传染性非典型肺炎患者泪液和血液中特异性抗体检测的对比研究

目的：通过传染性非典型肺炎(SARS)患者泪液病毒特异性抗体的检测，寻找SARS病毒是否通过泪液或眼结膜传播的理论依据，了解泪液和血液中SARS病毒特异性抗体检测结果的一致性程度。方法：用酶联免疫吸附法(ELISA)检测18例不同病程的SARS患者泪波及血清SARS病毒特异性抗体。结果：18例SARS患者泪液中的特异性IgM类和IgG类抗体均为阴性，而同组患者有13例其血液中的特异性IgM类和IgG类抗体呈阳性。结论：SARS病毒可能不通过泪液或结膜传播。     

Postpartum hemorrhage today: ICM/FIGO initiative 2004-2006.

Postpartum hemorrhage (PPH) is the main cause of maternal mortality. Yet, even though solutions have been identified, governments and donor countries have been slow to implement programs to contain the problem. While poverty and low educational level remain the underlying cause of PPH, the current literature suggests that active management of the third stage of labor can prevent it. The International Confederation of Midwives (ICM) and the International Federation of Gynecology and Obstetrics (FIGO) are attempting to address the chronic PPH crisis by educating their members on best practices and on troubleshooting where resources are inadequate. Some studies found oxytocin to be preferable to misoprostol in settings where active management is the norm. However, secondary clinical effects may prove more troublesome with oxytocin than with misoprostol, and misoprostol may prove to be more practical and equally effective in low-resource settings. Two new interventions are also proposed, the anti-shock garment and the balloon tamponade.     

支气管动脉灌注化疗联合直线加速器放射治疗Ⅲa期非小细胞肺癌

目的　研究支气管动脉灌注化疗联合直线加速器放射治疗Ⅲa期非小细胞肺癌 (NSCLC)的可行性及临床价值。方法　 76例NSCLC患者随机分成A、B 2组 ,A组先行 2次支气管动脉灌注化疗 (BAI) ,第 2次BAI 1～ 2周后再行直线加速器放射治疗 (RT) ;B组单纯行 2次BAI (对照组 )。结果　临床疗效 ,A组 (BAI RT)和B组 (BAI)分别为 89.47%和 60 .5 3% (Ρ <0 .0 1) ;1、3年生存率 ,A、B组分别为 81.5 8%、5 0 .0 0 %和 60 .5 3%、2 1.0 5 % ( 0 .0 1<Ρ <0 .0 5 )。结论　支气管动脉灌注化疗联合直线加速器放射治疗Ⅲa期非小细胞肺癌的临床疗效和患者 1、3年生存率均显著提高     

201例成人急性白血病诱导缓解—四种联合化疗方案比较

成人ＡＮＬＬ初治患者６１例用ＨＡ方案和ＤＡ方案治疗的效果相似（包括ＣＲ率，中位缓解时间），而ＨＡ方案的心脏毒性及并发严重感染率均较ＤＡ方案少见和轻微（Ｐ＜０．０１）。成人ＡＮＬＬ１４０例用ＶＭＰ或ＶＤＰ方案治疗的效果相仿，而ＶＭＰ方案治疗骨髓抑制率及并发严重感染率较少见和轻微（Ｐ＜０．０５）。结果表明，ＨＡ或ＶＭＰ方案对白血病细胞均有明显的杀伤，可以分别替代ＤＡ和ＶＤＰ方案，具有实用价值；ＨＡ和Ｈ     

Amiodarone-induced experimental acute neuropathy in rats.

Amiodarone was injected endoneurially at increasing doses into the exposed tibial nerve of rats to study its electrophysiologic and pathologic effects on peripheral nerve fibers. Forty-five male Wistar rats were used, and each of the following concentrations was injected into 15 nerves: 25 micrograms/mL, 50 micrograms/mL, and 100 micrograms/mL. Microinjection of a 25 micrograms/mL concentration of amiodarone resulted in a subacute, incomplete conduction block evident at day 3 postinjection. This conduction block remained stable until day 10 and recovery was complete at day 35. Microinjection of a 50 micrograms/mL concentration of amiodarone produced a faster evolving conduction block, and significant axon degeneration (approximately 40% of fibers). Injection of a 100 micrograms/mL concentration resulted in severe acute motor axon degeneration followed by complete but delayed regeneration. Results of morphological studies closely correlated with electrophysiological findings. Amiodarone thus seems to have a direct toxic effect on axons at high concentrations in the peripheral nerve, and we suggest that different pathological changes described in human amiodarone neuropathy could be related to different concentrations of the drug in the nerve, perhaps due to variability of blood-nerve barrier efficacy.     

高原环境对部队适应人群内分泌系统的急性效应

本文对初入海拔5170米高原5日内未发生急性高山病的46名新战士进行了17项内分泌指标的检测,发现高原环境对适应人群内分泌系统的急性效应主要有:下丘脑一垂体应激反应增强;甲状腺功能增强;皮质醇分泌增多,醛固酮分泌减少,肾素活性降低,血管紧张素Ⅱ增多;性腺功能无明显变化。     

围手术期肠外营养支持在胃癌根治术的应用

1986～1988年的胃癌扩大根治术221例及根治性全胃切除术19例非肠外营养(N-TPN)支持为对照组,与1991～1993年的198例及61例施行TPN治疗为实验组进行对比。N-TPN组术后并发症总发生率为7.2％,TPN组则为2.0％;全胃切除术并发症发生率分别为31.6％和4.9％(P<0.01)。吻合口瘘发生率N-TPN组分别为2.7％和10.5％,而TPN组则均为0.0％(P<0.01)。膈下脓肿发生率N-TPN组为31.6％和15.8％,而TPN组则为1.5％和3.3％(P<0.05)。但本研究是回顾性的,所以要有待进行前瞻性的、对照的、随机的大范围研究。
