天麻地黄钩藤汤联合卡托普利治疗阴虚阳亢型高血压随机平行对照研究

[目的]观察天麻地黄钩藤汤联合卡托普利治疗阴虚阳亢型高血压疗效。[方法]使用随机平行对照方法,将78例门诊患者按抽签法随机分为两组。注意饮食,忌烟酒、辛辣刺激食物,注意饮食卫生及规律。对照组39例卡托普利,12.5mg/次,3次/d。治疗组39例补虚泻实汤(干地黄、炒白芍、钩藤、丹参各15g,丹皮、菊花、女贞子、枸杞、天麻、罗布麻各12g,黄柏10g,知母8g),1剂/d,水煎200m L,早晚分服。连续治疗28d为1疗程。观测临床症状、证候改善率、不良反应。治疗1疗程,判定疗效。[结果]治疗组显效30例,有效8例,无效1例,总有效率97.43%。对照组显效20例,有效12例,无效7例,总有效率82.05%。治疗组疗效优于对照组(P0.05)。证候改善率治疗组优于对照组(P0.05)。[结论]天麻地黄钩藤汤联合卡托普利治疗阴虚阳亢型高血压效果显著,值得推广。     

天麻退化机制及其防治技术体系的研究思路与方法

天麻是我国传统名贵中药材,近年来,由于过度采挖,野生天麻濒临灭绝,而人工栽培天麻退化严重,品质低劣,产量下降。综述了天麻与蜜环菌的共生关系及其退化机制,从天麻种质、繁殖技术、播种方式,尤其是蜜环菌的优选等方面提出了解决天麻退化的防治技术体系,为天麻的优质高产栽培提供有效可行的研究思路与方法。     

天麻多糖提取工艺的研究

目的优化从天麻中提取多糖的工艺。方法采用正交试验法,以天麻提取液中多糖含量为考察指标,研究提取温度、提取时间、料水比对天麻多糖提取工艺的影响。结果各因素影响天麻多糖提取工艺的重要程度依次为提取温度、提取时间、料水比。最佳提取工艺参数为提取温度100℃,提取时间4h,料水比1：40。结论该提取工艺方法稳定、简便、可行。     

商品天麻中二氧化硫残留量的研究

目的对不同产地规格的天麻中二氧化硫残留量进行检测,为天麻的用药安全提供实验依据。方法采用盐酸副玫瑰苯胺法对不同产地规格天麻进行二氧化硫残留量的测定。结果除个别产地规格的天麻外,所检测的天麻中二氧化硫残留量均未超标。结论本研究可对天麻的用药安全性评价提供参考。     

Tuberous sclerosis complex: everything old is new again

Tuberous Sclerosis Complex (TSC) is a multiorgan genetic disease caused by loss of function of either the TSC1 (encodes hamartin) or TSC2 (encodes tuberin) genes. Patients with TSC have benign tumors (hamartomas) in multiple organs though brain involvement is typically the most disabling aspect of the disease as very high rates of neurodevelopmental disorders are seen. While first described well over 120 years ago, recent advances have transformed TSC into a prototypical disorder that exemplifies the methods and potential of molecular medicine. This review will detail historical aspects of TSC and its strong associations with neurodevelopmental disorders focusing on epilepsy and autism. Finally, promising new approaches for the treatment of epilepsy and autism in patients with TSC as well as those in the general population will be discussed.     

天麻饮片的HPLC指纹图谱鉴别

目的:研究建立天麻饮片的HPLC指纹图谱鉴别方法。方法:选择HPLC-UV指纹图谱法,Kromasil C18柱;流动相乙腈-0.5%冰醋酸溶液线形梯度洗脱,检测波长270 nm,流速1.0 mL.min-1。以指纹图谱软件计算相似度和图谱比较,以聚类分析比较不同来源饮片的差异。结果:建立了天麻饮片的标准指纹图谱,6个产地共10批次天麻饮片的指纹图谱基本一致,共能检出15个共有峰,10个样品的平均相似度>98%,但相对峰面积有较大差异;聚类分析结果与饮片产地基本一致。结论:该方法重复性好,结合聚类分析可用于天麻饮片的定性鉴别,为天麻饮片的质量控制提供了科学依据。     

Glioma-like proliferation within tissues excised as tubers in patients with tuberous sclerosis complex

We describe diffuse glioma-like infiltrates in excised tubers in five out of forty Tuberous sclerosis complex (TSC) patients undergoing excision of a tuber at our institution within the last 10 years. All patients presented with refractory seizures. Resection specimens from four patients had the pathognomonic histologic features of neuroglial hamartomas (tubers) and in one case there was cortical microdysgenesis lacking cells typical of TSC. All lesions were associated with an infiltrate of atypical, mostly elongate, glioma-like small cells, which were immunoreactive for GFAP in three, and pS6 (a marker for activity of the mTOR pathway), in two cases. MAP-2 and CD34, were negative and MIB-1 (Ki67) immunostains ranged from <1-21%. Array-based comparative genomic hybridization revealed that these proliferative phenomena were associated with 21 different copy number aberrations in comparison with a tuber without atypical infiltrates. Postoperatively (follow-up period ranging from 8 to 34 months) none of the patients have any evidence of a glioma. We report that tubers resected for treatment of seizures are sometimes associated with glioma-like lesions, which are indistinguishable from infiltrating gliomas by morphology and immunohistochemistry. Genomic analysis with SNP arrays revealed copy number changes which may be associated with the pathogenesis of such infiltrates.

天麻Gastrodia elata是我国名贵中药,具有镇静安神、抗抑郁、抗焦虑、抗氧化、防病毒和抗肿瘤等多种药理学活性。主要对天麻抗抑郁作用的药效物质基础及抗抑郁机制研究进展进行归纳总结,并结合当前天麻抗抑郁在临床上的应用,为天麻的进一步研究和开发提供参考。
