海马ＣＡ１区记录到细胞内放电５４个单位，其静息电位平均值为４５±５ｍＶ，当给予海马伞入口处一定强度刺激时，在海马ＣＡ１区记录到细胞内放电，向海马脑片灌注蝎毒液时，ＣＡ１区细胞内放电受抑制，说明蝎毒液有镇痛作用     

Controlled Release of Substituted Benzole and Naphthoic Acids Using Carbopol ® Gels: Measurement of Drug Concentration Profiles and Correlation to Release Rate Kinetics

Purpose. The objective of this study is to correlate drug release mechanism with measured drug concentration profiles in gel layers of Carbopol^® matrices containing mesalamine or benzoic acid. Methods. Release rate experiments with Carbopol^® matrices were performed using a rotating disk apparatus. Matrices were frozen and the gel layer in the matrices was sliced using a microtome in a cryostat. Drug concentration profiles were determined by direct measurement of the concentration of the drug in the gel slices. The pH of the slices was measured using microelectrodes, and water content was measured by Karl Fisher titration. Results. The concentration gradient in mesalamine matrices decreased over time and correlated with square root of time release rate kinetics. The concentration profiles of benzoic acid were unchanged over time and correlated with zero order release rate kinetics. Carbopol gel layers were highly hydrated (93–95% water). Gel layers in matrices with mesalamine had a more alkaline microenvironmental pH. This higher pH resulted in increased growth of the thickness of the gel layer and a reduction drug diffusivity in comparison to benzoic acid matrices. Conclusions. The release rate kinetics of mesalamine and benzoic acid correlated to the measured concentration profiles. The shape of the concentration profiles is determined by the rate of growth of the Carbopol^® gel layer and drug diffusivity.     

Muscle spasm induced sympathetic reflex bursts on microneurography in a case with pontine demyelination

Microneurography was performed in a 39-year-old woman with demyelination of the pontine white matter associated with muscle spasms in the lower extremities. Single bursts on the microneurogram were observed immediately after cessation of the spasm with no systemic changes in the blood pressure or heart rate. Voluntary tonic flexion of the lower extremities induced similar bursts with small amplitudes. These reflex bursts possessed a characteristic of muscle sympathetic nerve activity, because the latency between the peak of each burst and the prior R-wave on the electrocardiograph was constant. The occurrence of these bursts suggests that a segmental compensatory mechanism in the spinal cord may stabilize the muscle blood flow influenced by muscle contraction.

Modifying infant stress responses to major surgery: spinal vs extradural vs opioid analgesia

Twenty-six infants due to undergo major abdominal or thoracic surgery under general anaesthesia were randomized to receive additional analgesia with group A) spinal/epidural analgesia; B) epidural analgesia or C) opioid analgesia with fentanyl. We wished to determine if spinal analgesia followed by epidural analgesia might result in more complete control of cardiovascular or stress responses than the other two treatment groups. Heart rate and blood pressure were recorded at five min intervals throughout surgery. Blood samples were taken for measurement of catecholamines and whole blood sugar on induction, 45 min after skin incision and at the end of surgery. Heart rate rose significantly at the start of surgery in groups B and C but not group A. Systolic blood pressures were higher in group C compared to A and B. The rise in plasma glucose concentrations was significantly different between the groups in the order C>B>A (P<0·05). A similar trend was seen in the plasma adrenaline and noradrenaline concentrations but this failed to achieve significance due to the limited sample size.
